Un atascamiento en la boquilla #3 de la BC-M cuando encuentra que tiene unas piedras obtruyendo, una procede a utilizar barra para desalojar la piedra. Realizando un tipo de apalancamiento ejerce presión sobre la barra en direccion hacia el cuando de momento esta se va en blanco, lo que hace que la mano derecha choque con la pared del bajante.

Employee was removing a cement bag that was lodged on a conveyor belt and when the bag came loose it caused cement to blow onto employee's face falling in EE's eyes. Causing redness and inflammation to EE's right eye and eyelid.

Employee alleges that the employee was placing an iron tripod to place a cell phone to record and the legs of the tripod and the base were sapped, impacting the left hand, causing fractured nails and bleeding in the index and middle fingers.

Employee was handling a bag of ice and scraped their left pinky at the cuticle area with a wire used to close the 40lbs bag. They notified on 02/09/2024 that they felt pain and has redness and swelling at the area. Antibiotics and vaccine were prescribed.

Employee was cleaning the cabin of the excavator at the end of the shift. While standing on the wheel chain, EE slipped and impacted their left hand with the inside of the cabin door dislocating EE's index finger.

Fall down stairs

An employee was walking down a flight of stairs and tripped with own feet on the 2nd to the last step. EE stumbled down the remaining steps and hit hand with the wall in front causing a radial fracture.

A pallet jammed in palletizing machine B when the employee tried to remove the pallet a piece of the pallet broke off and EE's right hand hit the pallet above causing pain and swelling.

Employee stepped on a piece of wood with an exposed nail. The nail punctured EE's safety boot and also punctured EE's foot.

Over-exertion in lifting objects

Miner was allegedly filling a bucket and then emptying it on a locked out conveyor belt. After approx. 18 times their wrist bent backwards when raising the bucket. Miner had pain and limited range of motion. Miner was taken to ER where X rays were performed and found no fractures. Miner was discharged and given pain medication.

An employee was measuring silo levels, when the measuring tape got caught & employee used a long bar to move the part that was stuck. When the tape was freed, employee's right hand moved hitting the side of the silo opening causing a laceration in employee's right little finger joint. Employee received 4 sutures.

A cement silo overflowed and a cement dust cloud formed causing cement dust to fall in an employee's eyes causing irritation and redness.

Employee alleges EE was reclining pallets on a metal column, a burst of wind pushed one of the pallets and it hit the back of employee's left calf causing a laceration that required stitches.

When the employee placed right foot on the ground right ankle was twisted because ee placed foot where the sidewalk ends and the grass begins. The employee was holding the door and a laptop computer and was not holding on to the railing.

Employee was cleaning a conveyor belt's transition bellow the conveyor pushing accumulated material with a bar, the bar was pushed by the material causing it to slip, the employee tried to grab the bar and it pinched left middle finger with a ladder causing a cut.

Installing cement mill plates. Half of the line was installed and the cement mill was rotated in order to install the rest of the line. When the mill was rotated some plates fell, the contractor employees entered the mill for scaffolding installation. While installing the scaffolding, the plates fell impacting a contractor causing right ankle and rib fractures.

An employee reported on 6/26/2017 felt pain while getting out of car, EE stated it was not work-related. On 7/12/2017 EE stated that EE had been in a squatting position cleaning a grating floor on 6/24/2017 and felt back pain when EE stood up. Injury had no lost work days until Nov 2018.

The employee proceeded to remove a truck starter, which has an approximately weight of 60 pounds, alone, but it slipped and jabbed the left hand ring finger against the concrete; causing an open wound of approximately one inch in length. Stitches were took on the finger. The employee will return to restrictive work.

The employee was installing bricks inside the kiln and adjusting them with a brick hammer. During the process, he hold a brick with his left hand and finger hammer the tip of the index finger. First Aid was provided in the dispensary and was sent to a Hospital, where four stitches were taken to the base of his left index finger nail. The employee returned to work.

EE attempted to dislodge a paper bag which was pinned in the packing machine. When opening the top to gain access to the pinned bag the electrical power automatically shuts down as a safety measure but apparently the air lines maintained pressure causing the rake to move foward when he was removing the bag causing him an open wound.

Three employees were dislodging the terciary air duct at the second floor of the Preheater Tower. When they opened the duct gate hot material flushed away. At the same time EE was passing underneath the duct when the hot material reached his feet.

Employee was operating a forklift at the Packing House area. Suddenly he saw a defective cement bags pallet inside the palletizer. He entered inside the palletizer avoiding the safety devices used to stop the machine. The pallet continued its movement pinching accidentally his index finger between the pallet and a conveyor roller.

EE was installing an air slide in the Packing House. He passed from the stair to the vibrator's structure to reach the chain of the hoist. The vibrator's structure was energized due to a broken ground wire located inside a metal enclosure. EE received an electrical shock when he touched part of the metal structure. The ground was fixed but few wires were broken
