While removing CLA bolts from a continuous miner, the bolt broke loose causing the employee to jam EE's wrist. Employee originally went to ER on 4/1 and to orthopedic on 4/4 where EE received a full release and returned to work both visits. At a follow-up with orthopedic on 4/17/2025 x-rays were conducted for a third time with a diagnosis of a fracture to the wrist.

The shuttle car had pulled under the continuous miner to be loaded; the continuous miner operator turned to glance at the S\C operator, and at that same moment, a large rock came off the miner boom, hit the shuttle car, and landed on the miner operator's right foot.
Roof fall occurred in the face of #3 entry that was approximately 20 feet in length.
