Employee was diagnosed with pneumoconiosis on 4/16/14. Company was notified of this event on 6/24/14.
Employee stepped out of the motor without getting the brake set at the #5 switch when the motor rolled over his right foot causing a distal fracture of his 3rd metatarsal. He was excused from work.
Employee was pulling cable when a loop in the cable caught his right foot, pulling him down and injuring his right knee and leg. He was seen in the ER later that evening and diagnosed with a strained muscle and small fracture of the lower right leg. He has been excused from work.
Employee was loading bolts and glue into a scoop bucket when he twisted with materials in hand and heard a ""pop"" and felt pain in his back. He continued to work his shift but the following day, he went to the ER and was excused from work.
Employee was assisting with the installation of a miner head when he stepped on a rock twisting his right ankle. He continued to work but went to the physician later that day and was excused from work. Diagnosed with a sprained ankle.
Employee was unloading a shuttle car tire from a supply car, while lifting the tire with a slate bar, he slipped and smashed his left index finger between the slate bar and a rock, lacerating his finger. He was transported to the ER and excused from work.
Employee was applying rock dust and cleaning the area when he tripped over loose material causing him to fall and injure his left knee. He finished his shift but went to the ER that evening. He was diagnosed with a Left knee injury and calf strain and has been excused from work.
Employee was building a stopping when his foot became entangled in the band that secures the blocks causing him to twist his knee and fall. He went to the ER and was diagnosed with a knee sprain and released to return to work. Employee worked until 02/22/13; when he went for an MRI which revealed an ACL tear and he was excused from work. First day away 02/25/2013.
Employee removed a cable from a rope hanger and when he raised up and turned the hanger hooked him in the nose causing a laceration. He received 3 sutures.
Injured employee was installing a 10ft. cable bolt when he was struck by the drill steel causing a fracture to his left thumb.

Injured employee was using a pick hammer to raise up a cable when he accidentally grabbed the pick side of the hammer causing a puncture wound to his left hand between his thumb and index finger. He received 2 sutures and was released to return to work.
Injured employee was pulling on a beam when it became fouled and when it came loose it struck employees left knee. He was treated at the ER and released to return to work. Employee returned to work the next day but due to continued discomfort, employee went back to the ER on 11/23/12 and was excused from work.
An unintentional roof fall was discovered on the Southeast Mains section in the #6 Entry, inby spad 6975. The fall measured approximately 55ft W x 20ft L x 10ft H. No injuries resulted from this occurrence.
Injured employee was opening a metal storage container when he smashed his left middle finger between the handle and container causing a fracture to the distal tip of his left middle finger. He was seen and treated at the ER.
Injured employee was using a jack to lift a 9Man man-trip when the jack kicked out and struck the employee on the left foot. He was taken to the ER and diagnosed with a contusion of the left foot and ankle. He was excused from work by the ER physician.
Injured employee was using a ratchet to tighten a bolt when the ratchet slipped causing the employee to hit his hand against a flite. He was examined at the ER and diagnosed with a fracture to the tip of his left ring finger.
Injured employee and co-worker were carrying long piece of metal. Co-worker dropped his end, causing metal to slip from injured's hands resulting in laceration to right index/middle fingers.
Employee was lifting a bottom roller when he began to experience lower abdominal pain. He was seen by the ER physician and diagnosed with a Abdominal Hematoma and excused from work.
Employee was traveling down the slope belt entry on a rubber tire 4-wheeler when the chains broke on the suspended belt structure causing one side of the structure to fall. The structure struck the roll bar of the 4-wheeler and the employees left shoulder and head causing swelling and bruising. He was transported to ER by EMS.
Injured employee was walking when he struck his head against the shield canopy. Employee was seen at the ER and excused from work.
While lifting material at the stage loader, employee began to experience lower back pain. He completed his shift and was seen at the ER later that day. He has been excused from work.
Injured employee tossed a piece of material onto the belt when it struck a slate bar that was on the belt and the slate bar flew up and struck employee over the left eye causing a laceration. He was transported to the ER where he received 5 sutures and was released to return to work.
Injured employee was setting a beam when he injured his left shoulder. While setting the prop, the bottom of the jack slipped causing the beam to fall striking him on the left shoulder. He refused EMS and finished his shift. He was then transported to the ER by a Safety staff member where he was diagnosed with a Left Shoulder Contusion and excused from work for 7 days.
Injured employee was operating a shuttle car when he hit a rut in the roadway causing him to hit his head against the canopy. He continued to work and went to the ER later due to increased pain. Initial ER physician released him to return to work but due to continued pain he went to another ER on 01/13/12 and he was excused from work and referred to neurological services.

An unintentional roof fall was discovered near spad number 4800. The fall measured approximately 20 ft. wide x 8 ft. high x 40 ft. long. No injuries resulted from this occurrence.
Injured employee was using B-Bond/Mine Sealant to plaster a set of doors when he got the sealant in his left eye. The eye was immediately flushed and he was transported to the ER. He was treated for a chemical injury to the left eye, prescribed medication for his eye and referred to Optometry. He was released to return to work.
Injured employee was walking down the track entry when he stepped in a hole and twisted his right ankle. He was transported to the ER and diagnosed with a right distal tibula fracture. He was excused from work and referred to Orthopedics.
Injured employee was lifting belt structure when he suddenly began to experience lower back pain. He was transported to the ER. The ER physician excused him from work and referred him for a MRI due to the possibility of a herniated disc.
Injured employee slipped into a muddy ditch on the offside of the belt between 3 Head and Drive Motors causing him to sprain his right ankle. He was transported to the ER for evaluation and diagnosed with a sprain. He has been referred to orthopedics.

Injured employee bumped against a broken door handle causing a laceration to his left forearm. He received 3 sutures at the ER and returned to work after treatment.
Injured employee was accidentally shocked by another employee on the neck. An employee held a shooting battery next to the injured employee's ear and activated the detonator next to the injured employee's head causing a shock to the neck. Injured employee was transported to the ER by EMS.
Injured employee was trying to catch a rat and when he jumped onto the rat it bit him in the right arm causing a puncture.
Employee was shoveling 4 head trash pump @ 3 tailpiece when he slipped and struck hisself on back of his head and ear with shovel. Received 7 sutures on his ear.

Trying to raise rail to put blocking and gauge rod in and come-along came loose striking employee in mouth and lower right jaw. Chipped front tooth.
A roof fall was discovered in #4 entry inby spad 3106 and outby spad 3145 for a distance of approximately 420ft. No injury resulting from fall.
Injured member was stepping off a haulage car when he slipped in the mud causing his legs to split and injuring his left hip. He was diagnosed with a muscle sprain/strain and excused from work.
While attempting to pull gate shield the hook slipped off and struck member on left side of his face requiring 7 sutures.
While loading belt structure into mantrip he mashed his left little finger between two rollers requiring 7 sutures
An unintentional roof fall was discovered on the Mains Section, #2 Entry, Inby Spad #6543. The fall measured approximately 60 to 70ft long by 18 to 20ft wide by 8 to 10ft high. No injuries resulted from this fall.
Injured member was changing the bits on the head end of the shear when a small piece of metal from the bit or hammer broke off and became embedded in his left hand. The metal was removed on 03/14 and he received 4 sutures. He was excused from work on 03/14 & 03/15. Returned to work on 3/16 with a full release.
EE was riding on the mantrip, while passing through the air lock doors one of the doors began to close, that's when the member attempted to push the door open with is foot getting his left foot caught between the door and the mantrip. He was transported to the ER and diagnosed with a sprain and swelling. Member was placed on light duty for 1 week by the ER physician.
Injured member was cutting a piece of belt when the belt knife slipped out of the belt and into the injured's leg. He received 8 sutures.
Injured employee was adjusting the tail roller bearing when it slipped smashing the employees right little finger between the bearing and tailpiece frame. ER diagnosis: Fracture of right little finger and referral to specialty care. ER physician excused injured from work.

Injured member was lifting a pump at the bottom of the slope, when he felt a sharp pain in his abdominal cavity and he noticed a small discharge of blood from his rectum. He has not returned to work.
Injured employee was trying to remove a fitting by using a hammer and chisel when a small piece of metal came off of the chisel, it went through the employee's glove and into his right hand. The metal was removed and he received 2 sutures.
A roof fall was discovered on the slope track during the fire boss run. Fall measured approximately 12ft long, 12ft wide, and 7ft high. No injury occurred from this fall.
Injured was in the process of sitting down on the mine floor, when he accidentally laid his left hand on a belt knife blade that was laying on the mine floor, causing a laceration to his left thumb. Injured received 7 sutures.
Injured was walking down the belt line, he bent down to walk under wire mesh hanging from the top, when a piece of the mesh caught his right ear causing a laceration. He received 24 sutures.
EE leaned up against the fan wall. Wall folded and fell with the EE resulting in a bruise to the left knee.
Injured employee was removing a timber from the timber pile while another employee was cutting them with an electric chainsaw. The injured accidentally came in contact with the chainsaw causing a laceration to the posterior side of his right forearm and wrist. He received 23 sutures.
A roof fal was discovered on the slope track, the fall measured 12 ft high x 18 ft wide and 40ft long, located 800ft inby the portal No injuries from this fall
Injured employee was operating the shear when a rock fell from between the shields and smashed his fingers on the pan line. He was transported to the ER by EMS. Diagnosed with a fracture and lacerations to left index finger. He was hospitalized for observation and released on 04/14/10. He remains excused from work.
EE was carrying a door for a scoop and felt pain in his lower back, this door estimated to weigh about 40 pounds. He called in on 3-3-2010 and stated that the doctor had removed him from work and he was referred to a neuro-surgeon for futher evalation.

Injured employee was setting a prop and while hammering a wedge he lacerated his right hand on a sharp rock. He was transported to the ER and received 3 sutures. He was released to return to work.
Injured employee was pulling shields, when he turned around and struck his head on the shield. He completed that shift and his next shift on 09-15-09. He seen his physician on 09-16-09 and was excused from work untill 09-20-09. He returned to work on 09-21-09.
A roof fall was found at the #17 crosscut on the #3 belt track entry. The fall measures approximately 40ft long, 19ft wide & 8 to 10ft high. MSHA Inspector: AR: 23852
A roof fall occurred near the elevator bottom, Entry #1, Spad 2083. The fall measured 28ft long, 15ft wide and 4 1/2 to 5 ft high. No injuries resulted from this fall.
As the shear was cutting out on the tail side of longwall a small flame was osberved and was extinguished within 2 minutes using a wash down hose.
Elevator travel switch was out of time. Elevator would not come down. No one was on the elevator at the time of the malfunction. Elevator down time: 180 minutes.
EE had picked up two buckets of bits to take to the shear and as he turned to walk away, he felt a sharp pain in his right side of lower back. He did not cease work at that time. He later went to the doctor and was taken off work on 02/17/09 until he had an MRI. On 02/24/09 he had an MRI and was diagnosed with a bulging disc in his lumbar area. He remains off work.
The elevator door was left open and the elevator would not come down. The door on the surface had to be closed. No one was on the elevator at the time of malfunction. Elevator down time: 61 minutes.
A roof fall was found on the #3 track at the #15 break during a fire boss examination. The fall measured approximately 10 ft. wide, 15 ft. long and 4 ft. high.

Employee was operating the miner, while loading the shuttle car, the car moved causing employee to be pinched between the shuttle car's frame and the rib. He was taken to CAMC General ER were he was diagnosed with tissue bruising. He is off work awaiting to see specialized physician.
Employee was shoveling the #5 belt rollers between the 10th & 11th break when he felt a sharp pain in his mid & lower back. Within the hour he began to experience pain down his right leg. Specialized Physician removed him from work for 10 days.
Roof in number 4 face was mine out for an overcast, Injury had walked to the side of the roof bolter to move bolting supplies to his tray, A piece of rock 3 ft by 1 ft fell striking him on the top of right shoulder, resulting in a chip to his shoulder bone
Splicing Conveyor belt slipped on structure and cut leg just above knee.
A roof fall was discovered during the fire boss examination. The fall measured 20 ft wide, 20 ft long, and 8 ft high located at the # 25 cross cut on # 4 belt. No injuries occurred due to this fall.
He was removing a rock from the walkway and throw it in the panline, he felt a pain in his lower back.
EE was on top of mantrip putting it on charge, as he stepped down from mantrip, he twisted his right ankle. Resulted in a sprain.
A belt fire occurred on the tailpiece of the # 6 main line belt and was extinguihed in about 1 hour
Employee was putting the operator's end of a 15-ton locomotive back on the track. He had an airbag under the center of the motor fully blown and raised up. He had the other airbag on the left corner of the motor blowing it up to push the motor back over on the track and the airbag popped out striking his right arm.
Helping to make belt splice, he leaned forward to remove nail driver; he was hit on the right side of nose with a 3lb shop hammer, resulting in fracture of the nose.
A roof fall was encountered on the track travelway of the # 1 mine portal during a fire boss exaimation, the fall is located outby spad number 2041, the fall measured 19 ft. wide, approximatly 80ft long and 22ft high.
Injured was replacing turbo on tractor. When he was tightening up the clamp on turbo he felt a rip between his elbow and shoulder. He felt a burning sensation. His first day away from work was 8-21.
EE was moving timbers on the tail area He mashed his right thumb between the timber and pan line requiring 6 stiches
While helping to remove cross over walkway off belt line, his left arm was caught between the walkway and belt chain, causing a contusion to left arm. MRI on 8/15/08 was inconclusive and he was removed from work and placed on physical therapy for 2 days a week.
While cutting a bent roller out of the belt line, it kicked back hitting him on right arm requiring 3 sticthes resubmission of # 656751
A roof fall was discovered on the slope track as persons were traveling into the mine. It was about 10 ft. high, 50ft. long and 24 ft. wide.
While trying to remove a bent keeper bolt to replace a broken shear pin, he struck his left ring finger with a 3 lb. hammer requiring 13 stitches.
Helping to take down monorail along the belt line, he mashed little and ring finger between two pieces of rail.

Changing bottom roller on crossover belt. While holding roller up to use a slate bar, the bar slipped, striking him on the left side of his forehead. Caused a laceration, requiring 3 stiches. Returned to work next shift
Working on connecting 2 pices of 10 inch pipe, pipe slipped mashing his left little finger between the pipe and ground, requiring 7 stiches. No days were lost do to this accident.
A roof fall was encounter during the onshift examination of the slope and # 1 belt location the bottom of the Justice belt slope. This area is located at spad #2057. The fall measures 20 ft. long, 20' wide and 6 ft high. This fall will not be cleaned at this time and was timbered off. 6ft tension bolts were used in mining of this area
Door switch on elevator failed causing elevator to be down 32 minutes.
Employee was installing a 2 inch hose line. As he was trying to couple the 2 ends together, one end popped loose striking him on right hand causing a fracture to his hand.

Replacing dog bone that was broke while getting ready to raise pan line, using shield member, had his arm lying on panline shield to hook up chain. As he was lowering the shield, base dropped, causing shield to fall 8 to 10 inches and pinned members left arm between shield and pan line.
Roof fall no. 2 blong hole belt and track at the justice no 1. portal between crosscuts 29 and 30. Size 60 foot long. width and height unknown at this time.
Roof fall #15 cross cut. 20 x 20 x 6 foot.
Member was loading 2"" high pressure hose on mantrip end of hose slipped and smashed ring finger on left hand.
A roof fall occurred on the old belt line. The fall measured 20' by 20' and about 6' high. The fall will not be clean at this time.
A roof fall ocurred on the # 5 main line belt and track entry outby spad # 322, the fall measured 8 ft. high 40ft. wide and 20 ft. long, No injuries occured at this time.
Member was loading ram jack in scoop bucket when trappy shoe rolled over smashing right hand/index finger between ram and trappy shoe. Due to infection member was put off of work on 7/13/07.
Member was using a chain hoist, pulling on a roll of belt, while using the chain hoist with one hand and his left hand on an upright beam, when belt moved it pinched the end of thumb between the I-beam and up right, requiring stiches.
While he was standing at the side controls of the bolter a piece of roof rock fell from between the bolts striking him on the top of his left foot causing a bruise. On 5-25-07 he went to a doctor and was removed from work due to swelling of his foot.

A roof fall had ocurred in the corss cut along the track entry of the old #9 belt between the #4 & #5 entry. Spad #2399 of #5 entry. The fall measured 90'l x 20'w x 10' h. This area was bolted with 6' tension bolts & beams supported prior to the fall. No injuries. Area will not be cleared at this time.
Individual was setting rebar with hammer, when he missed, hitting rebar and hit his left index finger with blow from hammer he was using.
While standing on crib blocks working on miner, crescent wench slipped causing him to loose his footing, and he jarred his lower back
Member was loading a 100 ton jack into the company truck when he felt a sharp pain in back. He was unable to report to work on 3/8/2007 due to pain in back. He returned to work on 3/16/2007.
EE was attempting to change a blow plug on the fluid coupling at the G.H. 2 Belt Head. As he was turning an Allen wrench to loosen the plug, the wrench slipped causing him to strike his right wrist against the metal framing. EE ceased work immediately. X-rays were taken and were negative. Due to the excessive swelling he was removed from work.
Elevator malfunction occurred, elevator was down for 210 min. due to a grounded wire on the door.
Employee exited the mantrip to throw the track switch. The mantrip operator wasen't paying attention and ran into his right foot. Employee was transported outside and then drove himself to the hospitial. He saw a specialist on 1/31/2007 and was diagnosed with soft tissue damage to his right ankle. A cast was applied to help support the ankle from further damage.
Member was repairing a hub on a speed reducer. He was using a crib block to drive the hub on. As he hit it with the crib block his right hand struck the corner of the belt head frame cutting his hand. Six stitches were applied.
A malfunction of the elevator occured, causing it to be unoperatable for 935 min. The malfunction was caused by the driver card.

EE was loading a roller into a man trip a felt a sharp pain in his back. On 12-12-2006 he went to Dr. and had an MRI performed. The results showed a protrusion of the L5/S1 and bulging at L4/5 and was referred to a neurosurgeon. On 1/16/2007 he was removed from work.
He was attempting to put a wheel back on the mantrip by pushing on the axle with a bar. When doing so the wheel turned causing him to twist his left knee. On 10-7-06 he had a MRI. On 10-20-06 he learn that he would need surgery to repair 2 tears in his left knee.
Employee was at the tail drive preparing to build cribs where a high cavity had formed. He had started to set cribs. A piece of sandstone broke off above them striking the tip of shield 173, causing it to turn striking him in the back. On 10-13-06 he was removed from work by Dr. due to pain. He also was informed that he had fractured his #10 rib.
Employee was loading drill into #8 man hole, injured and twisted right knee.
EE was pushing on a piece of 3/8"" steel plate to straighten it up on the forks of a forklift. Stepped in a rut and twisted his ankle.
Employee was unloading belt out of the storage unit, as he was pulling on the belt, he slipped hitting his leg on the flat car the belt was being loaded onto. Employee went to the hospital on 9/2/06. Xrays were taken and revealed a slight fracture in his left leg. On 9/5/06 employee notified the Safety Dept. on his condition.
Employee was operating the longwall shearer and a piece of roof rock fell between the shields striking the shearer and then striking him on the mouth requiring 2 stitches. He returned to work on his next scheduled shift.
Ee was using a chain hoist to repair the miner cats. As he was tightning up the hoist to pull the cats back into place, it slipped off causing the hook to come back striking him above the left eye causing a laceration requiring 3 stitches. No days lost from this accident.
A malfunction occurred on the elevator for the Justice Mine overspeed latch causing it to malfunction and would not operate.
A malfunction occurred to the elevator due to a grounded wiring to the door switch on the bottom of the elevator. No one was in the elevator at this time.
EE was using a utility knife to cut off a piece of loose belt off the one belt line. His knife slipped cutting left forearm which required 5 stitches.
Employee was attempting to unplug drill steel by turning in while drill was in drive. Drill caught glove in rotation, twisting tight around wrist causing fracture of left wrist.
Malfunction of the elevator occurred which it was inoperable for 60 mins. Malfunction was electrical wiring to a door switch.
While removing a water line coupling, EE slipped causing him to hit his self smashing left middle finger between sledge hammer he was using and a relief valve, resulting in a fractured finger.
Employee was repairing a gear box in the miner. While using wrench with cheaterbar, bolt freed and he felt something pop in his shoulder. Hix first day away from work was 3/20/06
EE was starting to wash down rollers using a hose, when the pressure pump kicked on it caused him to slip and fall. He fell on his right thumb fracturing it.
AS HE WAS WALKING THROUGH THE #36 CROSSCUT ON THE LONGWALL BELT, HE SLIPPED IN A TIRE TRACK & FELT PAIN IN HIS RT. KNEE. HE CONTINUED TO WORK UNTIL 3/27/06, AT THIS TIME HE WAS DIAGNOSED WITH A TORN LIGAMENT.
As he was spotting shields he had his hand lying on a panline and a rock traveling down panline struck his left hand causing a laceration to the palm.
Employee was removing the side shield from the shuttle car reel. He removed all the bolts and was letting the shield down. The shield slipped out of his hand hitting him on top of his right foot.

A large rock was lying atop the cable troft. He was behind the rock and pushed it over into the face chain. One end of the rock went into the chain sending the other end up to the shield crushing his finger and causing a deep laceration on his right index finger. Sutures were required.
An elevator malfunction occurred at the Justice Mine due to a bad door switch and brake.
The elevator at the Justice Mine was down from 2:10 pm to 3:10 pm due to a malfunction with the control circuit.
Employee was performing start-up service on high pressure hydraulic pump for longwall. He adjusted packing to correct small leak and asked section mechanic to restart pump. On start up, high pressure hose & ball valve came apart striking employee under chin. Knocked out 3 teeth, broke mandible & damaged tissue in inner left ear.
Ee was lifting an oxygen bottle onto the mantrip. He smashed the bottle against the mantrip squashing his finger cutting it.
MEMBER WAS CHANGING THE DRIVE SHAFT IN THE SCOOP. HE WAS TIGHTENING UP THE BOLTS WHEN THE RATCHET SLIPPED OFF OF THE BOLT STRIKING HIM IN THE FACE JUST ABOVE THE LEFT EYE. SUTURES WERE REQUIRED.
A piece of rock fell striking injured's left hand above the knuckles between the little and ring finger. The rock turned vertical as it fell bringing a sharp edge in contact with the hand. Sutures were required.
Previous employee has been diagnosed with pneumoconiosis: company notification 7/2/14 Employee's date of last exposure 10/6/2005.
A ROOF FALL WAS DISCOVERED AT THE JUSTICE MINE ON THE LONGWALL TAIL AT ENTRY #1 AT SPAD #35 & 40. THE FALL MEASURED APPROX. 20'X 20'X 4'. ALL ENTRANCES HAVE BEEN BREAKERED OFF & THE FALL WILL NOT BE REHABILITATED.
EE WAS INSTALLING DRAWHEAD ON MOTOR. HE RAISED WINCH COVER AND PUT PIN IN DRAWHEAD. THE COVER CAME DOWN, HITTING EE'S LEFT MIDDLE FINGER. CUT THROUGH HIS GLOVE, CUTTING THROUGH HIS FINGERNAIL; STITCHES REQUIRED.
Member was helping to take down monorail when a rock fell from the top striking his middle finger. Sutures were required.
Employee was running the shearer when a rock came down between jacks and hit his finger, cutting it on the panline.
PULLING #144 SHIELD OFF LINE. AFTER SHIELD WAS REMOVED, MEMBER STARTED SETTING 10"" PROPSETTER WHERE THE SHIELD WAS LOCATED. CO-WORKER HAD PROP STOOD UP AND READY TO BE WEDGED. AS EE STARTED TO WEDGE THE PROP A ROCK 4' X 4' X 8"" STRUCK HIM ON THE LEFT HAND CAUSING A HAIRLINE FRACTURE.
EE WAS DRIVING MANTRIP WHEN TAPE FELL FROM THE OPERATOR'S STATION. HE REACHED DOWN TO PICK IT UP WHEN HIS LEG SWUNG OUT AND STRUCK THE RIB, FRACTURING HIS BIG TOE ON HIS LEFT FOOT.
EE ALLEGES HE INJURED HIS BACK REMOVING A ROCK ( 2'X 3'X1') OFF OF THE MAIN #3 GLORY HOLE BELT. HE WAS DIAGNOSED WITH A STRAIN TO HIS BACK. HE WORKED UNTIL 7/13/05 & HIS FIRST DAY AWAY FROM WORK WAS 7/14/05
Member was standing beside bolting machine helping reel cable. When the operator turned the pinner on, the cable reel took up slack causing the cable to strike him on the right cheekbone and across the bridge of the nose.
A roof fall occurred at the Justice Mine at #4 belt x-cut #21 in the track belt entry. The fall measured 20' by 20' by 8' and will be rehabilitated.
EMPLOYEE WAS CUTTING A PIECE OF 3'X 8' METAL THAT WAS LODGED IN THE FEEDER AT THE BOTTOM OF THE GLORY HOLE. HE REACHED THROUGH THE INSPECTION DOOR TO CUT ON THE RT. SIDE OF THE METAL; IT WAS UNDER TENSION & RELEASED STRIKING HIS ARM. HE RECEIVED A FRACTURE TO THE HUMEROUS.
EMPLOYEE WAS BOLTING IN THE HEADGATE ENTRY. HE WAS DRILLING FOR A RIB BOLT WHEN A ROCK APPROX. 2'X 2'X 3"" FELL FROM THE TOP & STRUCK HIM ON THE LEFT ARM & THE FORCE FROM THE WEIGHT CAUSED HIS ARM TO HIT PART OF THE PLATFORM ON THE BOLTER. THIS CAUSED A LACERATION TO HIS LEFT ARM & SUTURES WERE REQUIRED. NO TIME LOST.
EE WAS OPERATING THE #2 15 TON BROOKVILLE LOCOMOTIVE WITH NO SUPPLY CARS AT THE TIME OF THE ACCIDENT. HE WAS TRAVELING OUTBY UNDER THE #10 BELTLINE WHEN A PIECE OF CHAIN LINK FENCE GUARDING CAUGHT UNDER THE OPERATOR'S CANOPY & CAME INTO THE OPERATOR'S DECK STRIKING HIM ON THE LEFT EAR. HE WAS TRANSPORTED TO THE SURFACE & TAKEN TO CAMC WHERE SUTURES WERE APPLIED.
Employee was hanging cable and slipped in the mud when he twisted his right knee. His first day away from work was Aug. 28.
Member was using slate bar to pry rock. The bar slipped landing on his right hand causing a laceration. Sutures were required.
A ROOF FALL WAS DISCOVERED ON 3/3/05 AT THE JUSTICE MINE. THE FALL MEASURED 100' X 20' X 6-8'. IT WAS APPROX 7,000' FROM THE ACTIVE FACE. THERE WAS NO EQUIPMENT INVOLVED
EE & co-workers were in the process of cleaning rock off of the LW head drive. Member was lying on his belly with upper body positioned under the #2 LW shield. A rock approx 8"" x 6"" x 1/2"" fell off the shield tips striking his right calf. He walked to the mantrip & was brought to the surface. He was taken to Boone Memorial. He was diagnosed with a deep contusion to right calf.
A roof fall was discovered at the justice mine on 3/1/05 at 4:45 am. the fall was discovered at #4 belt, #4 entry, 20 feet outby spad #468. the fall measured 25'x20' an unknown height. the fall will not be rehabilitated.
EE & co-workers were in the process of relocating 8"" nyepak waterline to allow water to pump out of longwall gob system to water sealed area of mine. While lifting line to install gripper coupling EE stated he felt pain in lower back. He filed accident report & continued work. He called into work & stated his dr had taken him off work till he had results of MRI.
A co-worker was rolling a rock. The rock caught EE's right thumb between another rock. His finger became smashed between the rocks and was cut. He received stitches and also had a hairline fracture.
Justice mine discovered an unplanned roof fall on 2-7-05 on the tailgate section, #3 entry at spad #2910. The fall measured 80' length 20' in width and the height is unknown. the fall was 1,100 feet from the active face and will not be rehabilitated.
Injured was tightening bolt on brake head of #00-5 man Brookeville mantrip when he slipped on mine floor causing rt wrist to become fouled between brake head & frame of mantrip. He was brought outside by EMTs & transported to Boone Memorial. He was diagnosed w/contusion to rt wrist & released. He went to see another dr. later that day & was advised to take physical therapy.
EE was removing rock from the #7 head rock box when a piece of rock he had lifted shifted and slid onto his finger. He was transported to Boone Memorial where he received stitches.
Injured had just completed servicing scrapers on the #8 belt head and was walking toward the track entry. As he walked behind the belt take-up he stepped on a piece of conveyer belting that was laying on the mine floor and covered with water and rockdust causing him to slip and fall backwards straining his left knee.
Employee was walking through the last open crosscut towards the active longwall face when he slipped in mud and water, causing him to do a split with his right knee buckling under him. He was transported to BMH where x-rays were negative. He continued to work until Friday, Jan. 21st when he went back to hospital due to increased pain. First missed day was Jan. 24.

IE was putting forms on truck dump resulting in a strain to his back.
ON NOVEMBER 26, 2004 JUSTICE MINE HAD AN ELEVATOR MALFUNCTION. THE ELEVATOR WAS DOWN FOR APPROXIMATELY 101 MINUTES DUE TO ICE ON THE DOOR SWITCH.
THE ELEVATOR AT THE JUSTICE NO.1 MINE WAS DOWN FOR APPROX. 110 MINS DUE TO A BOLT MISSING FROM A BRACKET IN THE OUTER DOOR.
EE was servicing the scoop. He was picking up an acetylene bottle when it slipped and smashed his finger. He received stitches in his left index finger.
EE WAS OPERATING THE 12 CM IN THE #2 ENTRY. HE STEPPED OVER TO THE MIDDLE OF THE ENTRY TO CHECK THE CENTER LINE WHEN A PIECE OF ROCK FELL MEASURING 5' X 3' X 2"" TO 3"". THE ROCK STRUCK HIM IN THE HEAD KNOCKING HIM DOWN. HE WAS TRANSPORTED TO BOONE MEMORIAL WHERE X-RAYS WERE NEGATIVE. HE WAS DIAGNOSED WITH A CONTUSION TO THE BACK & NECK.
On Oct 7, 2004 justic no.1 mine had an unplanned roof fall at approx. 4:30 am. the fall measure 30'x20'x6'. the fall will be rehabilitated./
IE was plumbing under sectional bathhouse, hit elbow on ground resulting in a puncture wound to right arm.
He operated shuttle car in #3 heading when the accident occurred. He struck a depression in the roadway causing him to strike his head against shuttle car canopy. Transported to surface then hospital, he was diagnosed with sprain neck, suffered headaches. Continued work but saw dr on Oct 12. Dr advise him to say away from work until MRI was completed.
THE ELEVATOR AT THE JUSTICE MINE WAS DOWN FOR APPROX. 45 MINUTES DUE TO THE INSIDE CAR DOOR NOT BEING CLOSED ALL THE WAY. IT VIBRATED AND THE SWITCH OPENED.
EE WAS BUILDING A CRIB WHEN A ROCK APPROXIMATELY 3' ROUND, 0-4"" THICK FELL STRIKING HIM ON THE RIGHT SIDE OF THE FACE HITTING HIM OVER THE EYE AND NOSE.
Unloading ram jack for supply car.he was guiding end of ramjack his left hand when weight of ram jack shifted toward mine floor his hand was caught between ramjack & sharp jugged rock.size is 78""long x 12"" weight is approx. 500 lbs he was transported to gen. with avulsion of left middle finger and ring finger he was treated & released. the following day skin grafts were performed.
Ee was pulling a dead mantrip when the chain broke under stree striking ee above the left eye.
On August 27, 2004 an unplanned roof fall was discovered at the Justice No. 1 Mine. It was discovered during the weekly exam of the bleeder system. The fall size is unknown. The fall is in the # 1 entry near spad #427. Area was developed in June 2001 & was supported w/6' torque tension bolts; prop setters on a staggered 10' pattern.
THE ELEVATOR AT THE JUSTICE MINE WAS DOWN FOR APPROX. 120 MINUTES DUE TO WIRE BURNT OFF POWER POLE.
Ee was making a cable splice on the continuous miner with a utility knife. He cut his left thumb and received stitches.
EE was cutting to the head. A piece of draw rock came off the end of the drum striking member's right index finger. Employee was wearing fingerless metacarpal gloves. EE received sutures.
An unplanned roof fall occurred at the justice mine tailgate section No. 4 entry Bk 116 to 117. The fall measured 20' by 18' by 7'. The date and time of this fall is unknown. It was discovered 7/29/04.
EE WAS WALKING BY THE PANEL OF THE FEEDER AND STATED THAT HE SLIPPED/TRIPPED AND CAUGHT HIMSELF WITH HIS LEFT HAND IN THE PANEL, RESULTING IN AN ELECTRICAL BURN. EE WAS TRANSPORTED TO CAMC GENERAL AND TO WESTERN PENNSYLVANIA HOSPITAL WITH 3RD DEGREE BURNS. THERE WERE NO WITNESSES TO THE ACCIDENT.
THE ELEVATOR AT THE JUSTICE MINE WAS DOWN FOR APPROX. 270 MINUTES DUE TO THE BOTTOM DOOR SWITCH BEING GROUNDED.
Ee was picking the mantrip back onto the track when his right middle finger got mashed between the jack bar and mantrip. He returned to work on June 2, 2004 due to his 6/3 work schedule. No scheduled days were missed.
THE ELEVATOR AT THE JUSTICE MINE WAS DOWN FOR APPROXIMATELY 119 MINUTES. THE POWER WAS OFF. IT CAUSED THE ELEVATOR CALL SWITCH TO GROUND AND PUT IT IN STOP.
ON 5/20/2004 THE ELEVATOR AT THE JUSTICE MINE WAS DOWN FOR APPROX. 45 MINUTES DUE TO THE TERMINAL END WAS BURNED OFF AND GROUNDED. THIS WAS REPAIRED, BUT THE SAME PROBLEM OCCURRED AGAIN AT APPROX. 4 A.M.
The elevator at the Justice mine was down for approx 139 minutes due to the grounded tree and door switch on the bottom of the mine door.
THE ELEVATOR AT THE JUSTICE MINE WAS DOWN FOR APPROX. 112 MINUTES AND AGAIN TO CARRY OVER ON 5/19/04 FOR 360 MINUTES DUE TO GROUNDED CONTROL TREE TO THE SWITCHES ON THE BOTTOM.
On May 3, 2004 an unplanned roof fall occurred on the tailgate section in the #2 entry spad brk #121. The fall measured 50' by 20' by 20'.
EE WAS USING A CHAIN RATCHET TO PUT THE RAM ON PONTUNE WHEN THE RAM TWISTED CAUSING THE CHAIN TO COME LOOSE FROM THE HOOK. IT BACKLASHED AND STRUCK EE IN THE RIGHT FINGER CAUSING A HAIRLINE FRACTURE.
EE WAS MAKING A BELT SPLICE ON #9 BELT. HE SLIPPED AND PLACED HIS RIGHT HAND ON THE NAILBOARD TO CATCH HIMSELF, WHEN ANOTHER EE HIT HIS RIGHT THUMB WITH A FLEXCO HAMMER.
EE WAS KNOCKING DOWN A STOPPING WITH A SLEDGE HAMMER. HE KNOCKED THE TOP HALF OUT AND STOPPED TO CLEAN HIS GLASSES. HE REMOVED HIS GLOVES AND GLASSES. HE PUT HIS LEFT HAND ON TOP OF THE SOLID BLOCKS AND A ROCK FELL STRIKING HIS LEFT INDEX FINGER. THE ROCK WAS 0 - 1"" THICK, 20 1/2"" LONG AND 10"" WIDE. HE RECEIVED SUTURES.
AN UNPLANNED ROOF FALL OCCURRED MEASURING APPROX. 125' X 24' X 20'. THE FALL OCCURRED FROM FOOTAGE MARK 1850 INBY THE FACE. THE AREA WAS INITIALLY BOLTED WITH 5&6 TORQUE TENSION BOLTS, 8""&14"" CABLE BOLTS, TRUSS BOLTS W/ 10' LEGS, AND SCREEN MESH INSTALLED W/ 4' RESIN BOLTS. NO EQUIP. INVOLVED & PASSAGE OF PERSONS NOT AFFECTED. THE FALL WILL BE CLEANED UP.
EE was standing approx 20' out by the tailpiece at the face phone watching the tailpiece. He had his left hand on the monorail when a piece of rock fell striking his left hand. His hand was cut and he ceased to work and was transported to BMH to seek medical attention.
EMPLOYEE WAS CUTTING STRINGS FROM BELT STRUCTURE. HE PULLED TOO HARD ON THE KNIFE AND IT SLIPPED AND CUT HIS LEFT FOREARM. HE RECEIVED 8 STITCHES OUTSIDE OF HIS ARM AND 2 INSIDE.
EMPLOYEE'S MANTRIP HAD GOTTEN OFF TRACK. HE REMEMBERS ATTEMPTING TO JACK THE CAR BACK ON TRACK. THE BAR MUST HAVE SLIPPED, HITTING HIM IN THE JAW.
On March 19, 2004 an unplanned roof fall occurred at the justice no. 1 mine. The fall measured to be 120' by 20' by 12'. the fall was at 8HG 1-2 entry spad 618 Brk #7.
WHILE TAKING DOWN A KENNEDY STOPPING, EMPLOYEE PULLED THE LAST PANEL AGAINST THE RIB. A ROCK RIB ROLL FELL STRIKING THE LEFT SIDE OF HIS HEAD ON DOWN TO HIS ARM AND FINALLY LANDING ON HIS LEFT LOWER LEG AND FOOT.
