Employee was walking to the back of the roof bolter when a piece of rock fell from the mine roof, knocking them to the ground and causing a laceration above their left elbow. Employee received 10 sutures to close the wound and was given a full release to return to work on 9/10/2025. No Days Lost. Medical Treatment Only.

Employee was struck by a large section of top rock while kneeling beside a continuous miner during troubleshooting. The rock fell without warning, pinning EE's right leg and fracturing EE's fibula.

Employee was bolting in # 1 Entry when a rock measuring 3 ft. long x 1 1/2 ft. wide x 1-2 inches thick, fell from the mine roof striking employee on the left bicep causing a laceration. Employee received 8 sutures to close the wound and was given a full release with no restrictions to return to work on next regularly scheduled shift. No Days Lost

Employee was struck on the left forearm by a falling rock (10" long x 16" wide x 0.25"-1" thick) from the mine roof. The incident caused a laceration, requiring 10 sutures. The employee received a full release with no restrictions and returned to work on the next scheduled shift (7/25/2024) performing normal duties. No Days Lost

Employee was walking to the mantrip at the end of the shift when a rock measuring 3 feet, 6 inches wide and 2 inches thick, fell from the mine roof striking EE on the top of the head, back, and landed on EE's left foot / heel. Employee sought medical attention and was taken off work for 6 weeks due to pain in EE's foot / heel. This is being claimed as a Lost Work Days Incident.

Employee was bolting in 3 Left Face, when a rock measuring 3'x1' slid down the ATRS of the roof bolting machine striking employee in the left rib area. Employee finished shift & returned to work on next shift. On Tuesday, December 26, 2023, employee sought medical attention & was diagnosed with a spleen bleed & had surgery that day. This is now being claimed as Lost Workdays.

Employee was struck by a piece of rock measuring approximately 4' long x 4' wide x 2" thick on the left shoulder and causing a laceration on the right wrist. Employee received 3 sutures to close the wound on the wrist. Employee was given a full release and returned to work on employee's next regular scheduled shift (12/23/2022). This is a medical reportable only report.

Injured employee was adjusting a hose when the employee was struck on right leg by a rock measuring approximately 20" in length by 15" in width by 6" in thickness. Based on the results of an MRI that the operator received on 9/10/2020 it was determined that the employee will need surgery to repair ACL. This is now being reported as a lost time incident.

Employee was struck by a piece measuring approximately 5' in length X 2.5' in width X 8.5" in thickness tapering down to 0" while bolting. Incident resulted in a laceration and fracture to the employee's left hand middle finger that required surgery.

Injured employee was installing a metal roof strap when drill canopy came into contact with rib loosening the rib causing it to strike EE's back and lower left leg. EE finished the shift but sought medical treatment on 8/5/2019 and was diagnosed with a left fibula fracture and placed off work.

Employee was installing roof support in the number 5 entry when the employee was struck in the left shoulder with a rock measuring 76" in length by 24" in width by 2" in thickness. Employee sought medical treatment and was referred to a specialist. MRI results were made available to the operator on 7/9/2019 at which time this became a LOST TIME incident.

Employee reported ee place hand on the canopy of the S/C when a piece of rock fell striking the employee on the left ring finger causing a laceration. Employee sought medical attention after shift where ee received two stitches. This incident being reported as medical reportable due to getting sutures.

The employee stated was riding # 5 EMU down the track when a draw rock struck the employee on right shoulder, the piece of draw rock was 1/2"x2"x1' long. The employee sought medical attention on 8/30/2017 and was taken off work. This incident didn't become reportable until 8/30/2017. This incident will be a lost time incident.

Employee stated was operating the # 954 diesel mantrip when a rock struck employee measuring 3.5" wide x 1/2" thick and 6" long in the nose area causing a laceration. The employee finished shift, but later sought medical attention where the laceration took 6 sutures. This is being reported as a medical reportable incident.

On 10/10/2016 employee stated was working on the tracking and communication system in the # 6 entry, while walking out of the # 6 entry employee stated a rock measuring 1' 6" x 1' 6" x 3" thick fell on right shoulder. Employee sought medical attention and was taken off work. This incident is being reported as a lost time injury.

The employee was sitting on the bolter eating when a piece of rock fell out between the bolts striking right hand causing a laceration to right index finger. The laceration required 6 sutures and the employee returned to work on next scheduled shift.

The employee stated that they were operating the roof bolt machine and was starting to drill a hole, looked up when a piece of draw rock fell and struck front tooth. The piece of rock broke off one of the permanent teeth. The employee did not seek medical attention, however this is being reported due to the loss of a permanent tooth per Part 50 requirements.

The employee stated that EE was spot bolting on the 2 South track at break 40. While trying to position EE's plate and pizza pan over the tracking cables a piece of draw rock fell and pinched EE's left thumb against the mast. The laceration required 4 sutures and EE returned to work at EE's normal occupation.
