Injured was trying to lift a spool of wire rope on the bus when she felt a pain in her lower back, right hip, and leg.

Injured was moving a roll of cable. Injured bent over to block the cable roll and felt pain in his back. (Injured did not request any medical treatment until 07/11/07).

Injured was removing a 8" I beam from a flat car. As the beam was being positioned to be taken off the flat car, one end of the I beam fell, striking injured on the top of his right foot. This resuted in a fracture to the metarsal bone in his foot.

Injured was in the belt slope drive house preparing the take a conveyor belt roller down the belt slope. As injured placed the roller end on a stair step in the drive house, he caught his finger between the roller end and the metal step. This resulted in a tip fracture and laceration to the 1st finger on his left hand. Injured was wearing gloves.

Injured was pulling a high pressure hydraulic hose over the stageloader when he felt a pain in his back & leg.

INJURED HAD LIFTED A TOOL BOX LID UP TO GET ROOF DRILL BITS. THE LID FELL, CATCHING INJURED'S RIGHT HAND BETWEEN THE LID AND THE BOX.

INJURED WAS CLIMBING A LADDER TO CLEAN LIGHT FIXTURE IN BATHHOUSE. INJURED FELT PAIN IN HIS LEFT BICEP WHILE CLIMBING.

Building braces for belt line. I-beam slipped, pinning left hand between two I-beams.

Injured was delivering belt structure. As he was removing a piece of belt structure from the scoop blade he felt pain in his lower back.

Injured was helping take slack out of longwall face chain. The chain flight that was secured to the dogs broke causing the chain to strike injured on his right ankle.

Injured was changing a 3/4 high pressure hose on 108 shield. Injured had gotten under 109 shield and was going to function 108. When the tunnel lock was removed in the area where the injured was working the shield automatically pulled up pinning him between a shield power supply and the pan line.

Injured was passing a water hose to co-worker. The hose nozzle struck a metal beam and came on suddenly. The sudden water pressure forced the nozzle backward striking injured in the chin.

Injured was struck in the head by a clevis pin which broke while trying to extract a continuous miner from a pillar fall. Injured was unconscious for about 30 seconds.

Injured was using a water hose to wash debris to bucket elevator in dump. Injured stepped on wet coal/rock causing his foot to slip. When foot slipped, injured fell backward casuing the water hose to come up and strike him on the left side of his face. The hose lacerated his left cheek requiring three stitches.

INJURED WAS HELPING HANG HIGH VOLTAGE CABLE. THE CABLE SLIPPED FROM THE HANGERS AS IT WAS BEING RAISED. THE WEIGHT OF THE CABLE PUSHED INJURED RIGHT HAND BACKWARD RESULTING IN A SPRAINED WRIST.

injured and co-worker had loaded a batam rock duster into the bucket of a scoop. injured was pulling the rock duster toward the back of the bucket. the rock duster moved suddenly catching his left thumb between the frame of the rock duster and the push ram in the scoop bucket resulting in a broken thumb. injured was wearing gloves.

Inj employee and co-worker were stacking monorail. as injured stacked the monorail his glove became caught on the monorail end. the monorail being stacked pulled his hand between it and another piece of monorail resulting in a fractured finger.

Injured was unloading 8" steel pipe from car. Co-workers were pushing pipe to end of car. Injured hand was between the end of the pipe and the end of the car. The pipe trapped injured left hand between the pipe and car resulting in a fracture to the back of the hand.

INJURED EMPLOYEE WAS CHANGING A DRIVE BELT ON #8 WEMCO DRYER. INJURED WAS ATTEMPTING TO ROLL THE DRIVE BELT ONTO THE PULLEY. THE PULLEY WAS BEING TURNED BY HAND. THE PULLY TURNED, CATCHING INJURED LEFT LITTLE FINGER IN A PINCH POINT BETWEEN THE HOUSING AND THE PULLEY RESULTING IN AMPUTATION OF END OF LITLE FINGER.

injured was lifting wood crib blocks to build crib in longwall shield recovery stall when he felt pain in stomach muscles.

Injured was removing a rock from the shearer. The rock shifted and cut injured's hand.

INJURED WAS INSTALLING A 8 FOOT RESIN ROOF BOLT. INJURED WAS SPINNING THE ROOF BOLT TO MIX THE RESIN. THE PLATE ON THE ROOF BOLT TURNED. INJURED REPOSITIONED THE PLATE WHILE SPINNING THE BOLT. THIS RESULTED IN A LACERATION TO HIS RIGHT INDEX FINGER. 6 STICHES WERE REQUIRED.

INJURED WAS CLEANING THE CANOPY TOP OF LONGWALL SHIELD. A ROCK FELL WHILE INJURED WAS PULLING ROCK FROM THE CANOPY. THE ROCK STRUCK INJURED ON THE RIGHT HAND RESULTING IN A FRACTURE AND LACERATION.

Injured picked up a bag of pull chains in his right hand and swung the bag over his right shoulder. He felt a pain in his lower back while swinging bag over shoulder. He then fell striking his right wrist on pallet lying on floor.

Injured was cleaning rocks out of the jackline. He lifted a rock to put in the conveyor when he felt a pull in his right groin. The rock was approximately 2 feet by 2 feet by 18 inchs thick. Injured was on his knees at the time of the accident.

Injured was rehooking a piece of plastic pipe to his motor to pull outside. When he lifted the pipe up to hook it to his motor he felt a pain in his lower back.

EE'S FOOT SLIPPED WHEN CLIMBING OUT OF OLD POWER CENTER BEING WORKED ON. HE GRABBED A COME-ALONG HANGING AT POWER CENTER. HIS RIGHT THUMB WENT INTO THE HOLLOW BACK OF THE HANDLE AND WAS LACERATED.

Injured was attempting to lift a rock up on the belt line when he felt something in his back slip.

INJURED WAS PICKING UP A SHIELD CLEVIS THAT HAD FALLEN OFF A PONTOON WHEN HE FELT A PAIN IN HIS BACK. (INJURED DID NOT REPORT ACCIDENT UNTIL NEXT DAY.)

INJURED WAS HELPING PULL SLACK UP IN THE MINER CABLE & WATER HOSE. A WATER HOSE COUPLING CAUGHT ON THE HANGER. THE TENSION CAUSED THE ROLLER CABLE HANGER TO BE PULLED DOWN JUST AS THE INJURED EMPLOYEE WALKED INTO THE "V" TO CHECK IT. THE COUPLING STRUCK THE INJURED ON HIS HARD HAT AND PUSHED IT INTO HIS SAFETY GLASSES WHICH RESULTED IN A LACERATION TO HIS RIGHT LOWER EYE LID.

Injured was dragging a belt roller out of the way. As he was dragging the roller the hooked end caught a track tie causing injured to twist his back.

While attempting to lift out a bottom roller on the slope belt a roller fell out of a hanger and struck the injured on the top of his left foot.

Injured was helping to pull a joint of 3/4" air line and ran his head into a cable bolt, bolt was approx. 4" out of the top, area was 60" high.

Injured was picking up a bundle of roof bolt plates. The plates were wet and slick. She lost control of the bundle of plates she tried to catch them before they slid out of her hand. Her wrist was bent backwards.

Injured was riding in a jeep when the trolley pole came off of the wire. When he went to put the pole back on the wire he felt a pain in his back.

Employee was pulling a piece of curtain away from a stopping which dislodged a cinder block. The block fell 5 feet striking the injured on the top of his right foot.

Employee was crossing under a piece of water line and raised up and struck his head on a piece of ventilation tubing. This caused pain in his neck and back.

INJURED WAS CLOSING THE ELEVATOR DOORS AND CLOSED THE DOOR ON HIS LEFT FOREFINGER

EMPLOYEE WAS REMOVING A COVER FROM THE ROOF BOLTER TO WORK ON THE CABLE REEL. AS HE SLID THE COVER OUT OF THE WAY, THE COVER BEGAN TO SLIP OFF OF THE MACHINE. HE GRABBED THE COVER AS IT WAS SLIDING OFF AND TWISTED HIS BACK. SHORTLY AFTER THE INCIDENT HE BEGAN TO HAVE SPASMS IN HIS BACK AND WAS TRANSPORTED OUTSIDE.

slack was being taken out of face conveyor chain. the chain was anchored with wedges. pressure from the chain flights on the hold down metal caused the metal to break allowing the flight to strike injured on the left leg.

TWO EMPLOYEES WERE LOADING A LARGE VALVE INTO A MAN BASKET. THE VALVE DROPPED INTO THE BASKET, ROLLED OVER AND MASHED ONE EMPLOYEE'S LEFT PINKY FINGER.

INJURED EE WAS HELPING PUT TOP BELT ROLLER FRAMES ONTO BELT STRUCTURE. AS CO WORKER PUSHED THE DROLLER FRAME INTO POSITION FROM THE OPPOSITE SIDE INJURED RIGHT INDEX FINGER WAS CAUGHT BETWE EN THE ROLLER FRAME AND THE BELT STRUCTURE.

EE WAS HANGING LINE CURTAIN. SHE CLIMBED A LADDER WITH A PIECE OF CURTAIN AND STRAINED HER HAND AS SHE LIFTED THE CURTAIN.
