As EE was installing roof bolt in #7 right on #1 unit, a piece of slip rock approx. 2 1/2" long x 2 1/2" wide x 2" thick fell out from above the fly board & struck him on the face, neck & his lower bicep. He received a laceration to his neck & abrasions to his bicep & face. He left in an ambulance & returned to work his next shift. This is being reported as a medical incident.

EE stated that he was bolting in eight right, when a rock fell from the top and hit his right shoulder and right hip.

EE stated that he was putting up a bolt in the face of #7 entry as he touched the top with his drill steel, a rock fell from the top, striking his upper arm. The EE suffered a laceration that required sutures.

Employee was spot bolting and left his hand on tray of bolter. A piece of rock fell on his left hand.

A piece of rock fell (about 20" long, 12" wide, 1" thick) and some coal hit him. He was building an intake stopping. NOTE: We are suspicious of this alleged accident. There were no witnesses and no visible marks on his body.

WHILE SHOVELING 5 SOUTH BELT, A ROCK FELL FROM THE MINE FOOR & STRUCK HIM.

A ROCK FELL FROM THE MINE ROOF FROM BETWEEN BOLTS & STRUCK EMPLOYEE WHILE HE WAS CLOSING THE DUST BOX ON THE ROOF BOLTER.

VICTIM WAS STANDING BESIDE ROOF BOLTER WHEN A PIECE OF ROCK FELL FROM BETWEEN ROOF BOLTS AND COAL RIB STRIKING HIM.
