Employee was bolting on the right side of the continuous miner when a piece of slate fell out of roof and struck employee in the right shoulder. Rock was 6"x8"x4".

EE was roof bolting when a 2 1/2' long rock fell from roof, striking ee's Lt arm. Even through ee was wearing all PPE, fall still caused a laceration on ee's Lt forearm that received 6 sutures. This is being reported as a medical treatment (NDL).

EE was standing behind miner on Lt side. Miner was 2' from corner. Operator called for the jacks to be set. EE walked between the corner and miner. EE hit the pie pan which dislodged a piece of rock/coal and it struck EE on the back. EE began lost days on 10/24/2014.

EE was checking dust hose at tail gate when a piece of slate fell between 233 and 234 shield, striking ee in Rt wrist. EE received 7 sutures. EE had zero lost time and returned to work his next shift.

Injured was standing along CM waiting to install a roof bolt when a rock fell between the draw slate and the coal seam measuring approximately 3 inch X 12 inches X18 inches. It struck him on the right ankle. He continued to work until 3/4/13 when seen at the SRMC emergency room. He was taken off work for a broken ankle.

As Alpine miner was cutting coal he was installing channel & after tightening bolt on channel a piece of rock apprx 20" wide X 42" long x 5"-10" thick fell from roof & struck him on the back. He was pushed into metal box that protects drill levers & cut him above lt eye & fractured his nose. 52 sutures were required to close the lacerations (2 cuts).

Holding remote for shields while moving walking shield, rock fell pinning legs against a pallet of cribs EE continued to work. Leg was still sore MRI revealed small crack, his last day worked was 12/29/2011.

While scaling down loose ribs with a slate bar he was struck on the back of the neck by a piece of draw slate that fell from the roof. He received two lacerations and they required 10 sutures to close. NO TIME FROM WORK WAS MISSED, THIS IS A MEDICAL TREATMENT INJURY.

Observing co-worker drill a hole for a rib bolt and was struck on the left knee by a rock that fell from the rib and rolled into him. The rock was 12" X 18" X 5" thick. It struck him on the left knee and caused a contusion.

Having advanced a slider tube with co-worker, he was struck on the right foot by a piece of rock measuring 2 ft X 3 ft X 6 in thick. This resulted in fracture of three areas of his right metatarsal on the right foot. Injured began losing time on 3/8/10.

While pushing a roof bolt into a hole, a piece of slate (25" x 27" x 1.5") fell from the roof and struck the bolter and broke up when it hit the bolter. A piece of the rock struck the injured just above the left eyebrow causing a laceration that required 5 sutures. No time from work was lost. This is a medical treatment incident.

After installing roof bolt, victim stepped back & a piece of shale struck him on the left foot, fracturing it. Rock measured 46"l, x 12-16" w x 0-6" thick. It fell out of roof between 3rd & 4th roof channed from face.

The injured had just scaled loose head coal from the work area. A piece of head coal fell and struck the injured on the head knocking off his hard hat. When the injured looked up to see what happened a second piece of head coal struck him in the face causing contusions & abrasions to face,forehead, & a laceration to the L side of his nose & upper lip required stitches.

Bending a loose rib bolt that was sticking out in the tram road and was struck from behind by coal and slate that fell out in three pieces from a corner in an intersection. He was driven to the mine floor. He received 8 fractures to various cervical discs and the blow resulted in a blood clot. Injuries required surgery to resolve.

Hanging canvas, injured was holding canvas up in the air as a co-worker was putting a spad into the roof. A piece of roof coal broke off in two pieces. The first piece knocked off hat and the second piece struck him on the top of the head lacerating it. The cut required six sutures. No time was lost from work.

Mine roof had potted out during the mining process. CM was backed up to add roof support. Injured was installing screen and when he put his drill steel into the screen some material fell from the roof and struck him on the right side of his head causing a laceration. Four sutures were necessary to close the wound. Injured did not miss any work.

INSTALLING A 24" VENTILATION TUBE. THE HOOK ON THE ROPE USED TO HANG THE TUBE CAME OUT OF THE HOLE ON A ROOF CHANNEL. THE ROOF CAME AWAY FROM THE CHANNEL AND THE TUBE STRUCK HIM ON THE LEFT SIDE OF THE HEAD STRAINING HIS NECK.
