Employee had just got done fueling the bolter when they heard crumbles hitting their hard hat, they then went to exit the area when a rock fell an estimated 22 feet from roof of the mine hitting their arm. Was taken by ambulance to hospital where x-rays were taken and revealed a broken left forearm.

A piece of rock fell from 25' roof, striking the employee's hardhat and left shoulder, fracturing clavicle and scapula. Trying to flee the area, employee likely twisted an ankle and fell on large rocks, fracturing fibula above right ankle and seven ribs left side. Employee was hospitalized.

Employee requested follow up apt. due to reoccurring pain. Apt. 12/31 with specialist found possible fracture, employee given restriction no use of right arm with a CT scan and follow up apt.
