The employee stated that he bent over to lift the handle of the tracking and communication cart and when he felt pain in his lower back. On 1-15-14 this incident was deemed a medical reportable because the employee received medical attention in the form of an injection and diagnosed with a sprain.

The employee stated that he was walking by the power center and tripped over the continuous miner cable. As the employee fell, he stuck his hands out to try and brace his fall. However, his left hand struck a metal ear on the outer edge of the platform, puncturing employee's left hand, resulting in three sutures.

A roof fall was discovered at approximately 2:00 pm in the 2 West Mains primary escapeway. The fall occurred at break 15 in the #6 entry of the 2 West Mains. The fall was approximately 35' long, 18' wide and 6' high.

EE was setting up to sheave belt to lap in preparation for a belt move. During this process, he was pulling the sheave cable up the entry beside a parked scoop. As he pulled the cable, he twisted & struck his left elbow on the parked scoop's canopy. The EE sought medical attention & was given antibiotics for infection because he had struck the bursitis sack in his elbow.

The employee stated that while he was in the process of dropping miner cable and water line that was hung from the mine roof by a rope hanger. The hanger hook caught between his fingers as he tossed the cable to the mine floor. The hook contacted the webbing between his index and middle finger causing a laceration The employee sought medical attention and received 6 sutures.

The employee stated he was attempting to dislodge a rock that was fouled in the chain of a shuttle car, and while pushing the rock over the bumper of the car the rock cantilevered upwards striking his right index finger between the rock and mine roof. This resulted in an avulsion to the tip of the employees right index finger that required 11 sutures.

The EE stated that while dismounting the rear of the 404 roof bolter, he stepped into a small hole of water that contained a hidden piece of rock. While dismounting, he stepped onto the rock with his right foot. The rock was slicken sided which caused his foot to slip off the rock twisting his ankle.

The employee had recently returned to work due to a medical issue with his neck, but stated he felt pain. The employee stated he was being treated for a back injury through a previous employer at the time of the injury. EE was fatally injured while installing a rib bolt in the 8 Right Crosscut on Unit 1 when a slick-n-side formation fell from the mine roof inby the ATRS of the bolt machine, struck the mine floor and protruded into the employees work area. The employee was struck against the back with the rock and caught between the drill pot/mast area of the Fletcher DDR-13 WT he was operating.

As EE was installing roof bolt in #7 right on #1 unit, a piece of slip rock approx. 2 1/2" long x 2 1/2" wide x 2" thick fell out from above the fly board & struck him on the face, neck & his lower bicep. He received a laceration to his neck & abrasions to his bicep & face. He left in an ambulance & returned to work his next shift. This is being reported as a medical incident.

IE was assisting another with a roof bolt.IE was holding support EE was securing it with a bolt,he placed his right hand on the mast.Once EE installed bolt he dropped the drill pot smashing IE's fingers in the mast.EE suffered an amputation of his right middle finger just below the second knuckle and an amputation of the right ring finger just above the nail bed.

EE did not report the incident at the mine & was laid off on 8/22/12 during a work reduction. The EE sought medical the day he was laid off and diagnosed with a strain. This incident report is being filed to abate citation 8153546 as a medical reportable incident.

During the preshift examination of all underground workings after a 7 day power outage a roof fall was discovered in the 1 South Mains track entry from break 28 to 29. The fall was 5 ft thick and 95' long. The fall was reported at 9:15 a.m.

Employee was installing a set of doors with 2 other employees at the bottom of the slope. 1 door was in place & they were working on the 2nd door. Door was blocked in place & 2 EEs raised 1 side causing the door to creel pinching employee's finger. The accident resulted in a laceration requiring sutures. He returned to work his next shift. Further investigation is ongoing.

The employee stated he was hanging curtain in 4 cross cut left when a piece of rock fell from between the first bolt and the rib striking the employee in the top of the head. The employee did not begin missing work until 3-14-12.
