Employee was operating diesel mantrip on 3B supply road and a piece of rock fell struck employee's left shoulder.

Employee was walking in the last open between #4 and #5 entries on #3 unit. Rock fell out between roof bolts striking employee's right foot causing injury.

Employee was roof bolting in the face of 5R on #1 unit. Employee was in process of adding pinner steel and was struck by rock from immediate roof on right index finger causing injury.

Employee was operating #5 stinger ride on #1 unit traveling outby the last open in the #5 entry. The employee drove thru a line curtain and was struck by a rock. The rock was approximately 4'W X 5'L ranging from 1/2" to 5 1/2" thick. No indicators of adverse roof and roof support was installed IAW approved roof control plan. The rock fell out between middle roof bolts.

Miner in process of repairing roof bolter dust skirt out by face area when draw rock fell from roof striking left leg which was positioned on mine floor. Miner sustained an inline fracture of left tibia and ankle.

While reaching for a roof bolt from the top of the bolter, a small piece of rock fell from the roof striking employee on the left thumb resulting in a slight fracture.

Employee entered entry 1 to follow up on cleaning. 12 - 15' inby the xcut he heard the roof pop, looking up but not seeing anything he went on. A rock 73"x48"x4" fell from between the center bolts, knocked him to the floor and landed on his left foot, fracturing 4 toes and causing the amputation of his pinky toe.

A roof bolter operator, in the process of bolting the #3 entry on the #2 section when a rock fell from the convergence area between the rib and roof. The rock struck the miner in the left waist area causing a dislocation of the left hip.

Employee was drilling a bolt hole when a rock broke from the roof onto his drill steel. Before he could stop the rotation, the rock flew from his steel striking him in the mouth, resulting in a laceration that required 5 sutures.

While operating a roof bolting machine, the employee was swinging his boom in to install the second roof bolt for that row when a piece of rock fell from the mine roof striking him on the right leg. This accident did not become lost time until 9-15-10.

While sitting on the ground the employee heard some rock start to fall and as he turned to run the rock hit him on top of the right knee. He also twisted his knee as he tried to get out of the way.

While pulling some curtain out of the cable reel on a shuttle car a rock fell from the mine roof pushing the employee down. When the employee put his hand on the fender to catch himself another rock fell and mashed his middle finger on his right hand causing him to fracture his finger and receive 7 sutures.

While installing roof bolts a piece of rock fell from the mine roof striking the canopy on the roof bolting machine and flipped under the canopy striking the employee on the left elbow. The employee suffered a chipped bone in his elbow.

While walking over to the #9 entry a rock fell from the mine roof striking the employee on the head causing him to drop to his knees. When he dropped to his knees the employee suffered a contusion to his left knee.

While standing next to a roof bolter, a rock fell from the mine roof, striking him on the left shoulder and knocked him to the ground. The rock then slid off of his left shoulder and landed on his left hand, busting his middle, ring and small finger open. He received 20 sutures.

A roof bolter operator was installing his second drill steel into the rotation pot on the roof bolting machine, when a piece of rock fell from the mine roof and struck him on the right hand. The employee received 5 sutures.

While standing at the side of the roof bolting machine, the employee reached for his lunch bucket when a piece of rock fell from the mine roof striking the employee on the lower right leg. The employee suffered a fracture due to this incident.

A piece of rock from the mine roof located between the rib and the first roof bolt struck a roof bolter operator on the back and as the piece of rock slid off of his back it landed on his left foot as he attempted to move a roof bolter to install the next row of roof bolts. The employee suffered a fractured foot.

A rock measuring 36"x31"x1.5" fell from the mine roof, striking the employee on top of the head. This did not become a lost time accident until 5/11/09.

While the employee was bent over walking, a rock fell from the mine roof striking him on the back of the head and neck. This did not become a lost time accident until 05/08/2009.

Employee had his hands on the ATRS while checking a blown hose and a rock fell from the mine roof striking him on the left hand. The ring finger on the left hand sustained a laceration and the very tip of the finger was fractured. The employee received several sutures.

She proceeded to walk past the back of the duster when a piece of rock measuring 2ft. x 3ft. x 3in. fell from the mine roof striking her in the head, left shoulder, left hand and left ankle. She sustained a fractured left ankle.
