A piece of stone fell from between the staps in 12 Miner section and struck employee on the top of the head and neck. *** Lost Work days began 4-20-2009 ***

Employee was reaching for a roll of tie wire on top of the miner in 12 I. Piece of stone fell from between the roof straps and hit him on the left hand. ***LTA began 4/29/09

While mining in 12 I section a piece of roof stone fell from between the roof straps and struck employee on the right foot. This caused a laceration to his foot - 8 stitches to close laceration.

Employee was removing the oil plug on top of the shearer when a piece of rock fell from between the shields and struck him on the right hand resulting in a fracture to the right little finger. Employee was wearing gloves

EE was walking back to get a roof strap from rear of miner in #1 Bleeder Entry, E-19 Section, when a piece of rock that measured 18" X 24" X 3" thick fell from between the roof straps and struck him on top of the head, jamming his neck. EE began missing work due to the injury on 3/23/09.

Employee was operating a miner when a piece of stone fell from between the straps and struck him on the left hand.

During a weekly exam run, a piece of stone fell from between the roof straps striking employee on the back of the neck and head. **Lost Work days began 8/28/2008

Employee was scaling loose material from the mine roof with a slate bar in #3 Entry, E-19 Section when a piece of rock 4 1/2" wide, 24" long & 1" thick fell & struck him on the left arm causing a laceration requiring 10 sutures. Employee was wearing long sleeves.

Rock fell and hit EE on the head.

Employee was struck by a piece of roof coal that fell from between the roof straps.

Employee was in the process of placing the tower drill steel into the top steel he was holding with his left hand when a piece of rock 12" x 12" x 6" thick fell from between the roof straps and struck him on the right forearm. Employee sustained a fracture to the right forearm.

Employee was bolting on left side of miner when a slab of rock fell inby the last strap knocking the vent tubing down on top of him causing him to strik his forehead against the miner. Employee sustained a laceration to the forehead requiring 7 sutures.

EE was installing a bolt at corner of #1 to #2 crosscut, F-16 Section when a piece of rock 12" x 12" x 2" thick fell and struck his hard hat forcing his safety glasses down causing a laceration to the left eyebrow requiring 6 sutures.

Employee was making a gas check at the face of #2 Entry, 3 North Left Section when a piece of rock fell from between roof straps and struck him on top of the left shoulder.

Employee was helping to install cable bolts. As he was picking up a bolt, rock fell from between the roof straps striking his right hand.

Lifting a skirt board with a come-a-long when a piece of rock measuring 3' x 2' x 2" fell from the top and struck him on the left ankle, resulting in a fracture to the ankle. (The employee had returned to work on 7-10-07 with 21 days away from work, worked 2 days and began missing work again on 7/12/07 due to the injury)

Employee was drilling on left side of the continuous miner in #3 Entry, E-17 Section when a piece of rock 30" long, 24" wide and 2" thick fell and struck him on top of the left forearm, causing a laceration requiring 9 sutures to close.

Employee was drilling on right side of the continuous miner in #3 Entry, E-17 Section when a piece of rock 22" long, 14" to 4" wide, and 2" thick fell and struck him on top of the right foot, resulting in a fracture to the right foot.

Employee was side bolting in #1 entry, E16 Section when a rock fell knocking ventilation tubes down on top of him causing him to strain his lower back.

Employee was installing cable bolts with the center bolter, after installing a bolt he lowered the atrs and a piece of rock fell from the roof and struck his right cheek causing the laceration.

7I longwall, standing in walkway at #1 shield a piece of rock fell from between straps and struck right ankle.

Injured was driving a wedge on top of jack when a rock 6" x 1-1/2" struck him in face. He has lip lacerations.

Injured was winching scoop through mud hole and rock fell from ceiling bruising his neck and shoulders.

While pulling shields on the F-13 longwall face a rock slid out of the panline and struck employee in the right lower back and he fell onto a shield toe striking his face. Employee sustained a fracture to two ribs.

8I section, using loader to pull up slack cable. He walked back to move cable out of tram road. Rock fell from rib and roof line, striking his left foot, causing fracture to his big toe.

EE was operating the miner in the #1 Entry in the E-Tailgate Section, when a piece of rock fell out from between the straps, striking EE on the right hand causing a laceration requiring 11 sutures.

EE WAS CHANGING AN AUGER BIT ON A DRILL STEEL WHEN A PIECE OF ROCK FELL FROM BETWEEN THE STRAPS, HITTING HIS RIGHT MIDDLE FINGER WHICH WAS ON TOP OF THE AUGER BIT. 10 SUTURES.

Employee was walking along panline on headgate side of shearer between #174-#175 shield F-11 longwall face. He had his right hand on the handrail and a piece of rock, 6" x 10" x 18" fell from between the shields striking him on the right hand, causing a fracture to the right little finger. Employee began missing work on 10-05-05, and returned to work on 10-07-05.

While reaching for a wrench off a rib protector a rock fell striking left hand. Rock was approximately 14" long and 8" wid eand 4" thick. The roof was 9 foot in height. Diagnosed as a contustion.

While planking through the 3-2 x-cut, the miner bolters were installing a strap. As they were pushing up the bolts two small rocks fell and one knocked his hat off and the other struck him on the head causing a laceration to the scalp requiring two sutures to close.

EE WAS STANDING AT THE REAR OF THE MINER MAKING SURE THE RAM CAR DID NOT RUN INTO THE MINER CABLE. A ROCK FELL FROM THE ROOF STRIKING HIM ON THE FOREHEAD & CHIN LACERATING BOTH. THE LACERATIONS REQUIRED 14 STITCHES TO CLOSE.

#2 mother belt at C-2 crossover overcast- injured was descending outby ladder on overcast when a piece of rock fell and caused him to slip and loose his balance on the steps and fall striking his left hip and leg on the steps.

While tubing in 5H section a piece of slate fell from between the straps, bounced off of a tube and hit employee in the back of the neck causing a laceration. 19 stitches to close.

EE WAS WALKING AROUND THE CENTER BOLTER TO CLEAN OUT DUSTBOX WHEN A PIECE OF ROCK FELL FROM BETWEEN ROOF STRAPS & CUT HIS RIGHT FOREARM SUTURES REQ'D TO CLOSE CUT.

INJURED STEPPED AWAY FROM WORK AREA TO USE FACILITIES WHEN A PIECE OF SLATE FELL FROM TOP STRIKING HIM ON TOP OF HEAD RESULTING IN A BRUISE.

EE WAS REPLACING A BROKEN PIN IN THE DYNA-TRAC AT THE F-8 LONGWALL SECTION TAILGATE WHEN A PIECEOF ROCK FELL FROM BETWEEN #189 AND #190 SHEILDS AND STRUCK HIM ON THE RIGHT FOREARM CAUSING 3 LACERATIONS REQUIRING 20 SUTURES , THE ROCK MEASURED 8" THICK, 6" WIDE AND 36" IN LENGTH.

EE WAS OPERATING REMOTE CONTROL MINER AND A PIECE OF ROCK FELL FROM ROOF STRKING LEFT INDEX FINGER ON OR ABOUT THE KNUCKLE CAUSING A LACERATION. INJURED RECEIVED STITCHES TO FINGER.

WHILE MAKING WEEKLY AIR COURSE RUN, A ROCK FELL FROM BETWEEN STRAPS AT 11 1/2 WALL GLANCING OFF HIS HARD HAT KNOCKING HIM TO THE GROUND. HE EXPERIENCED BURNING IN THE BACK OF HIS NECK & RIGHT KNEE & SOME SWELLING IN HIS NECK. HE TRIED TO WALK IT OFF. BURNING WOULD NOT STOP. INDIVIDUAL BEGAN MISSING 7/7/03.

THE BOLT STRAP WAS SITTING ON THE TIMBER JACK & AS HE WAS PUTTING THE STRAP EXTENSION ON THE STRAP A PIECE OF ROCK FELL FROM THE ROOF AND STRUCK THE STRAP EXTENSION AND PUSHING HIS RT. THUMB BACK TOWARDS THE WRIST. THE INJURY WAS DIAGNOSEDAS A FRACTURED WRIST.

THE INJURED WAS IN THE WALKWAY UNDER THE SHIELDS WHEN A ROCK CAME OUT OF THE FACE, SLID DOWN THE TOP OF THE SHEARER AND STRUCK HIM IN THE RIGHT LEG. HE RECEIVED A LACERATION WHICH REQUIRED NUM EROUS SUTURES INSIDE THE WOUND AND 8 STAPES TO CLOSE THE WOUND.

A ROCK FELL OUT OF THE ROOF AS HE WAS INITIATING THE START OF THE DRILLING PROCESS. HE WAS BOLTING ON THE SIDE OF A MINER. THE INJRY WAS DIAGNOSED AS A CONTUSION.

THE F6 LONGWALL SECTION WAS DOWN AT START OF SHIFT FOR A BROKEN DOG BONE AT TAILGATE. INJURED ATTEMPTED TO CROSS BETWEEN 191 AND 192 SHIELD AND A PIECE OF STONE 6" TO 8" THICK 18" WIDE AND 37" IN LENGTH FELL OUT FROM BETWEEN THE SHIELDS AND STRUCK HIM ON TOP OF THE RIGHT FOOT.

4 SOUTH SECTION, HE WAS CLEANING OUT THE DUST COLLECTION BOX ON THE CENTER BOLTER. HE HAD HIS LEFT HAND ON TOP OF DUST BOX WHEN A PIECE OF ROCK FELL FROM ROOF & STRUCK THE BACK ON HIS HAND, CA USING INJURY. *LOST DAYS BEGAN 6-14-02.

INJURED WAS SETTING POST WHEN A PIECE OF ROCK FELL STRIKING INJURED ON BACK OF HEAD AND SHOULDERCAUSING A STOVE TO THE NECK.

19A, USING CENTER BOLTER TO SUPPORT CUT THRU INTO OVERCAST ENTRY. HE HAD INSTALLED LEFT HAND BOLT AND CENTER BOLT AND WAS INT HE PROCESS OF INSTALLING FINAL BOLT INT HE RIGHT SIDE WHEN ROCK BR OKE AND FELL. THE ROCK ENDED UP FALLING OUTBY STRIKING RIGHT LEG, ANKLE AND FOOT. LOST DAYS BEGAN 4-22-02.

THE INJ WAS WORKING ON A FALL WHEN A PICE OF THEWALL FELL & STRUCK HIM ON THE BACK. HE RECEIVED 11 STITCHES TO CLOSE THE LACERATION WHICH HE REC'D

EE WAS PULLING THE DUSTER ON THE LOADER WHEN A PIECE OF SLATE FELL FROM THE MINE ROOF AND STRUCK HIM ON THE RIGHT WRIST. THE PIECE OF SLATE MEASURED 6 1/2"X16"X0 TO 3/4" THICK 7 SUTURES WERE U SED TO CLOSE THE WOUND.

WHILE SCALING DOWN LOOSE STONE FROM THE MINE ROOF IN #1 ENTRYOF 3-N LEFT, A SMALL PIECE OF STONE FELL, STRIKING HIM ON THE CHEEK. WHEN HE BACKED UP, A LARGER PIECE (12" X 3/4" X0") FELL AND ST RUCK HIM ON TOP OF HIS RIGHT ARM ABOVE THE WRIST

INSTALLING WIRE MESH ON ROOF. A SHARP PIECE OF ROCK FELL FROM ROOF & SLID DOWN MESH & STRUCK RIGHT SIDE OF HIS FACE CAUSING LACERATION.

2D SECTION WHILE STANDING IN AN INTERSECTION, A PIECE OF ROCK FELL FROM BETWEEN THE ROOF STRAPS AND STRUCK HIM ON TOP OF HARD HAT. JAMMED NECK.
