Bolting (and screening) in 1East #1 entry, 17-18 block, in an area with exposed shale. The ATRS was set and he was starting to drill first hole when a rock (18"x24"x5") broke loose outby the drill canopy, swung around and struck him in the lower back. Started missing work on 10/2/15 for contusions. No problems with training, PPE, mining machines or compliance issues.

Employee was bolting off of the continuous miner with the rock deflector up & the rib door extended when a rock (46"x35"x6")fell from in front of the last strap hitting the rock deflector then hitting the rib protector. The rock then came into the bolter station striking EE in the back. It didn't involve job skills, proficiency, training, or attitude. No PPE issues involved.

Bolting off the miner in 2 East while cutting a turn in #1 entry a rock fell, broke on the rib protector and rolled on the back of his left foot resulting in a blood blister and possible soft tissue damage. No problems with training, ppe, mining machines or compliance issues.

While traveling down main line haulage a piece of loose rock/coal came down striking employee in his left eye. (abrasion to the left cornea) This does not involve any compliance issues, no equipment involved. It did not involve any job skills, training, or attitude. Personal safety items were being utilized.

A small section of roof material fell from around the bolt he was installing striking the ee about the head and shoulders. The event became reportable when the employee reported the incident to management on 7-30-2015. The incident occurred because the ee failed to conduct an exam of the roof prior to drilling. The employee was properly trained and had on the appropriate PPE.

Miner operator in 2 East, bumping inby right corner #2 entry at 10 block. Rock brow was left, so the miner was scaling the brow down (1'x3'x4') with a slate bar and the rock bounced off the miner and struck his legs, resulting in a contusion. No problems with PPE, training, mining machines or compliance issues.

EE just finished putting up bolt he walked back to back of miner when rock fell from between straps striking him in the head. Rock measured 4x4x2 & consisted of roof coal & rock. There were no compliance issues. Didn't involve job skills proficiency training or attitude PPE or protective devices on equip. involved. Acc. occurred in 8South Right Section #6 at Surrey Station 81+06

Miner was sitting in a turn from #2 to #3 at 74 block 23 West. Injured was putting in a test bolt underneath a rock brow when he noticed an oil leak on the bolt station. When he turned to yell for the mechanic the rock brow fell and struck him in the neck and right shoulder resulting in minor soft tissue damage. No problem with mining machines, training or PPE.

Employee was looking up while hanging a highline cable when a small piece of rock fell from the mine roof causing dust to go into his right eye. The injury resulted in a corneal abrasion. He was given eye ointment for his injury. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training, or attitude.

Employee was beside of mine checking a pin in the pod jack and a rock fell from the brow and struck right arm. No problems with training, PPE, mining machinery, or compliance issues.

In 23D, cutting the turn for #1 at 60 block, the injured was bolting off the right side of the miner when a 4'x4' rock fell from the top and struck his right humorous resulting in a compound fracture. No problems with PPE, mining machinery, training or compliance issues.

Mining 2-1 at 60 block in 23D the miner operator was preparing to make another sump when a rock (1 1/2' square and 5" thick, approx. 50 pounds) fell from the top (about 3 straps from face) and struck the miner operator in the back of the neck and arm resulting in possible ligament damage. No problems with training, mining machines, PPE or compliance issues.

1 E Sect-Employee was standing by the boom on left side of miner when a rock fell 30"X24"X2" thick struck him on the head, neck, shoulder, & left arm. He stated that he was unconscious for a few seconds. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training or attitude. No PPE or protective devices on equipment involved.

In 23D at 39 block 2 to 1 (track chute) the employee was on the right side of the Joy 14ED25 miner installing the last strap in the entry (after holing). After finishing the drill hole, he lowered the timber jack and a piece of roof rock (18"x12"x1") fell and struck his right elbow causing small lacerations and contusions. No issue with mining methods, compliance, PPE or training.

Injured was carrying rock dust in 23D in crosscut 2-1 at 36.5 block, when a piece of head coal (4' long, 3' wide, 1" to 8" thick) fell and struck the employee knocking him to the ground. Miner sustained contusions to the top of his head and some neck pain. No problems with mining machines, methods, training, PPE, or compliance issues.

Employee was standing along side the miner when a piece of rock fell from the mine roof and struck him on the head, left shoulder, and left arm. It resulted in a contusion. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.

Rock dislodged from the roof while setting bits on the shearer striking the left thumb, trained on job task, wearing proper PPE. received 4 sutures and fracture to the thumb

Employee was rib pinning on the left side of the miner when a rock fell from the mine roof pinching his right middle finger between the rock and the pinner. Employee was scaling down a rock when another rock fell hitting the slate bar and pushed into his Right thigh causing a laceration. His body wasn't properly positioned. Employee was working on the mainline track at 47+00 when a rock fell from the mine roof striking right ankle causing a sprain. A rock fell because it was not scaled down. Employee was handling miner cable when a piece of rock 10"x8" fell from the mine roof striking his left hand causing a fracture. Loose top was not scaled by employee. The injury did not involve any compliance issues. No mining equipment was involved. The accident didn't involve job skills, miner proficiency, training or attitude. No PPE or protective devices on equipment involved.

While bolting off the miner, the employee was struck by a section of roof rock or head coal in the head, left shoulder and back resulting in a contusion. The event became reportable when the employee began seeking medical attention on 11-08-2013 and began missing work.

Individual was bolting on the right side of the miner when top of the roof/rib fell out striking him on the right leg and foot.

Individual was bolting on the Alpine-Voist Miner when a piece of rock fell from the roof / rib line striking the individual on the back shoulder / neck area.

While Operating shear, rock fell between the shields, striking him in the back of the head. He began missing work on 8/1/2013

Individual was lowering a trolley pole that came off the wire when struck on top of the head with a loose rock which knocked him down. Individual continued to work until 8/21/2013 due to pain, doctors removed him from work.

He was running the tail gate end of the shearer when a piece of rock approximately 4 ft by 3 ft and 10 inches thick struck him on his lower left leg causing a fracture.

The injured was installing the last bolt of the cut.The EE let the drill mast down to retrieve the wrench out of the drillpot with his right hand, when he got a hold of the wrench a rock fell from top and caught his little finger between the rock and the wrench. The rock meas. 20" X 20" and 1/4 to 4" thick. Right little finger was amputated just behind his finger nail.

While standing on the right side of the continuous miner in the 19 W face area. A rock fell from the roof striking the left forearm causing a laceration. Individual received 5 sutures.

He was miner bolting on the 16A bleeder section when his drill steel became plugged. He unplugged it and attempted to put it back into the hole when a piece of rock fell from the mine roof pinching his left pinky finger between the rock and the miner. The laceration required 3 stitches.

The injured was installing a roof bolt in the brow of an overcast on 5 North Mains when a piece of rock was dislodged from the overcast. The rock struck the injured in the face, causing a laceration that required 8 sutures in the right cheek, and a small puncture hole through the bottom lip.

He was installing an end bolt off the miner, when he had to stop to fix the suction on the bolter. As he was bent over, a rock fell from the roof and struck him at the base of the neck which resulted in a fracture of the C6 vertebrae process.

The IE and the hauler operators were attempting to extend the ventilation tube slider, when the roof collapsed. The falling roof hit the tubing which knocked the IE down and then the rock hit the EE on the left foot. The falling material cause laceration to back and laceration and crushing fractures to his lt. foot. Rock size 1' - 3' thick and approx. 20' L. X 8' W.

The employee was struck by a small section of head coal which fell from permanently supported top. The material struck the employee about the head, neck and shoulders. This event became reportable when the employee began missing work on June 10, 2011 due to complications (mild concussion) resulting from the incident.

The injured was using a portable Z drill to roof bolt on the face of the 15 Right Longwall Panel, when he started to drill into the mine roof, a piece of rock fell out and struck the injured on top of the head. The injured sustained a fracture of the C1 vertebrae of his neck.

Individual was bolting on the alpine miner on the left side when a small piece of rock (4"x4") fell striking individual on the upper lip. Individual received 3 stitches.

Employee was using a spad gun to install a curtain. A rock dislodged and struck him on the chin.

Two bolter operators were re-bolting an area that was breaking up. They had bolted to a simplex jack. They decided to manually remove the jack. While one bolter started to let the jack down the other bolter pulled back on the mesh. At this time a rock fell out of the roof between the roof bolts striking the bolter pulling the mesh back on the legs causing a fracture.

Roof rock fell while drilling and landed on EE's left foot.

EE was walking main north belt when a piece of coal/rock fell from the roof, striking EE on the left side of the hard hat. EE experienced stiffness to the neck and pain.

EE was bolting on the left side of the Joy miner and finished the bolting cycle. EE walked to the rear of the miner and with no warning the rib (22' long & 13" thick) rolled out striking him in the head area and pinned him against the miner. EE suffered a broken collar bone & a bruised lung.

While hanging the miner cable in a cable hanger, the employee was struck on the head, neck and shoulders by a section of head coal which came from between the roof straps.

The injured was roofbolting on 14 right section and was installing his last roofbolt when a piece of rock fell from the top and hit him on the left side of the face and neck, causing an abrasion and laceration that required 12 sutures to his neck.

Employee stated that he was roof bolting from the miner in 4B 8North section when he bent over to look at a hose connection on the ATRS when a piece of stone approx. 10" x 6" x 1/2" fell striking him on the left shoulder. This resulted in a non displaced fracture of the scapula.

The injured was roof bolting on the 13 right longwall developement section. He was bolting in the 2nd cut out of the left crosscut. As he let the drill head down he grasped the wrench with his right hand. A rock measuring 23" x 24" x 0-2" thick with sharp edges fell approx. 3' striking his right thumb between the web & 1st knuckle causing a laceration requiring 5 stitches.

The injured was operating the offside of the roof bolter on 15rt. when a piece of rock fell from the top and struck him above the ankle of his right foot. Causing a contusion and puncture.

A piece of draw rock fell between roof straps striking EE on the head and shoulder. The EE was treated and released back to work the next shift. EE continued to work until 12/16/09. EE went off work with fluid on the spine, ligament torn from spine at L-1, compression received from the rock fall.

EE was working on 12 right longwall. EE was setting shield #116 half the way in and was walking by it when a rock fell in front of shield #117. The EE had his hand upon the hand rail when the rock struck his hand. The EE broke his right little, ring and middle fingers and also lacerated the palm of his right hand.

EE was walking to underground work area a rock 56" long, 9" wide 2" thick fell from mine roof striking him on the left elbow.
