Employee was placing glue in the tray of the roof bolter when a rock fell from the roof striking their right foot. Employee was placed off work on 4/13/26.
CM operator was injured when a piece of rock fell from the roof, hitting the floor then rolling onto the operator's right leg and foot.

Employee opened a door in order to move supplies with a forklift. When employee was getting ready to close the door, a rock fell out from the roof and hitting employee on the head causing an injury.
Continuous miner operator was struck by a rock that fell from the roof that broke the operator's right fibula.
While operating roof bolter employee was struck by a glancing blow from a piece of drawrock. Employee finished regular shift, continued to work and did not go to the doctor until 6-14-17. Employee was released full duty. During a follow-up visit on 7-24-17 employee was taken off work pending an MRI. It is at this time the incident became a reportable injury.
While employee was standing on the off-side of roof bolter #13, employee was struck in the right side of head and face by a rock that fell from the top which was 9' high. Injured went up to the bolter to ask the bolt crew if they needed any supplies when the rock fell from the top. The rock measured 42"long x 18"wide x 3".
While employee was setting bits on the longwall shearer, a rock approximately 4" thick x 3' wide x 7' long rolled out of the top striking the employee.
While building a stopping, a rock fell form the top striking him in the head.
Principal was preparing to drop in a track switch. As he was hanging a chain ratchet to drop the switch into position, a piece of draw rock fell from the top striking the injured in the left upper arm.
As the EE was installing roof bolts the drill steel that he was using became clogged. When he was removing the clogged drill steel from the drill hole a piece of draw rock struck the EE's left hand pushing it down onto the drill pot. This caused pinching of his left ring finger between the fallen draw rock and the drill pot.
EE was super bolting at the #4XC on 1 North Track. As he started drilling into the top, a piece of draw rock approx 2' wide X 2-3' in length and 2-3" thick, fell from around the drill steel. The rock hit the canopy and drill head, bounced over the drill deck striking the EE, then knocking him down to the drill deck.
On 9 left panel building cribs, a piece of the roof scaled out between roof bolt,striking the EE on his head sliding down his shoulder striking injured EE on his left foot causing injury to three toes.
While setting timbers beside the feeder, a piece of loose rock fell from the top striking employee on the left side of the head, lacerating his ear.
While hanging a line curtain in the #4 entry, a loose place of rock that was strapped with a 4" metal strap felt from the top striking EE on his lower left back siding down his back and landed on his left foot, bruising his lower left back and the top of his left foot.
The employee was standing and talking with the shuttle car operator when a piece of rock fell from the top and struck him on the right hand and finger.
The employee was grading the mine floor with a Joy CM1415 in the 2 Lt. #3 bleeder entry. In the process a piece of roof rock fell unexpectedly between roof bolts and struck the injured on the left shoulder and back. The average height of the No. 3 entry is approximately 9.5'. The employee started missing work on August 4, 2008.
Injured was in the process of pulling down loose material. A piece of rock fell along with the main rock that injured was intending to pull. The small rock struck him above the left knee resulting in a laceration requiring sutures.
A piece of rock fell from the mine roof and hit the drill pot on the roof bolter machine and then bounced and hit the employee over the right eye resulting in a laceration that required sutures.

The employee was hanging a ventilation curtain at the long wall head when a rock fell and struck him on the right leg.
Employee was in process of installing roof support cribs, for shield removal. A piece of top rock fell and struck him in the lower back with a glancing blow. The employee continued work until 2-7-06 before starting to miss.
Rock fell from top and struck the injured on the left side of the face & nose.

EE WAS HELPING COLLAR HOLE FOR RESIN BOLT. A ROCK FELL ON RIGHT THUMB CAUSING LACERATION
AN EMPLOYEE WAS STRUCK BY A PIECE OF TOP ROCK MEASURING 54" X 31" X 1/2" TO 12" THICK.
While sitting against rib a piece of draw rock fell out between roof bolts, striking ee of his left leg above the knee, peeling the layers of skin back about 6 inches.
An ee was struck in the back with a piece of top rock measuring 40" x 28" x 0-4" thick.

EMPLOYEE WAS BUILDING A CRIB IN THE TRACK ENTRY AT THE MOUTH OF 2 RT SECTION. A PIECE OF ROCK FELL HITTING HIM ON TOP OF HIS HARD HAT. HE FELT A BURNING SENSATION AND PAIN IN HIS BACK.
HIT BY 1" THICK BY 1' LONG PIECE OF DRAW ROCK WHILE WALKING UP BELT LINE.
EE HAD JUST ARRIVED AT THE FACE WHEN A ROCK ~2 1/2" X 1 1/2' X 1 1/2' FELL AND STRUCK THE EE ON THE RT. HAND, RING AND LITTLE FINGER. EE'S HAND WAS ON DRILL POT.
EMPLOYEE SITTING ON RIB, FILLING OUT REPORT, ANDDRAW ROCK FELL ON HEAD.
THE EE HAND WAS LAYING ON TOP OF THE ROOF BOLT MACHINE WHEN A PIECE OF TOP BROCK FELL AND STRUCK HIM ON THE LEFT MIDDLE FINGER.
WALKING ACROSS SECTION BETWEEN 2 & 3 ENTRY AND DRAW ROCK FELL OUT BETWEEN BOLTS AND STRUCK HIM ACROSS RIGHT FOOT.
CLEANING UP LOOSE COAL AROUND TAIL PIECE LOOSE SLAB OF DRAW ROCK FELL OUT BETWEEN ROOF BOLTS AND STRUCK HIM.
PLASTERING STOPPING WHEN FALL ON LONGWALL OCCURRED CAUSING BLOW OUT OF STOPPING BY AIR DISPLACEMNT. INJURED KNOCKED BACKWARDS BY STOPPING BLOCK AND WAS KNOCKED DOWN TO MINE FLOOR AND STRUCK LE FT ARM ON BOTTOM.
THE INJURED EE WAS DIRECTING A MOLE INTO THE SHIELD RECOVERY AREA. HIS LEFT HAND WAS RESTING ON TOP OF A PILE OF CRIB BLOCKS. A ROCK ~1" X 7" X 6" FELL FROM THE ROOF AND STRUCK HIM ON THE BACK OF THE HAND.
WHILE CHANGING OIL IN A GEAR CASE, A ROCK FELL FROM THE SHIELD TIP TO THE FACE AND STRUCK THE EE. THESHIELDS HAD BEEN PULLED UP TO PROTECT THE WORKERS. THE ROCK MEASURED 80" X 16" X 7".
