While employee was on 2 right sect., #6 heading, 9 block a piece of slate 1" thick x 2' wide x 1 1/2' long fell from between 6' resin bolts with 2"T x 6"W X 12"L hole boards, striking employee on his right hand. Employee hasn't called mine to inform us of his injury. Company has tried several times to reach him.
MAN WAS LOOKING AT SCOOP & HAD HAND ON FINDER & ROCK FELL OUT BETWEEN BOLTS. ROCK APPROXIMATELY 1" THICK, 10" WIDE, 11" LONG, HIT MAN ON THUMB.
WHILE CHANGING BITS ON MINER EMPLOYEE WAS BENT OVER TO PIECE A WEB MEASURING 1"-3" THICK, 16" WIDE 24" LONG FELL OUT AND HIT HIM ON BACK. (ROCK FELL OUT BETWEEN ROOF BOLTS.)
