A piece of draw rock fell striking employee on the back of the legs and ankles causing a fracture to left heel.
While moving cut ends from timbers, the employee picked up a timber end and a piece of draw rock fell striking EE on the right forearm and wrist causing a laceration on the arm requiring sutures.
While worker was repairing a water valve, a piece of draw rock fell striking EE on the right shoulder and head. EE continued working and began missing work on 3/27/2017 when taken off work by a physician.
While moving the roof bolter, a piece of roof rock measuring 13" x 15" by 2" in thickness fell striking employee's hard hat causing the hat to slide down employee's face. Employee sought medical attention on a later date and began missing work on 2/9/2016 when employee had surgery on their neck.
While employee was hanging the miner cable a piece of draw rock fell striking right hand which was still on the cable. The employee received a laceration on right middle finger which required sutures. EE was wearing gloves at the time of the incident and returned to work next scheduled shift.
After the ATRS had been set worker was walking up the drill station when a piece of rock measuring 48" by 26" by 1 1/2" fell and slid down the T-bar and struck him on the head and back. He suffered a laceration on his head that was 2 centimeters in length that required three staples.
While employee was installing a permanent stopping, a piece of draw rock fell striking him on the left foot resulting in a fractured big toe requiring treatment.
While employee was tramming the miner, a piece of draw rock (25" X 18" X 0-7") fell striking the employee on the left foot causing a hairline fracture to the second metatarsal.
While employee was bolting a piece of draw rock fell striking the canopy of the machine. When the rock fell he turned and placed his hand on the bolter fan. The rock fell off the canopy and struck his left hand causing a fractured little finger and lacerations.
While the foreman was walking accross the section he was hit by a piece of draw rock measuring 52" by 24" and 0-2" in thickness. He suffered contusions to his back and right leg.
While he was tightening a roof jack, a piece of draw rock fell striking the employee on the left arm, causing a laceration that required 3 sutures.
While bolting, a small piece of draw rock fell striking the employee on the left ear, causing a laceration requiring sutures.

While removing a panel from a Kennedy Stopping a piece of draw rock fell striking the employee on the right forearm causing a laceration requiring sutures. The rock measured 6' by 3' and was 2" in thickness. The employee returned to regular duties the next shift.
A piece of draw rock measuring 24" by 12" and 1-3" in thickness fell hitting the canopy of the roof bolter. A piece of the rock measuring 8" by 12" and 3" in thickness broke off the larger rock and struck the roof bolter pusher plate mounted on the drill boom, and then bounced back striking the employee in the nose resulting in a laceration requiring 12 sutures.
As the employee was drilling a hole to install a roof bolt, a piece of draw rock fell lacerating his right arm requiring sutures.
As the injured employee was walking down the # 2 entry in the 1-Left panel a piece of draw rock measuring 36" by 20" and 2 inches in thickness fell striking his right arm causing a laceration requiring sutures.
As EE was operating the continuous miner, a piece of rock hit him on the front of his hard hat, pushing it down into his face. Either the bill of the hard hat or the rock hit his nose, causing his nose to be fractured. The EE does not know if the rock came from the roof or rib. He is scheduled to have surgery on 10/3/07.
Employee was plastering a stopping when a piece of draw rock fell from between roof bolts, struck his back, went down his leg, and struck his right ankle. He suffered a fracture of the ankle.
Employee was driving a 4-man ride along the roadway when a piece of draw rock of undetermined size fell & struck his upper lip & chin causing lacerations requiring sutures.
As employee was walking to his bridge carrier, a piece of draw rock fell from between roof bolts striking him on the right leg.

WHILE OPERATING A CONTINUOUS MINER FROM A REMOTE CONTROL, A PIECE OF DRAW ROCK FELL FROM BETWEEN THE BOLT AND RIB AND STRUCK HIM IN THE BACK. IT RESULTED IN 3 FREACTURED VERTEBRAE. IT OCCURRED ON THE 005-0 SECTION. AT THIS POINT, SURGERY HAS NOT BEEN REQUIRED.
EMPLOYEE STATED THAT AS HE WAS WALKING IN #3 ENTRY OF THE 002-0 SECTION, A PIECE OF DRAW ROCK FELL FROM BETWEEN THE ROOF BOLTS IN THE MINE ROOF AND STRUCK HIM ON HIS RIGHT FOREARM. IT CAUSED A LACERATION AND REQUIRED SURGERY TO REPAIR SOME MUSCLE AND TENDON DAMAGE.
HE & CO-WORKERS WERE SETTING BREAKER POSTS BETWEEN #4 & #5 ENTRIES AT #34 CROSSCUT ON THE 8 LEFT PANEL (005) SECTION. HE WAS ON HIS LEFT KNEE AND CLEANING OUT A SPOT TO PLACE A TIMBER WHEN A PIECE OF ROCK APPROX. 33"X 35"X 2"THICK FELL FROM BETWEEN BOLTS AND STRUCK HIM ON THE LEFT LEG, ABOVE THE ANKLE.
WHILE WALKING TO THE ROOF BOLTING MACHINE ON THE 0003-0 SECTION, A PIECE OF DRAW ROCK FELL FROM THE MINE ROOF AND STRUCK HIM ON THE HEAD, NECK AND SHOULDER. THE ROCK FELL FROM BETWEEN BOLTS AND STRAPS PREVIOUSLY INSTALLED. HE COMPLAINS OF PAIN IN THE NECK AND SHOULDER AND IS OFF TAKING THERAPY.
HE AND A CO-WORKER WERE BUILDING THE LAST OF 3 CRIBS IN THE LEFT CROSSCUT OF #2 ENTRY, ON THE 5 NORTH SECTION (003 MMU), WHEN A PIECE OF ROCK FELL FROM BETWEEN THE ROOF BOLTS AND THE COAL RIB STRIKING HIM ON THE SIDE OF THE LEFT FOOT. THE ROCK WAS APPROXIMATELY 3' WIDE X 4' LONG X 0" TO 8" THICK.
INJURED STATED THAT AS HE WAS HANGING A CURTAIN IN THE ENTRY, A PIECE OF DRAW ROCK FELL BETWEEN THE RIB AND ROOF BOLT AND STRUCK HIM ON THE TOP OF HIS RIGHT FOOT. IT RESULTED IN A FRACTURE TO A BONE ON THE TOP OF HIS FOOT.
WHILE CHANGING DRILL STEEL FROM THE STARTER STEEL TO THE FINISHER STEEL, A PIECE OF ROCK FELL FROM THE ROOF AND STRUCK HIM ON THE LEFT HAND.
HE WAS SETTING BREAKER TIMBERS BETWEEN #4 AND #5ENTRIES ON THE 005 SECTION WHEN A PIECE OF ROCK APPROX 20" X 36" X 2" THICK FELL FROM BETWEEN THE ROOF BOLTS AND STRUCK HIM ON THE LOWER LEG.
HE WAS HELPING THE CONTINUOUS MINER ON A PILLAR SECTION WHEN THE POWER WENT OFF OF THE MACHINE. HE WENT TO THE REAR END OF THE MINER TO SEE IF THE BREAKER HAD KNOCKED AND A ROCK (APPROX. 57" X 78" X 15" THICK) FELL TRAPPING HIM BETWEEN THE MINER AND THE ROCK.
HE WAS SITTING BESIDE THE COAL RIB NEAR THE FACE ON A PILLAR SECTION WHEN A SMALL PRESSURE BUMP OCCURRED. HE WAS STRUCK IN THE BACK WITH COAL OR ROCK.
HE HAD NOTCIED A PIECE OF ROCK THAT NEEDED TO BE SCALED WON. HE RETRIEVE A SCALE BAR AND PULLED THE ROCK. THE ROCK FELL ON HIS LEFT LEG RESULTING IN TWO FRACTURES. THE ROCK WAS APPROX 5' LONG X 40" WIDE X 0-4" THICK AND FELL FROM BETWEEN BOLTS.
HE HAD SAT DOWN BESIDE THE RIB TO EAT A SANDWICHAND A PIECE OF DRAW ROCK APPROX 52" LONG BY 26" WIDE AND 5" THICK FELL FROM BETWEEN THE ROOF BOLTS AND THE RIB, AND STRUCK HIM ON THE SHOULDERS AND BACK.
HE WAS ROCKDUSTING THE #1 ENTRY FACE AREA BY HAND. AS HE REACHED FOR A BAG OF ROCK DUST, A SMALL PIECE OF SHARP ROCK FELL FROM THE ROOF AND STRUCK HIS HAND.
HE WAS INSTALLING A BOARD OVER A LOOSE ROOF PLATE WHEN A PIECE OF ROCK FELL AND STRUCK HIM ON THE NOSE, RESULTING IN A LACERATION THAT REQUIRED SUTURES.
HE WAS CHANGING A REMOTE CONTROL BOX ON A CONTINUOUS MINER WHEN A PIECE OF ROCK APPROX 6" BY 6" BY 1" FELL AND STRUCK HIM ON TOP OF HIS RIGHT HAND.
HE WAS SCALING DOWN A LOOSE RIB WHEN A PIECE OF COAL OR ROCK STRUCK HIM ON THE LEFT FOOT. HE WAS WEARING RUBBER BOOTS WITHOUT METATARSAL GUARDS.
A PIECE OF ROCK MEASURING APPROX 3' X 3' X 2" THICK FELL FROM THE ROOF, FROM BETWEEN THE BOLT AND RIB, AND STRUCK HIM ON THE LEFT FOOT.
BENT OVER TO PICK UP A GREASE GUN, PIECE OF DRAW ROCK FELL HIT HIM ON THE BACK.
EE WAS WORKING ON A SCRUBBER SCREEN WHEN A PIECE OF DRAW ROCK FELL OUT BETWEEN THE 1ST BOLT AND RIB STRIKING HIM ON THE HEAD AND SHOULDER RESULTING IN A FRACTURED CLAVICLE & COLLARBONE.
