Employee was backing out of the loader compartment when a piece of stone from in between roof straps struck him on the back.

A fully trained miner was adding oil to the miner when a rock fell from between the straps striking him in the neck shoulder and arm The area was properly bolted and he was wearing required PPE but he should have made a work place exam of the area before starting work.

A fully trained miner was changing a broken piece of monorail in the belt line in 13 east while removing the come-along a piece of rock fell from between the straps striking his R. wrist. He was wearing the correct PPE .

While bolting with a fletcher twin boom bolter in the #9 entry on the rt side of the mains, at the overcast area a piece of draw rock measuring 1'x1'x6" fell from approximately 10' between the canopy and the ATRS striking the miner on the top of his head. The miner continued to work until around 1:00 pm and then came out of the mine and was transported to Wheeling Hospital

A fully trained roof bolter had drilled a 6 foot hole and had lowered the first steel and proceeded to add 3 foot pusher and went to pickup the steel and a piece of slate fell on top of the employees left hand while on the tray resulting in a laceration and swelling. The employee finished his shift and afterwards drove himself to get medical treatment.

A fully trained employee was bolting in the #1 entry when the steel got hung up he then started drilling a second hole and a piece of the top (3'x5'x3") fell out hit the ATRS and bounced off striking the employee in the left arm and leg

A fully trained roof bolter was installing a cable bolt in an overcast area. As he was drilling small piece of stone broke loose striking his safety glasses. The safety glasses pushed on the bridge of the safety glasses cutting his nose. 6 stitches.

Employee had removed a rib strap and piece of stone struck his wrist. Began missing time on 9/20/14.

He was a trained utility man who was scaling roof material by the miner on the 13 East Miner Section. He was wearing all of his personal protective equipment including his metacarpal gloves. As he was scaling, a piece of material fell out of the top and struck his right hand.

As he was moving the boom of the miner, a piece of roof material measuring 2 1/2 feet by 1 1/2 feet by 6 inches thick fell out between two straps above his head and struck his hardhat and neck.

He was installing roof support with a hand held air drill. He was wearing his protective equipment, and trained properly for the job. He examined the roof and confirmed it was sound. A piece of material fell from the roof and struck his left thumb causing a fracture and laceration which required sutures.

He was rib bolting off of the continuous miner which is the normal job he is trained on. He had examined the top before the miner advanced forward. A piece of roof material fell from the top and struck his hardhat knocking it from his head and another rock fell striking his head resulting in a laceration requiring sutures.

He was filling in on section as a utility man. He was standing next to the continuous miner while the operator was cutting coal. A piece of material 4 feet long by 2 feet wide and 1 1/2 inches thick came out of the top and struck his left shoulder causing a separated shoulder. He was wearing all of his Personal Protective Equipment and the crew stated that the top was good.

Employee was at the 11A shear when a rock struck his right ankle resulting in a fracture. The shear was in the process of being serviced at the time of the accident.

The employee was rib bolting on the 12A Section. A rock fell from the top, struck the continuous miner and then struck the employee's right foot. The rock fractured a bone in his right foot.

He was placing bolt plates onto the continuous miner when a piece of roof coal fell out and struck his right hand. *We dispute the allegations in Citation #8055883 and submit this report solely to terminate the citation.

EE was struck in the face by a channel which fell while installing supplemental support on the 7 belt 15x6. EE recieved sutures.

While installing supplemental bolts on the 2B longwall face, employee was struck on left leg by a piece of material which fell from the top. The impact caused a fracture of her lower leg.

While installing supplemental bolts on the 2B longwall face, the EE was struck on the left foot by a piece of material, which fell from the top. The impact caused him to fall, striking his left shoulder.

While tramming a mantrip on the 5 south mainline the employee was struck on the right side of the mouth by a piece of stone which fell from the roof(between the mesh).

Employee was holding rib stake for rib pinner when a rock fell from top striking employee on the left forearm.

Hanging screen and bolting when a piece of rock fell between screen and struck employee on the nose.

Installing rib pin on tube side when rock fell from between the straps (roof). He was struck on back of head, shoulder.

Employee struck in back of head by a 13"x16"x 1/4" piece of stone which fell from between the straps as he was preparing a bolt.

Employee was bolting on Miner when a piece of material fell from the top and struck left little finger.

While reaching for drill steel, a piece of material struck his left hand pinching his left index finger between the material and the steel.

Moving Miner out of #1 entry stone fell between straps, hit employee in the mouth.

5 NORTH FACE - #4 ENTRY. WHILE CENTER BOLTING, MATERIAL FELL FROM TOP (FROM BETWEEN ROOF STRAPS) AND STRUCK OPERATOR ON RIGHT SHOULDER.

1 left-5 south longwall-top bleeder entry. When pressurizing water rocprop support against roof, part of roof gave away - causing rocprop to slip hitting man in the chest.

5 NORTH HEADGATE IN CROSSCUT, MAN WAS BOLTING OF LEFT SIDE OF MINER WHEN A ROCK FELL FROM BETWEEN THE ROOF STRAPS HIT HIM ON NOSE HARD HAT

5 SOUTH FACE. MAN WAS WALKING PAST LOADING MACHINE. THE LEFT RIB FELL KNOCKING HIM TO THE BOTTOMAND PART OF RIB FELL ON LEFT LOWER LEG.

WHILE DRILLING, RIB ROCK FELL IN SHAFT STRIKING EMPLOYEE. ROCK HIT RIGHT SHOULDER AND DOWN RIGHT CHEST WALL.

EMPLOYEE WAS DRILLING WHEN A ROCK FELL, GRAZING HIS LEFT SHOULDER & THEN HIT HIS LEFT INDEX FINGER & KNUCKLE.

XCUT IN 5 SOUTH FACES. TOP FALLIN OUT IN CENTER AS MINER WAS CUTTING IT. ATRS PADS RAISED AGAINST TOP AND ROOF STRAP RAISED TO TOP AND WAS SOLID ON SIDES. MAN USED CRIB & WOODEN MATERIAL TO LA G CAVITY OVER STRAP. AFTERWARDS AS BOLTERS STARTED BOLTING, ROOF FELL OUT AND HIT MAN.

1 RIGHT 5 SOUTH LONGWALL FACE. MAN WAS BETWEEN #1 SHIELD AND MINE RIB, SHIELDMAN ON #2 SHIELD RAISE TOES OF #1 SHIELD. MAN SLID CRIB BLOCKS UNDER TOES OF #1 SHIELD TO RAISE TOES OFF THE BOTTOM . WHEN FINISHED HE STEPPED BACK AND RIB ROLLED OUT BETWEEN TWO DUKE JACKS AND HIT HIS LEFT KNEE.

LOCATION, IN A CROSSCUT ALONG THE MAIN LINE AT 14-07. MAN WAS PUTTING UP A PANEL FOR A KENNDY STOPPING. A SMALL THIN PIECE OF SLATE FELL FROM THE ROOF HITTING HIS RIGHT ARM BETWEEN THE CUFF OF HIS PROTECTIVE GLOVE AND HIS SHIRT SLEEVE.

A PIECE OF ROOF FELL OUT BETWEEN STRAPS (5" X 8"X 3 1/2'). IT HIT TOP OF MAN'S HARD HAD KNOCKINGHIM TO THE GROUND.

SETTING BITS AND ROCK FELL AND HIT TOP OF HAND. DID NOT INVOLVE ANY ASPECT OF COMLIANCE WITH RULES AND REGS. DID NOT INVOLVE MINE EQUIPMENT OR MINING SYSTEM. MAY HAVE INVOLVED PROFICIENCY BUT NOT TRAINING OR ATTITUDE. MAN HAD PROTECTIVE GLASSES ON.
