Showing all 41Injured member was installing roof bolts when a piece of draw rock fell and struck his right arm just above the elbow. He was transported to the ER and diagonsed with tissue bruising. The ER physician placed him on restricted work activity.
Injured employee was walking between the rib and a shuttle car, when a piece of draw rock fell from between the rib and first bolt and struck him on the back of his neck, just below his hard hat. Rock measured approx. 3'x2'x1"-2". The ER physician excused the injured for 7 days due to a strain of the neck and tissue injuries.
Injured employee was operating the shear when a rock fell from between the shields and smashed his fingers on the pan line. He was transported to the ER by EMS. Diagnosed with a fracture and lacerations to left index finger. He was hospitalized for observation and released on 04/14/10. He remains excused from work.
Injured employee was unloading sand jacks from the motor when he struck the roof with one of the jacks breaking loose a rock which stuck him in the corner of his left eye lacerating his eye lid. He received 3 sutures. The rock measured approximately 6"x6"x2".
Injured employee was walking the roadways prior to the start of his shift when a piece of draw rock approx. 26"x48"x1-3" fell from between the roof bolts and struck the left top of his head and continued down the left side of his torso. He was airlifted to the ER and admitted to the ICU. ER Diagnosis: 4 Fractured vertebrae, small bleed on the brain and injuries to his left ribs.
Injured was stocking his tray on the roof bolter when a rock approx. 41''x31''x4'' fell out of the roof and struck his left hand causing a laceration. Injured was taken to the ER by EMS. The laceration was sutured at the ER.
Roof in number 4 face was mine out for an overcast, Injury had walked to the side of the roof bolter to move bolting supplies to his tray, A piece of rock 3 ft by 1 ft fell striking him on the top of right shoulder, resulting in a chip to his shoulder bone
While he was standing at the side controls of the bolter a piece of roof rock fell from between the bolts striking him on the top of his left foot causing a bruise. On 5-25-07 he went to a doctor and was removed from work due to swelling of his foot.
EE was loading bolt supplies from scoop to bolt machine in #4 entry, when a rock fell from the top, approx 12" X 12" X 1/2" thick, hitting him on the right forearm.

EE & co-workers were standing near the front of the RRII when the accident occurred. Scoop operator was scaling down a piece of draw rock that measured approx 3'x 3'x 3/4". When rock came down it struck a piece of drill steel that was hanging over the edge of the roof bolter supply tray. This caused drill steel to come up & hit EE on right cheekbone.
PULLING #144 SHIELD OFF LINE. AFTER SHIELD WAS REMOVED, MEMBER STARTED SETTING 10" PROPSETTER WHERE THE SHIELD WAS LOCATED. CO-WORKER HAD PROP STOOD UP AND READY TO BE WEDGED. AS EE STARTED TO WEDGE THE PROP A ROCK 4' X 4' X 8" STRUCK HIM ON THE LEFT HAND CAUSING A HAIRLINE FRACTURE.
EMPLOYEE WAS BOLTING IN THE HEADGATE ENTRY. HE WAS DRILLING FOR A RIB BOLT WHEN A ROCK APPROX. 2'X 2'X 3" FELL FROM THE TOP & STRUCK HIM ON THE LEFT ARM & THE FORCE FROM THE WEIGHT CAUSED HIS ARM TO HIT PART OF THE PLATFORM ON THE BOLTER. THIS CAUSED A LACERATION TO HIS LEFT ARM & SUTURES WERE REQUIRED. NO TIME LOST.
Ee was refilling glue box on the tray at the drill boom station. immediatelly after filling glue he placed his hand on the tray when a piece of draw rock measuring 44" x 16" x 0-3" thick fell from the roof striking his finger. left hand causing lacerations.
EE WAS BUILDING A CRIB WHEN A ROCK APPROXIMATELY 3' ROUND, 0-4" THICK FELL STRIKING HIM ON THE RIGHT SIDE OF THE FACE HITTING HIM OVER THE EYE AND NOSE.
EE WAS KNOCKING DOWN A STOPPING WITH A SLEDGE HAMMER. HE KNOCKED THE TOP HALF OUT AND STOPPED TO CLEAN HIS GLASSES. HE REMOVED HIS GLOVES AND GLASSES. HE PUT HIS LEFT HAND ON TOP OF THE SOLID BLOCKS AND A ROCK FELL STRIKING HIS LEFT INDEX FINGER. THE ROCK WAS 0 - 1" THICK, 20 1/2" LONG AND 10" WIDE. HE RECEIVED SUTURES.
EE WAS PICKING UP CRIB BLOCKS AND A PIECE OF ROCK FELL AND HIT HIM INT HE HEAD. HE SAYS THAT THE INJURY IS MINOR.
EE WAS ON HIS KNEES PUMPING OIL INTO THE ROOF BOLTER. A PIECE OF ROCK MEASURING APPROX 18" X 8' X 0-2" THICK FELL FROM THE MINE ROOF STRIKING HIM IN HIS RIGHT LEG. EE ALSO STATED HIS LEFT KNEE WAS EXPERIENCING SOME DISCOMFORT. MINING HEIGHT WAS APPROX 8 FT HIGH.
HE WAS WORKING ON PUTTING WEAR STRIPS ON MINER. A PIECE OF ROCK FELL FROM TOP AND HIT HIM ON TOP BACK PART OF HEAD. HE HAD PAIN IN HEAD AND NECK.
EE WAS CLIMBING ON TOP OF A SHIELD HAULER WHEN A ROCK FELL ON HIS HAND CUTTING HIS FINGER.
INJURED WAS WALKING RETURN AIRWAY IN OLD WORKS ON #1 SIDE. PIECE OF DRAW ROCK FELL (18" X 15" X 2") STRIKING HIS BACK. CONTINUED WORKING BUT HAVING TROUBLE WITH NUMBNESS IN LEFT ARM. DID NOT M ISS ANY WORK UNTIL 2/11/03.
EE WAS CLEANING THE SCRUBBER OUT WHEN A PIECE OF DRAW ROCK FELL OUT. THE ROCK HIT HIM IN THE LEFT HAND CAUSING A LACERATION.

HE WAS FRIED FROM THE FALLEN MATERIAL TREATED IMMEDIATELY BY PARAMEDICS AND TAKEN TO SURFACE WHERE AMBULANCE TOOK TO LZ HEALT
EE WAS SETTING A TIMBER WHEN A FALL OCCURRED. A ROCK MEASURING 2'X2'X4" FELL, STRIKING EE ON L SHOULDER.
INJURED WAS WALKING UNDER SHIELDS WHEN A PIECE OF DRAW ROCK FELL & HIT HIS LEFT FOREARM. HE WAS TAKEN TO HOSPITAL WHERE HE RECEIVED 2 STITCHES.
INJURED WAS TALKING WITH SECTION FOREMAN WHEN A PIECE OF DRAW ROCK APPROX 24" X 18" X 3" FELL AND STRUCK HIM IN THE RIGHT SHOULDER. HE FINISHED HIS SHIFT AND CONTINUED TO WORK HIS REGULARLY SC HEDULED SHIFTS UNTIL 5-30-01. HE SOUGHT MEDICAL ATTENTION AND WAS ADVISED TO REMAIN AWAY FROM WORK UNTIL FURTHER NOTICE. THE SAFETY DEPT. WAS NOT NOTIFIED UNTIL 6-4-01.
INJURED WAS KNOCKING BLOCKS FROM SUPPLY DOOR AREA WHEN A ROCK FELL FROM THE TOP STRIKING THE BACK OF HIS RIGHT FOOT. THIS CAUSED HIS TOES TO BE FORECED INTO THE METATARSAL OF THE BOOT. HE WAS TRANSPORTED TO HOSPITAL WHERE HE WAS DIAGNOSED WITH A HAIRLINE FRACTURE TO THE BIG TOE.
EMPLOYEE WAS PREPAIRING TO PUT SUPPLIES BACK ON THE SERVICE SCOOP WHEN A PIECE OF DRAW ROCK APPROX 1' BY 1' BY 2' FELL STRIKING HIS KNUCKLE OF HIS LEFT HAND RING FINGER. HE WAS TRANSPORTED TO CAMC WHERE HE WAS DIAGNOSED WITH A FRACTURE. THEINJURY WAS SPLINTED. HE RETURNED TO WORK THE NEXT SHIFT.
EMPLOYEE WAS LIFTING A TIRE INTO PLACE WITH THE CHAIN HOIST HOOKED TO A ROOF BOLT PLATE. A ROCK FELL BETWEEN THE BOLT PATTERN STRIKING HIS LEFT LEG 4" ABOVE THE ANKLE REQUIRING 4 STICHES.
INJURED WAS STANDING UNDER #4 SHIELD PULLING #3 SHIELD IN AS A ROCK FELL FROM THE TOP BETWEEN JACK #30 AND #4 HITTING THE CIRCUIT CABLE AND HIGH PRESSURE HOSE, BOUNCING AND STRIKING HIM IN THE SIDE OF THE HEAD. INJURED WAS TRANSPORTED TO HOSPITAL WHERE HE RECEIVED STITCHES.
STRINING OUT THE SHOOTING CABLE. THERE WAS A LARGE ROCK BLOCKING THE NORMAL WALKWAY. HE HAD ONE FOOT ON TOP OF THE SPILLBOARD AND ONE ON THE RACK. FACE ROLLED IN CAUSING HIM TO FALL AND COVERE D BOTH LEGS FROM CALF DOWN.
INJURED WAS REACHING FOR HAMMER FROM THE SHEARER AS A PIECE OF ROCK FELL OUT BETWEEN THE SHIELDS. THE ROCK SHATTERED WHEN IT STRUCK THE SHEARER AND A PIECE OF THE SHATTERED ROCK STRUCK INJURED 'S RIGHT ARM AND WRIST.
EE WAS TRAMMING CONTINUOUS MINER IN THE NO 2 ENTRY WHEN A PIECE OF DRAW ROCK APPROXIMATELY 2' X 2' X 2" FELL STRIKING HIS LEFT HAND. HE WAS TRANSPORTED TO HOSPITAL WHERE SUTURES WERE APPLIED T O THE LACERATION.
EMPLOYEE WAS STANDING BESIDE SCOOP CHECKING WATER IN BATTERIES. A PIECE OF DRAWROCK FELL OUT BETWEEN BOLTS AND STRUCK HIM ON HIS HARD HAT. EMPLOYEE ALLEGED IT STRAINED HIS NECK.
