Employee felt pain in his stomach and groin area as he attempted to lift a drill head in 4 south left section. Continued to work. *** Diagnosed with Hernia 10/28/2009.
EE was under the 4 South No. 2 belt, at about 24 X cut, changing bottom rollers. The area was wet and muddy. A roller fell about 24" and struck the little finger of the right hand, resulting in an "open fracture", requiring 5 sutures and a splint.
Employee was removing crib block from in front of #11 Voest Alpine Miner with a co-worker in the 3 North Right Section. Both employees picked up a crib block to toss aside when the crib blocks collided catching the tip of injured's right middle finger between the two crib blocks. Employee sustained a fracture to the right middle finger. Employee was wearing Meta Miner gloves.
An unplanned roof fall occurred in the 12 I longwall tailgate. The fall measured approx. 16ft wide 8 feet high by 25 feet long. No injuries or equipment damage as a result of this incident.
A malfunction of the crabapple elevator occurred due to worn out contactor tips. The elevator was able to be operated by manual means, but could not be operated in automatic. This problem occurred throughout this shift and was completely corrected at the end of Midnight shift by replacing the worn contactor tips and replacing a relay.
crew of 4 men were turning over Dosco beams that were stacked upside down, EE did not let go as others were dropping the beam and the tip of his right middle finger was caught between the beam s as he pulled away. the end of his finger was severed and remained in his glove, no bone fragments were in the glove.
Employee was throwing a belt roller under the bottom belt during a belt move in J tailgate section and felt pain in his left groin. Employee has continued to work but was diagnosed with a hernia on 10/5/2009.
Employee threw a track switch on bailey bottom. The switch throw spung back and struck the employee in the right leg. He continued to work with pain in his leg for several days. He went to see a doctor and a blood clot was detected in his calf. Lost work day began 9/30/2009
EE was walking past #19 630 scoop parked in #2 entry. E-20 section when he brushed against a metal burr sticking out from the scoop battery frame causing a laceration to the back of the right hand. EE was wearing leather Meta miner gloves.
EE was installing a bottom belt roller on wide side of the 3 north #2 belts at #36 walls when the roller slipped and caught the tip of his left index finger between the end of the roller and frame of roller hanger. EE sustained a laceration to the tip of is left index finger requiring 7 sutures. EE was wearing Meta miner gloves.
An unplanned roof fall occurred in the 12 I longwall section tailgate entry. The fall occurred at 43 x-cut. The fall measured approx. 20ft long by 16ft wide by 10ft high. No injury or equipment damage as a result of this occurance.
In process of making weekly exam; Mine Examiner discovered a roof fall 2 cross cuts outby the "AT" Evaluation point. The roof fall was a failure in the inersection and partial failure of cross cut. There were no persons injured or equipment damaged as a result of this roof fall.
EE has been diagnosed with carpal tunnel syndrome of the left and right wrist. Started missing work on 12/2/2009 due to surgery.
An unintentional roof fall occurred in the return entry of the 13 I section inby 15 xcut. The fall measured approx. 70 ft long by by 16ft wide 8 ft high. No equipment damage or injury to report as a result.
While moving roof support supplies, employee was throwing lincoln logs through a man door. While throwing a lincoln log through the door, his glove caught on a splinter on the log causing his glove to be pulled off and then he struck his right hand on the frame of man door.
The shop mechanic was cutting a piece of plexiglass with a circular saw. The dust from cutting accumulated onto his safety glasses. He took off his glasses to clean them and a piece of dust went into his right eye.
13-I-The miner operator was sumping the miner out. The right side miner bolter was beside the miner. Some bolts were stored on top of the miner. One bolt fell, struck a stack of augers bits that were also being stored on the miner. The augers then struck the right side bolter operator on the lip. This resulted in a laceration to his lower lip.
EE was checking the charge on a set of batteries when the lid fell and hit his left hand.
While stepping out of the high lift in Crabapple yard, he stepped off of the last step and his foot was on uneven ground. His leg twisted and felt pain in his left leg. *** Lost work days began 10-22-2009

Helping to lift miner cable off of the boom of the miner in 4 South left and felt a pain in his left knee. Lost workdays began 10-28-2009.
EE was disassembling a CMI EB-42 dryer, while standing on top of the unit - he repositioned his feet - his right foot stepped into the opening causing him to fall while his foot was still in the open center section - the EE fractured his right tibia.

A roof fall occurred at #14 wall 3 North in #3 entry at survey station 153 + 23. The roof fall was above anchorage. There was no damage to equipment. The fall was 45 feet long, 25 feet wide, and 15 feet high.
EE was roof bolting off the miner in #280 wall crosscut,#5-#6 entry, 3 N. left section. While reaching to pick up a drill steel that had fallen across the drill head to use in retrieving the drill steel still in mine roof, the steel in the mine roof fell & struck him on the L. hand resulting in a fracture to the metacarpal of the left little finger. Also a laceration-3 sutures.
Employee was struck in knee with muck bucket when it was being hoisted and received a knee contusion.
An unintentional roof fall occurred in the #1 entry of 4 South mains. The fall was located at SS 175+20 in a return entry. The fall measured approx. 25 ft long by 16 ft wide by 8 ft high. No injury or equipment damage as a result of fall. Fall will not be cleaned up.
Employee was using a knife while removing a pipe band/gasket, slipped, cut right hand.
While putting supplies on the miner at the Bailey expansion section, EE was pulling a metal banding strap out of a stack of roof straps and the free end flipped back and cut his left forearm causing a laceration. ***accident resulted in 10 sutures

Empoyees left boot became stuck in the mud in 12 I. As he attempted to free himself he felt pain in his left knee and hip. Lost time began 7/23/2009.
Twisted lower back. Employee was holding one end of a spreader, the other end was unhooked from the crane and dropped.

The miner was removing rollers and frame. The frame slipped and caught two fingers of left hand between frame and bed rail, cutting 2nd and 3rd finger of left hand.
Employee was assisting co-worker in putting a bolt in the roof. He was keeping the drill in line while pushing the bolt up when the bolt slipped off the drill head and caught his right thumb in between the bolt and the drill head.
Employee was in the process of setting or had set a roof jack, on the left inby corner or #60 Crosscut between the No 2 and No. 1 entry in the 4 west miner section. A piece of stone fell out of the roof and struck the employee, pinning him to the ground.
The miner was tightening the belt using a come-along (tug it). He felt pain in right hand. Pulled muscle & tendons.
EE was holding the 16" slider tube with both hands while tow other EE's were sliding the 18" vent tube over it in #5 entry, 3 North Left Section, when the 18" tube caught his left thumb between the slider and vent tube. Ee was wearing gloves.
Employee was picking a 2 inch air pump out of the mud in 13H can line and felt a strain in his groin. ***Diagnosed with hernia and began missing time on 6/19/2009***
Employee was moving the wheel burro rock duster between #4 and #4 1/2 wall crosscut in #3 entry. E-20 beltline when she slipped on wet uneven bottom and felt pain in her left knee and left shoulder. She continued to work and began missing due to the knee injury on 6/23/09.
The miner was trying to pry a battery cover open with a hammer the lid slipped and struck small finger of left hand causing contusion and abrasion to the finger.
