EE was pulling excess curtain from the mine floor to advance curtain on bolt cycle in the face. A rock measuring approximately six inches wide by seven inches long by three inches thick dislodged from the mine roof and struck ee on left shoulder. EE finished shift but went to the emergency room later in the evening with complaints of shoulder pain.

Employee was loading roof bolts onto the Roof Bolter. While doing so, a piece of mine roof approximately 8" x 8" x 2" fell and struck EE on the right middle finger causing a laceration that required part of the fingernail to have to be removed and 2 stitches to close the wound. The mining height is 8 1/2 feet in this area.

The employee was constructing a stopping at 59cc #1 Section between #7 and #8 entries. The area was 9' 3" high and 18' 8" wide. EE was standing on a ladder to place another block on the stopping. As EE placed the block, a section of rock measuring 24" x 13" x 1" fell out and struck EE on top of the right hand. The injury took 7 stitches to close.

Employee was getting ready to drill a roof bolt hole in the mine roof. A piece of rock measuring approximately 1'L x 1'W x 3"T fell from the mine roof when the drill steel touched the mine roof. This resulted in a laceration to EE's right forearm and took 4 staples to close the wound.

Employee was operating a roof bolter in the intersection of #5 entry on the #2 section when a piece of rock approximately 7"L X 3-5"W X 0-2"T fell from the mine roof and struck on the right forearm causing a laceration that required stitches to close.

EE was operating a remote box for a miner when a piece of rock struck right hand causing a small laceration. On 3/1/2017 during a follow-up visit to Dr. EE was placed on restricted duty to prevent potential infection.

Employee was struck by a small piece of rock on the right foot. Employee suffered a contusion to the top of foot. Employee continued to work until 2/14 when employee went to the ER with a small swollen area. The employee is a diabetic and as a precaution the ER Doctor recommended restricted duty for 3 days. Employee returned to normal duties.
