Installing cement mill plates. Half of the line was installed and the cement mill was rotated in order to install the rest of the line. When the mill was rotated some plates fell, the contractor employees entered the mill for scaffolding installation. While installing the scaffolding, the plates fell impacting a contractor causing right ankle and rib fractures.

THE STONE HIT HIS HEAD CAUSING PAINS IN THE HEAD, NECK, AND SUPERIOR PART OF THE SHOULDERS, BACK AND CHEST.
