Showing 50 of 279Based on audiometric evidence and review on March 13, 2026, the employee meets the reportable hearing loss threshold under 30 CFR Part 62. The age-corrected Standard Threshold Shift was 25.6 dB in the right ear, exceeding the 25 dB average at 2000, 3000, and 4000 Hz from the employee's baseline/revised baseline audiogram. All 30 CFR Part 62 requirements were followed.
Employee was pulling ventilation curtain from beneath the continuous miner pan when the tripped over a hydraulic oil can concealed under the curtain and twisted a foot/ankle. Employee was diagnosed with a Jones fracture of the left 5th metatarsal and underwent surgical repair on January 23, 2026. The employee is expected to remain off work until April 17, 2026 (Lost Work Days).

Employee was bolting in the #1 Entry when, while checking hole depth with a 6-ft., 0.804 diameter bolt, it slipped from left hand and smashed right ring finger between the bolt head and drill chuck. Employee self-transported to CAMC General Hospital, received four sutures, and returned to full duty on 11/13/2025.
Employee was walking down #1 Entry when EE tripped over gob on the mine floor, fell toward the rib, and struck EE's right fifth digit on an installed rib bolt, causing a laceration. EE returned to work, sought medical attention after shift, received four sutures, and returned on 10/08/2025 to normal duties. Medical Treatment Only.
Employee was lifting short pieces of beam to load them in the back of a truck. On the last beam they twisted after they picked it up, causing them to strain their lower back. Employee went to doctor the next day and was told to rest their back every 40 minutes. Employee has been placed on modified duty.
An unintentional roof fall occurred as the 6 Left Entry was connected into the # 5 Entry, Break # 74. MSHA and WV State officials have been notified and are on-site conducting their investigation. No accidents or injuries occurred from the incident.
Employee was walking to the back of the roof bolter when a piece of rock fell from the mine roof, knocking them to the ground and causing a laceration above their left elbow. Employee received 10 sutures to close the wound and was given a full release to return to work on 9/10/2025. No Days Lost. Medical Treatment Only.
Employee was scooping block, the scoop canopy struck the rib, dislodging material that fell into the operator's deck and hit the employee's left elbow, causing a laceration. Employee was bolting in the #4 Left Entry when a piece of rock fell from the mine roof striking EE on the right hand. Employee was diagnosed with a laceration of the right 5th distal finger (pinky) and received 6 sutures to close the wound. Employee was given release and return to work on 8/26/2025 performing EE's normal work duties.
Employee was loading trash into an insert when a sharp piece of a torn-up pizza pan, that was hidden from view in the trash, struck EE's left forearm causing a laceration. Employee received 8 sutures to close the wound and was given a full release with no restrictions. Medical Treatment Only.
Employee was changing a battery in rubber-tired ride when EE dropped the new battery in place, battery acid splashed in EE's right eye. Eye was flushed, EE was diagnosed with a corneal abrasion, and employee received prescription eye drops. Medical Treatment Only.
The Winifrede Slope Hoist display screen located in the hoist drum building was to be non-operational for over 30 minutes. Electricians are currently troubleshooting on-site and conducting necessary repairs. No accidents or entrapments have resulted from this issue, and miners are not riding the Hoist Car until repairs are made. This is not an emergency escapeway.
An unintentional roof fall was discovered at the Winchester Peerless Rachel Mine, Sydney Eagle Mains on the #3 Section at #64 Break in the #3 Intersection, spad #13368. The area was dangered off. MSHA and State Officials have been notified. No injuries or entrapments. No ventilation disruption and no travel ways have been blocked. The fall is approx. 10-12 ft thick and 19 ft wide.
The electronic tracking system became inoperative at approximately 8:00 p.m. on 6/16/2025 from 9 North to the #2 Section in the Track and Intake Entries. Manual tracking procedures are in place, and troubleshooting efforts are underway to resolve the issue.
Employee was struck by a large section of top rock while kneeling beside a continuous miner during troubleshooting. The rock fell without warning, pinning EE's right leg and fracturing EE's fibula.
Employee at 2 East Belt take-up found rope fouled in sheave. While using 3/4-ton chain ratchet on roof bolt plate, the plate eye broke, causing a fall and left knee injury. Later diagnosed with torn ACL. Employee continued normal duties until 8/26/2025, when they went off for surgery. First missed shift 8/26/2025. Claimed as NFDL.

At approximately 9:00 a.m., the Winifrede Slope Hoist was found to be non-operational, displaying a drum fault condition for over 30 minutes. Electricians are currently troubleshooting on-site and conducting necessary repairs. No accidents or entrapments have resulted from this issue, and personnel can safely enter and exit the mine. This is not an emergency escapeway.
EE rolled left ankle while dismounting the Co. #402 roof bolting machine in the #5 entry due to an unseen 4"x4" hole in the mine floor. EE reported the incident later in the shift and continued working. That night they sought medical attention and was diagnosed with a sprained ankle. EE was fully released to work on 02/12/2025.
An unintentional roof fall was discovered at the Winchester Peerless Rachel Mine, 2nd Left Panel at #4 Brk, between #6 and #7 Entry, spad #'s 12654 and 12655. The area has been dangered off. MSHA and State Officials have been notified. No injuries or entrapments. No ventilation disruption and no travel ways have been blocked. The fall is approx. 5-6 ft. thick and 18 ft. wide.
An unintentional roof fall was discovered in the track entry on 6 North Mains, Entry # 6, Break 15, 2 Breaks outby spad # 4837. MSHA and WVMHST Officials have been notified. No injuries or entrapments occurred. Ventilation is not disrupted, and travel is not impeded. This location is in old works and is not a primary or secondary escapeway.
Employee stated that EE struck EE's right forearm and right elbow on a slate bar while positioning a trickle duster. EE continued working EE's full shift. The employee continued to work, completing EE's normal work duties until 2/13/25, when EE began missing work.
Employee was assisting a roof bolter move from Entry #6 to Entry #5 when the bolter slid into a hole in the mine floor causing its rear to drop onto the employee's left foot, catching it between the bolter and mine floor. Employee was transported to a hospital, where employee was diagnosed with two fractures and two dislocated toes in left foot. This is a Lost Workdays Incident.
An unintentional roof fall was discovered at the Winchester Peerless Rachel Mine, 1st Left Panel off 2 West Submains at 9 Break #8 Entry. The fall is approximately 20 ft. long and 7 ft. thick. The fall has been dangered off and MSHA and WVOMHST officials have been notified. No injuries or entrapments occurred from this incident. The fall does not impede ventilation controls.
EE was turning 6-inch aluminum waterline on when incident occurred, the waterline unexpectedly burst. The waterline struck EE in the back of the head. EE received six staples in the back of the head. EE was given a full medical release without restrictions, allowing EE to return to EE's regularly scheduled shift. No days lost
Employee was bolting in # 1 Entry when a rock measuring 3 ft. long x 1 1/2 ft. wide x 1-2 inches thick, fell from the mine roof striking employee on the left bicep causing a laceration. Employee received 8 sutures to close the wound and was given a full release with no restrictions to return to work on next regularly scheduled shift. No Days Lost
Employee was cutting sprocket chain that was fouled on the offside drive axle to install a new intermediate shaft when the chain flew apart as it was being cut. The chain struck the employee on the left middle finger, causing a laceration. Employee received 6 sutures to close the wound and was given a full release to return to work on next scheduled shift. No Days Lost
Employee was using utility knife to skin back outer jacket of lifeline to connect to mine phone when the knife slipped, causing EE to lacerate their left thumb. Employee sought medical attention where EE received 3 sutures to close the wound. Employee was given a full release with no restrictions to return to work on the next shift, performing EE's normal duties. No Days Lost.
An unintentional roof fall was discovered in the Primary Escapeway on 7 North Belt Intake, Entry # 6 between breaks 4-5. The crew has withdrawn outby the affected area and is currently dangering off the fall and re-routing the lifeline. MSHA and State Officials have been notified. No injuries or entrapments occurred from this incident. The fall is approx. 10' high and 50' long.
An unintentional roof fall was discovered at the Sydney Eagle Portal, Joe's Creek Mains between 14-15 Break. The fall is approx. 40 ft. long and 6 ft. thick. The Fall has been Danagered Off and MSHA and WVMHST Officials were notified. No injuries or entrapments occurred from this incident. The fall does not impede ventilation or travel.
Wet, muddy conditions on slope. Employee was leveling a railroad track and was removing the jack used for leveling. When the employee released pressure on jack the bar slipped and contacted EE's left arm causing a laceration
The Sydney Eagle Portal personnel elevator had a malfunction of the door switch, making the elevator inoperable for more than 30 minutes. MSHA and State Officials were notified and have been on-site investigating the event. NOTE: The elevator has been checked by MSHA and State Officials and is now operational. No entrapments or injuries occurred from this event.
Employee was retrieving a chain from the supply motor. As EE pulled, the chain entangled with other tools. When EE jerked it free, it flew back and struck EE on the bottom lip, causing a laceration. EE received 5 sutures and was given a full release with no restrictions. EE returned to work on 8/7/2024, performing EE's normal duties. No Days Lost

An unintentional roof fall was discovered at approx. 9:45 p.m. an called into both MSHA and State Agencies. The fall is in the Right Return on 10 North Belt between 5&6 Entry near spad #10279. MSHA and State Officials have been on site investigation the fall area. No Injuries or entrapments occurred.
Unintentional roof falls were discovered at approx. 6:00 a.m. in the Intake and Right Return of the #1 Section. Adjacent to spad #10241 in the Intake and just inby spad #10284 in the Right Return. Also, in the Intake from spad #10223 to spad #10245. MSHA and State Officials were notified and are on-site investigating. No injuries or entrapments occurred.
An unintentional roof fall was discovered at approx. 8:30 a.m. in the #6 Entry at Break 24 on 10 North Belt Intake just inby spad # 12281 and was reported to MSHA and State Agencies. Both agencies have already been on site and a clean-up plan was submitted and approved. No injuries or entrapments resulted from this incident.
An unintentional roof fall occurred on the # 1 Retreat Mining Section, between the #1 and #2 entries at Brk 14 covering up the Mobile Roof Supports (MRS). The affected are was dangered off and MSHA and State Agencies were notified and have been on site. A MRS retrieval plan was submitted and implemented. No injuries or entrapments resulted from this incident.
Employee was walking down track entry when EE tripped over gob along the mine rib causing EE to fall into the rib. As EE was falling, EE stuck arm out to brace fall causing EE to strike right forearm against a rib bolt plate causing a laceration. EE received 6 sutures to close wound and received a full release to return to work on EE's next scheduled shift on 5/28/2024.
While passing through a mandoor due to an inoperable inby airlock door, the outby airlock opened, causing a sudden air pressure change that made the mandoor slam shut, injuring EE's right thumb. EE visited CAMC Surgery on 5/22/2024 and was diagnosed with a displaced fracture of the distal phalanx. EE was released and returned to work on 5/23/2024, performing EE's normal duties.

MSHA and WVMHST agencies were notified and have been on site to examine the area. A MRS retrieval plan has been submitted and implemented. No injuries or entrapments resulted from this incident.

Employee was walking to the mantrip at the end of the shift when a rock measuring 3 feet, 6 inches wide and 2 inches thick, fell from the mine roof striking EE on the top of the head, back, and landed on EE's left foot / heel. Employee sought medical attention and was taken off work for 6 weeks due to pain in EE's foot / heel. This is being claimed as a Lost Work Days Incident.
Employee was placing tools away in the tool sled when another employee, operating a scoop, had the bucket under the tool sled, accidentally lifting it, causing the sled to fall and injure the employee's right foot. Employee was diagnosed with a fractured foot. This is being claimed as a Lost Workdays Incident.
The employee was climbing a ladder to begin welding. The ladder was erected on a sloped surface with uneven legs. Once EE reached the 4th step, the ladder fell sideways causing EE to fall to the concrete below. The employee sustained a few abrasions and contusions to lower back and rib area. EE missed zero days of work but is still on physical restrictions.
Employee unhooked crossbar of CAB U-Hanger to let shuttle car cable pull down out of it when the cable jerked down and one of the hangers, unexpectedly still attached to the cable, contacted EE's left hand as it jerked down, causing a laceration. Employee received 20 sutures to close the wound. This is a Medical Reportable Incident.
