Employee was using a utility knife to cut nylon straps holding roof bolts. Knife jumped while cutting a strap and cut EE's left leg above the shin bone. Employee received sutures to close the wound.

An operator in a skid steer with an enclosed cab could not get the bucket lowered. To get the bucket lowered another employee was standing on the bucket and jumping on it. While jumping on the bucket EE felt a pop in EE's left achilles.

An operator was driving a haul truck down a heading when the haul truck started to slide on a downhill grade. The truck slid for roughly three headings then overturned in the travel way. The operator was wearing a seat belt and removed from the cab. EE was diagnosed with a hairline fracture on EE's left clavicle

Employee was assisting shoveling a pile - EE fell into the pile and got lime in eyes. EE went to local medical center to get eyes flushed. Treating physician prescribed EE eye drops.

Employee got dust in their eye, scratched an eye and sought medical attention. Went to the eye doctor and was diagnosed with a conjunctival abrasion. There was nothing in the eye but was prescribed antibiotic eye drops.

An employee was observing a drag chain conveyor. EE's left hand was holding a drill with chisel attachment, the chisel slipped, and EE's finger contacted the corner casing of the elevator that carries coal to the conveyor. The emergency department classified this injury as a laceration/open fracture to EE's left-hand ring finger, requiring surgery to close the wound.

Our driver was attempting to unhook hoses from the tractor to the trailer so they could disconnect the tractor from the trailer. Their foot got tangled with the step on the back deck of truck. Although they were holding on to the safety bar, they slipped and fell. They bruised their left side.

Our loader was at the Carbis rack at the mine's surface location attempting to open the lid on a pneumatic tank trailer. When they pulled on our latch, they struggled with getting the latch to move. They pulled too hard causing a strain to their groin area. This caused a hernia to their groin area. They will need to have surgery.

Employee was welding a patch on the enhanced hydrate blow down line. EE stated EE got something in EE's right eye, and thought it was slag from chipping EE's weld. EE washed out EE's eyes and used eye drops on site before going to a local medical center. At the med center the doctor used a Q-tip to remove a spec from EE's eye and prescribed EE eye drops for a scratch on EE's cornea

Employee was holding onto railcar gate opener "rail wrench", while it was closing the railcar gate. The "rail wrench" vibrates when closes the lids, during this they felt a pop in their left bicep.

Employee was replacing a take up on a conveyor belt when, while doing so EE slipped resulting in a loss of balance and EE's arm impacted a guard adjacent to the belt. Went to doctor and was released to full duty that day. Followed up with doctor and fluid containing blood was drained from hematoma. Received doctors' notes on 12/4/23.

Employee was moving railcars and was very hot due to the high temperatures outside (90 F+). EE realized EE stopped sweating, which EE recognized as a sign of potential heat stroke and told EE's coworker EE needed to get out of the sun. Coworker helped get EE inside to air-conditioned break room. EE was taken to Medical Center via ambulance where EE received multiple IV's.

Employee had just got done fueling the bolter when they heard crumbles hitting their hard hat, they then went to exit the area when a rock fell an estimated 22 feet from roof of the mine hitting their arm. Was taken by ambulance to hospital where x-rays were taken and revealed a broken left forearm.

EE was trouble shooting high differential pressure on a dust collector. When EE entered the collector EE found a blank with a punch plate instead of a bag. EE used a pair of plyers to try and free the punch plate from the blank, while doing so EE's plyers slipped and hit EE in the mouth breaking a tooth and chipping another

Employee was using a bar working on unplugging a pipe. While working, employee's hand struck employee's helmet-mounted earmuffs and knocked hard hat and safety glasses partially off face. At the same time, an employee one floor above had opened an inspection door, and vibration caused ground lime to fall through grate flooring and into the lower employee's eye.

The employee was loading trailers with another employee. EE told co-worker that didn't feel well. When EE was in the passenger seat of the tractor, EE passed out and fell over. EE's co-worker sought help from lime mine employees because EE couldn't move EE. EE was in cardiac arrest.

Employee was replacing guard on return roller of 196 belt. The right hand side of the guard fell off the pin and the guard swung to the left, pinching EE's left hand between guard and frame. Laceration to left index finger which required 5 stitches.

Employee was attempting to knock debris loose from from a dump chute located above a vibrating feeder on the no.8 kiln. The debris inside broke loose creating a large cloud of lime dust that surrounded the employee. This got into both eyes. Employee was wearing safety glasses with dust seal and respirator which were current requirement.

Employee reportedly lost consciousness while driving work truck and crashed into a concrete barrier. Employee wasn't injured from the wreck. A non-emergency ambulance was called to come to the site and check the employee out. The ambulance arrived at 1:30 pm and conducted some tests on the employee, employee's family took them to the hospital for further evaluation.

The employee was cutting angle iron with a 4.5" grinder. Employee shut the grinder off but, with the wheel decelerating they set it down and it made contact with the back of their hand causing a laceration wound.

At approximately 3:40 PM, the employee contacted the foreman to tell the foreman the employee wasn't feeling well and had chest pains. The foreman brought the employee out of the mine and the Mine Superintendent transported the employee to the hospital. An examination and tests were performed at the hospital and the employee was released.

Hose on tire machine disconnected suddenly when employee was pulling; struck back of left hand on frame of machine. First aid applied to middle knuckle laceration initially; one week later noted limited finger movement, physician diagnosed tendon injury requiring surgery, making injury reportable 5/5/22.

Employee entered area where blasting occurred <15 minutes prior to inspect shot. A piece of rock fell from 25' roof, striking the employee's hardhat and left shoulder, fracturing clavicle and scapula. Trying to flee the area, employee likely twisted an ankle and fell on large rocks, fracturing fibula above right ankle and seven ribs left side. Employee was hospitalized.

Scale ring in Kiln 7 broke apart and created ash dust, which entered the cooler. The hot dust flowed through the cooler belt feeder and onto the cooler belt, which ignited due to the high temperature. Damage was limited to the cooler belt, head pulley, some rollers, and nearby electrical wires/cables.

Tramming Fletcher Scaler and the tram motor went out causing it to free wheel downhill. Miner applied manual break. Scaler traveled 10-20 ft. and hit rib. On impact, miners leg slipped off break and hit the bracket holding the fire suppression system in cab causing a laceration.

Drive belts on Metso vibratory screen were improperly tensioned, which either caused or was caused by the screen being overloaded. This caused the belts to slip and catch on fire, which ignited the screen dust cover. Damage was limited to drive belts and dust cover.

Miner complained of chest pains while driving truck through the portal on way to surface office. Forman contacted and ambulance called for care. Miner was transported to hospital. Returned to work next day OK, pains believed to be caused by acid reflux, pollen allergies and high blood pressure.

Employee was washing a small glass cup used in an instrument to test lime quality. The bottom of the cup cracked and fell out, employee cut hand on sharp edge.

4:30am flames observed on kiln 6 guard. Employees used garden type hose used for wash down located near site of the fire, reduced it to point where the guard could be opened. Once opened, fire extinguisher used to extinguish existing flames by 5:00 am. Oil level was above operating level for the reservoir causing it to drip off gear and onto hot kiln shell where it ignited.

11/9, EE was opening a dribble chute on K6 preheater. Chute opened more quickly than expected. Momentum of the handle caused EE's hand to be caught between the handle and a dumpster. It did not hurt much initially but the next day it was worse, EE went to the ER and was diagnosed with a fractured second right metacarpal. Reported to company on 11/12.

Employee unloading 50-lb bags of ANFO. After unloading 3-4 bags, employee felt a "pop" in EE's shoulder then unloaded the rest of the bags (47). Reported to supervisor 30 min later. Orthopedic evaluation and X-ray next day confirmed nothing broken or dislocated. Employee given restriction, no pushing/pulling/lifting greater than 5 lbs with left arm. Follow up after MRI.

Employee was steam cleaning haul truck (H-14) at the steam jenny. Climbing down off of truck and foot slipped. Was holding wand in one hand. Fell from 2nd rung from the top landing on the ground hitting the left side of EE's body. ER visit confirmed nothing broken. Put on work restriction due to soreness and limited motion.

At approximately 510 PM there was a radio call that employee was having seizure. It was break time, employee was at the UG office. 911 was called. Foreman responded with the emergency truck and employee was brought out of the mine to surface mine office. The ambulance was directed to surface mine office. Employee was transported to hospital by ambulance.

Employee cleaning floor hole at the face. Small (size unknown) rock fell from roof or face (location unknown), hit EE in the shoulder. Initial x-rays (12/17) at ER did not show fractures. Employee requested follow up apt. due to reoccurring pain. Apt. 12/31 with specialist found possible fracture, employee given restriction no use of right arm with a CT scan and follow up apt.

Lime sat in silo and plugged over the chute. Before starting to load trucks, employee was attempting to unplug discharge chute from Silo 4 by pushing bar into discharge chute. Material gave way, bar pushed into chute and hand hit part of the chute. Contact with chute broke left index finger.

Employee heard on radio gasping for air. Radios have name indictor on them. Employee found in seat of fuel truck struggling to breath and then became unresponsive. First response team found EE had no pulse. CPR and AED was used. Employee taken by ambulance to ER with cardiac arrest. After investigating this appears to be a medical emergency due to natural causes.

Employee was shutting a sliding guard. As EE shut the guard EE pinched wrist between the handles of the guards. The handle of the guard hit into EE's right arm resulting is a fracture in the right wrist.

Oil from the Kiln 6 drive gear sump contacted the kiln shell, exceeding the oil's flashpoint and started a fire. The kiln was shut down for 36 hours to replace the guard, which had buckled due to the heat of the fire.

Employee was using a wrench to remove a swivel fitting. The head of the wrench broke and the wrench came back and hit the employee in the face, cutting EE's nose. EE received 5 stitches.

Employee cleaned out air hood, put it on, piece of lime fell out got into their eye, behind prescription safety glasses. Employee flushed eye, no pain, saw lime in eye. On 1/8/19 they observed lime was still in eye, wanted it removed. 1/9/19 - Went to doc., lime fell out. Prescribed drops to prevent infection.

While inspecting the area of a pipe for a hole that required to be repaired lime fell on the employee as EE was looking up. The lime fell directly on the employees face, getting behind EE's foam lined safety glasses.
