Employee complained of of pain in right foot and knee. After medical treatment it was determined that the employee had degenerative conditions in right knee and right foot and required surgery on both. The EE walks six miles a day on slippery uneven bottom for last six years. this is reported as an Occupational Illness due to repeated trauma. EE worked till 08//29/2016

Employee was putting oil in a banana screen. There was a panel removed from the screen for service. The employee stepped in the hole created by the missing panel and fell and fractured three ribs.
The EE complained of knee pain. There was no specific event. The EE continued to have reoccurring pain and had an MRI on 01/20/2017 which revealed several degenerative conditions in left knee. EE went off work for surgery on 02/24/2017. This is being reported as an occupational illness due to repeated trauma.
Stepped in rut in roadway and bent knee backwards causing a knee sprain.
The employee was breaking up rock from on top of the continuous miner when a piece of rock in left eye. Employee suffered a corneal abrasion and was taken off work.

Employee was dragging a vent tube when they felt a pop in right groin area. The employee continued to work till 6/22/2016 when they were diagnosed with a right hip flexor strain and taken off work.
Employee complained of pain in elbow and was diagnosed with tendonitis in right elbow. This is being reported as an occupational illness due to repeated trauma from frequent over head lifting.
Employee slipped while walking over a coal pile and felt pain in knee. Employee is a weekend warrior and first missed shift was 05/28/2016.

The #9 Portal elevator went down at 2:15 PM and was back in service at 3:00 PM. Two counter weight wheels and a governor switch were changed. This incident was reported to the MSHA hotline at 2:45 pm.

This was originally a record only accident but the employee is now receiving Physical Therapy for neck and back pain and is being reported as a Medical Treatment accident.
The employee was removing the miner cable from a hanger when the hanger swung over and chipped one of their teeth. This was originally a report only accident. The employee has since been treated by a dentist and is now being reported as a Medical Treatment accident.
Chipping rust with air chipper, rust/dust entered eye
The employee retired on 01/04/2016. On 3/31/2016 the retiree filed an occupational lung disease claim. This is being reported as an occupational illness.
Employee was walking out of Prep Plant and stepped in a drainage ditch and sprained EE's right ankle.
The #6 Portal elevator broke down for 60 minutes with no personnel on it. It was repaired by clearing a door latch fault. It was put back in service at 5:50 pm.
Employee complained that right arm was swelling at end of shift. Went to ER and it was determined that EE was suffering from deep venous thromboembolism in the right clavicle area. This is a condition caused by repeated trauma from years of lifting overhead. This is being reported as an Occupational Illness.
Employee was hosing off the catwalk on RC-1 belt when EE stepped on a piece of coal and strained EE's left knee. EE was given an injection in EE's left knee on 3/23/2016. The employee continues to work. This is reported as a Medical Treatment Accident.
While pushing roof bolt into hole the roof bolt stopped abruptly and the employee felt pain in abdomen. The employee was diagnosed with a hernia and worked till 5/13/2016 when employee went off for surgery.

The employee was shoveling snow when EE felt a pop in EE's back. EE continued to work till 2/15/2016 when EE went off work for a herniated disk in EE's lower back.

Burning with torch. Torch came into contact with right hand.
The injured was walking across mine parking lot and slipped on a patch of ice causing a sprain right knee. The injured's first missed day of work was 1/15/2016
The employee was struck in the right cheek with a metal wedge from a wedge socket that was being used to pull belt onto belt structure. The wedge socket connection failed causing the wedge to fly thru the air and struck the injured causing a laceration and broken orbital bone.
The #6 Portal elevator went down at 4:55 pm for CSR2 fault. There was a man trapped on the elevator. Schindler Elevator was called and came out and repaired the elevator. The elevator was running again at 6:55 pm and the man got off the elevator.
The employee was walking across the track when EE tripped over the rail. The employee fell and caught self with EE's right arm and felt pain in EE's right shoulder. The employee received an injection in EE's right shoulder on 3/7/2016. This is a Medical Treatment accident.
Employee was walking on slope belt and felt pain in his right knee. The employee went off work on 2/8/2016 and had surgery on 2/17/16. An MRI revealed several degenerative conditions including degenerative changes to the meidal meniscus, a Bakers cyst and arthritis. This is being reported as a Occupational Illness.
The employee reported feeling pain in his left knee. The employee was given an MRI which revealed a torn meniscus and multiple degenerative conditions due to repeated trauma over many years of working in the mine. This is being reported as an occupational illness. The employee continued to work until his surgery on 12/23/2015.
Employee was trying to disconnect ropes on a barge when they felt pain in the right shoulder. The employee continued to work until 3/3/2016 when they went off work to have surgery.
At Surface mine. Installing 10oz.felt and 40 mill plastic felt for impoundment. He was pulling plastic down the slope and at the bottom of the slope he went down. Stepped on uneven ground which made him fall. fractured left tibia and fibula.
The employee was doing his normal job and was feeling pain in his right shoulder. He was examined and given an MRI which revealed a rotator cuff tear due to multiple degenerative conditions in his shoulder. He was taken off work 12/01/2015. This is being reported as an Occupational Illness.
Caught his left ring finger between the centralizer and the drill steel while bolting. Diagnosed with laceration and a simple fracture to the tip of his left ring finger. Received 4 sutures.
Employee was rock dusting when the rock dust hose came apart at a joint and struck EE in the face and blew rock dust in eyes. The employee was diagnosed with concussion symptoms on 11/25/2015 and taken off work.
A piece of rib (12"-6"-4") fell out and bounced off the ventilation tube and the continuous miner and then struck the injured in his left knee. The injured was bolting on a full face miner. The injured continued to work till going off for surgery on 1/27/2016.
EE WAS TWISTING A 4' PIECE OF GLUE TO MAKE A 2' PIECE. AS HE CUT THROUGH GLUE, KNIFE SLIPPED, CAUSING A LACERATION TO HIS LT THUMB. EE WAS TRANSPORTED TO WHS-GREENE, WHERE HE RECIEVED 5 SUTURES TO CLOSE THE LACERATION.
EE WAS IN A MANTRIP GOING TO HIS FIREBOSS RUN WHEN MANTRIP JUMPED RAIL AND RAN INTO THE RIB AREA. IN INCIDENT, EE RECIEVED A CRACKED RT RIB AND A BROKEN TOE. EE CONTINUED TO WORK THE REST OF HIS SHIFT. EE CAME INTO SAFETY THE NEXT MONDAY, 10/26/2015, AND FILLED OUT AN ACCIDENT REPORT. EE WAS TAKEN OFF WORK BY DR. UNTIL A MRI CAN BE TAKEN. FIRST DAY OFF WAS 10/28/2015.
The elevator at the #9 Portal had a malfunction due to slack in the Governor Rope. There was no one on the elevator at the time of the incident. Outside electrician was called and the elevator was up and running at 10:30PM.
EE was pulling the supply cart off the elevator on the surface and felt a pop in his right shoulder. EE continued to work until 3/22/2016 when he went off for surgery. This is being reported as an Occupational Illness. An MRI revealed multiple degenerative conditions including, moderate arthrosis, subscapular tendinosis and humerus head degeneration.
