THE INJURED EMPLOYEE WAS OBSERVING THE INSTALLATION OF AN ADJUSTING JACK TO THE SIDE OF A BELT DRIVE BOOM. THE BOOM SLIPPED ON CRIB BLOCKS AND FELL APPROX. 8". THE EMPLOYEE CAUGHT HIS LEFT HAND BETWEEN THE BOOM AND A CRIB BLOCK (FRACTURED 3RD & 4TH METACARPALS).

WHILE EE WAS RETRIEVING A HYDRAULIC COUPLING FROM HIS TOOL BOX HE WAS STRUCK BY A ROCK CAUSING HIM TO FALL FORWARD AND STRIKING HIS HEAD AGAINST THE TOOL BOX. THE EE SUFFERED LACERATIONS TO HI S FOREHEAD WHICH REQUIRED SUTURES FOR CLOSURE.
