FMC employees were attempting to seal the hoist shaft at the Bremen Portal at Ken American Paradise Mine No. 9. Steel beams were welded in place metal decking was secured covering the shaft. Methane accumulated in the shaft and caused a series of two explosions 4-5 seconds apart. The second explosion killed FMC employee. No other fatalities or injuries occurred.

Employee was in the underground shop area picking up rockdust bags to throw in dumpster when EE struck right ear on a sprinkler head on the fire suppression system. This resulted in a laceration to the right ear causing sutures.

Employee was operating diesel mantrip on 3B supply road and a piece of rock fell struck employee's left shoulder.

Employee was operating 4 man diesel ride in reverse. Employee struck head on high voltage cable, panicked and instead of applying brake the employee floored the accelerator causing injury to neck.

Employee was operating shuttle car #34 on the #3 unit. The employee turned too early in the shuttle car causing the shuttle car to turn into the rib instead of intersection between entries 7 & 8. Restricted duty as of 6/11/18.

Employee walked thru the underground shop and did not get low enough while walking under fire suppression system and bumped head causing laceration.

An unintentional roof fall occurred in the First Southwest Mains entry seven, between crosscuts thirty-four and thirty-five. The roof fall measured 6'T X 19'W X 60'L. The roof fall will not be cleaned up and immediate area was re-supported. The roof was supported initially with 6' resin/tension roof bolts. Date of development was June 2010.
An unintentional roof fall occurred in the First Northwest Submain number eight entry, between crosscuts seventeen and eighteen in the straight. The roof fall measured 18'W X 16'L X 6'T. The roof fall will not be cleaned up and the immediate area was cribbed off and secured. The date of development was October 2017.
Employee was operating #5 stinger ride on #1 unit traveling outby the last open in the #5 entry. The employee drove thru a line curtain and was struck by a rock. The rock was approximately 4'W X 5'L ranging from 1/2" to 5 1/2" thick. No indicators of adverse roof and roof support was installed IAW approved roof control plan. The rock fell out between middle roof bolts.
IE was repairing bad splice on shuttle car in #5 entry. EE left for 10-15 min, returned & began working on splice. EE had not seen scoop in the area. With cap light & blinking light on, EE was on knees looking down. Scoop carrying supplies in elevated bucket was heading inby in #5 entry at walking speed. Scoop operator rang bell before & while passing through the airlock curtain.
Employee was in the process of attaching bull hose to the unit water tree, the water valve was inadvertently turned on the 1 1/2 inch water valve causing the bull hose to strike the employee in the mouth. The employee received a laceration to the upper lip. The employee received 8 sutures to the injury.
While roof bolting, miner complained of left knee pain and stated heard a "popping" sound. Miner was transported to local hospital, treated and released. On 08/22/2017 miner underwent surgery of unknown diagnosis therefore became a lost time accident on that date.
Miner in process of repairing roof bolter dust skirt out by face area when draw rock fell from roof striking left leg which was positioned on mine floor. Miner sustained an inline fracture of left tibia and ankle.

A roof fall occurred on a worked out section not an active section in #1 entry spad #7850. 2 pins wide, 4 to 5ft high, pulled 5ft fully grouted pins, 20ft long, and 12ft in width.
The miner was helping the certified electrician splice A 300 volt D.C shuttle car cable. The miner placed the cable on employee's leg then cut the insolated jacket of the cable with A razor knife when the knife cut through the cable causing A laceration to the miners leg.
A water leak has occurred on the #2 section in the #1 entry. The water is leaking from the mine roof and adjacent pillars of the #1 entry. No injury occurred during the water leak. MSHA hotline was utilized and district personnel arrived at the mine site shortly following the report.
Miner was pulling up 995 continuous miner cable slack with A nylon strap in the deck of A shuttle car. The miner put hammer handle through the strap, the miner had hand on the hammer head when the cable became tight the miners hand was caught between the hammer and the car deck. The miner received A fracture to the left index knuckle.
Miner was in process of assisting to make a belt splice when employee's hand slipped and was struck on the Right ring finger causing laceration and fracture of right ring finger.
Miner was using sledge hammer on coal chute inside Coal Preparation Plant when the chute was missed with hammer head. Miners hand struck chute causing fracture to Left index finger and laceration to said finger.
A continuous miner operator was injured while operating the continuous miner, when a rock fell from the mine roof and striking small left toe on the side of the foot resulting in a small toe fracture.
An accident (unplanned roof fall) occurred at this operation, between the crosscut #29 and crosscut#30 of the #11 seam. MSHA was notified within 15 minutes of the roof fall being discovered, and issued a 103(k) order upon inspection. an action plan was developed by mine management to provide adequate supplemental roof support in the area.
