The employee stated that he bent over to lift the handle of the tracking and communication cart and when he felt pain in his lower back. On 1-15-14 this incident was deemed a medical reportable because the employee received medical attention in the form of an injection and diagnosed with a sprain.
A roof fall was discovered at approx. 11:10p.m. in the 2 West Mains travel way. The fall occurred in the #3 entry of Spad 1062 and measured 40' long X 20' Wide X 8' long.
The employee stated that he was helping to construct a stopping when he struck his right index finger with a hammer. The finger was splinted as a result of the incident.
The employee stated that he was walking by the power center and tripped over the continuous miner cable. As the employee fell, he stuck his hands out to try and brace his fall. However, his left hand struck a metal ear on the outer edge of the platform, puncturing employee's left hand, resulting in three sutures.
A roof fall was discovered at approximately 2:00 pm in the 2 West Mains primary escapeway. The fall occurred at break 15 in the #6 entry of the 2 West Mains. The fall was approximately 35' long, 18' wide and 6' high.
EE was setting up to sheave belt to lap in preparation for a belt move. During this process, he was pulling the sheave cable up the entry beside a parked scoop. As he pulled the cable, he twisted & struck his left elbow on the parked scoop's canopy. The EE sought medical attention & was given antibiotics for infection because he had struck the bursitis sack in his elbow.
The slope hoist was removed from service due to a coupling needing replaced. Once repaired, the hoist was placed back into operation.

A roof fall was discovered in the return air course between the East Mains Seals and Return Shaft 1 break outby spad 211. The fall was located in an intersection and was approximately 50' long x 20' wide x 8' thick. The fall will be dangered off.
The employee stated that while he was in the process of dropping miner cable and water line that was hung from the mine roof by a rope hanger. The hanger hook caught between his fingers as he tossed the cable to the mine floor. The hook contacted the webbing between his index and middle finger causing a laceration The employee sought medical attention and received 6 sutures.
The employee stated he was standing in front of the RBM cable when the reel engaged which caused him to fall backwards onto his back. After further consideration and because the insurer found the injury compensable, this is being reported as a lost time injury.
At approximately 7:00 p.m. the slope hoist experienced an electrical malfunction which prevented it from functioning normally.
A roof fall was discovered in the South Mains located at break number 5. The fall was approximately 20' long and 5' thick at the highest point. The fall occurred in number 6 entry. The fall was dangered off.
The employee stated he was attempting to dislodge a rock that was fouled in the chain of a shuttle car, and while pushing the rock over the bumper of the car the rock cantilevered upwards striking his right index finger between the rock and mine roof. This resulted in an avulsion to the tip of the employees right index finger that required 11 sutures.
The employee stated he was lifting muddy line curtain and felt pain in his left shoulder. The employee sought medical attention and received physical therapy, but did not miss any work. After further consideration, this is being filed as a medical reportable.
During the examination of a weekly aircourse a cont. of an old fall was found. The fall occurred in the Ellis Main at break 27 and now extends from #5 entry to #4 entry. The travelway was not affected.
The EE stated that while dismounting the rear of the 404 roof bolter, he stepped into a small hole of water that contained a hidden piece of rock. While dismounting, he stepped onto the rock with his right foot. The rock was slicken sided which caused his foot to slip off the rock twisting his ankle.
A roof fall occurred in #7 Entry of the 2 West Mains panel above anchorage. The fall was located at spad 1109.
The employee stated that he was operating a shuttle car when the car slid into rut causing his head to contact the shuttle car canopy. The employee had recently returned to work due to a medical issue with his neck, but stated he felt pain. After further consideration and because the insurer found the injury compensable, this is being reported as a lost time injury.
The employee stated that he and two other employees were lifting miner cable to place in hangers outby the face of #5 entry when he felt pain in his back. The employee sought medical attention and was diagnosed with arthritis and returned to work. After further consideration and because the injury was compensable, this is being reported as a lost time injury.
The employee stated he was bolting in #7 entry and had been experiencing back pain for a week after bending a bolt. The employee stated he was being treated for a back injury through a previous employer at the time of the injury. After further consideration and because the insurer found the injury compensable, this is being reported as a lost time injury.
On 4-18-13 a roof fall was discover in the intake aircourse on the 2 South Mains at spad 1107. The intersection fell above anchorage that this location up to 6' high.
On 4/8/2013 a roof fall was discovered 150' inby the mine fan in the intake air course. The fall measured approximately 30' in length and fell up to 7' in height.
The employee has been awarded 10% permanent partial disability for occupational pneumoconiosis

On March 25, 2013 employee was awarded 10% pulmonary function impairment attributed to occupational pneumoconiosis.
EE was fatally injured while installing a rib bolt in the 8 Right Crosscut on Unit 1 when a slick-n-side formation fell from the mine roof inby the ATRS of the bolt machine, struck the mine floor and protruded into the employees work area. The employee was struck against the back with the rock and caught between the drill pot/mast area of the Fletcher DDR-13 WT he was operating.
As EE was installing roof bolt in #7 right on #1 unit, a piece of slip rock approx. 2 1/2" long x 2 1/2" wide x 2" thick fell out from above the fly board & struck him on the face, neck & his lower bicep. He received a laceration to his neck & abrasions to his bicep & face. He left in an ambulance & returned to work his next shift. This is being reported as a medical incident.
Employee has been diagnosed with 5.5% permanent partial disability due to hearing loss.
IE was assisting another with a roof bolt.IE was holding support EE was securing it with a bolt,he placed his right hand on the mast.Once EE installed bolt he dropped the drill pot smashing IE's fingers in the mast.EE suffered an amputation of his right middle finger just below the second knuckle and an amputation of the right ring finger just above the nail bed.
The EE stated he was walking beside the parked CM when he stepped on the miner cable and his left foot slipped of the cable causing his right leg to land awkwardly and his knee twisted. EE started to miss work to the injury on 1/31/13 and RTW on 2/4/13.
The EE stated he was walking along #2 East Belt when he slipped in a hole of water and strained his left knee. EE started missing on 1/30/13 and RTW 2/1/13.
EE stated he was walking by the scoop and stepped in a hole causing him to twist his right ankle. EE finished his shift on 1/28/13 and reported for work on 1/29/13 but was unable to work the full shift. EE sought medical treatment and was placed off work for a strained knee.
EE stated that he was bolting in eight right, when a rock fell from the top and hit his right shoulder and right hip.
The employee sought medical attention for the onset of back pain on 12-13-12 and on 1-2-13 medical information provided to the operator denotes degenerative changes to the lumbar region.The ee stated that he felt pain in his lower back while loading roof bolt glue.
EE stated that he was putting up a bolt in the face of #7 entry as he touched the top with his drill steel, a rock fell from the top, striking his upper arm. The EE suffered a laceration that required sutures.
The employee stated, while traveling to the section a roll of plastic mesh entered the cab of the mantrip and struck him in the back of the head. The employee suffered a cervical strain and contusion.
A roof fall occurred at break 10 of the 1 South Mains. The fall occurred in the track entry. The area was dangered off.
EE reported to mine's compensation provider that he was struck by a block that fell from a stopping on 8/21/12. EE did not report the incident at the mine & was laid off on 8/22/12 during a work reduction. The EE sought medical the day he was laid off and diagnosed with a strain. This incident report is being filed to abate citation 8153546 as a medical reportable incident.
During the preshift examination of all underground workings after a 7 day power outage a roof fall was discovered in the 1 South Mains track entry from break 28 to 29. The fall was 5 ft thick and 95' long. The fall was reported at 9:15 a.m.
Employee was installing a set of doors with 2 other employees at the bottom of the slope. 1 door was in place & they were working on the 2nd door. Door was blocked in place & 2 EEs raised 1 side causing the door to creel pinching employee's finger. The accident resulted in a laceration requiring sutures. He returned to work his next shift. Further investigation is ongoing.
At 4:00 am the slope hoist was removed from service due to an electrical issue in the hoist panel that occurred after a power outage. The hoist remained out of service until repairs could be made.
At 7:15 pm the Spice Lick elevator was taken out of service due to mechanical issues.
At 11:57 a.m., the slope hoist began having issues with the electrical system and was taken out of service and the mine was evacuated.
Injured was using a cutting disc with 4" grinder. Grinder kicked back and cut injured's left forearm.
The employee stated that he was helping move a roof bolter from #3 entry to #2 entry when the cable reel engaged and caused the RBM cable to strike him in the back of his head. Employee started missing work on 4-20-12.
A roof fall occurred on the #1 Section #4 entry break 15 while retreat mining. The area affected included the intersection to the push-out that had just been mined.
At the 3:38pm the spice lick elevator was taken out of service due to an electrical issue.
The employee stated he was hanging curtain in 4 cross cut left when a piece of rock fell from between the first bolt and the rib striking the employee in the top of the head. The employee did not begin missing work until 3-14-12.
