Injured was removing a 8" I beam from a flat car. As the beam was being positioned to be taken off the flat car, one end of the I beam fell, striking injured on the top of his right foot. This resuted in a fracture to the metarsal bone in his foot.

injured and co-worker had loaded a batam rock duster into the bucket of a scoop. injured was pulling the rock duster toward the back of the bucket. the rock duster moved suddenly catching his left thumb between the frame of the rock duster and the push ram in the scoop bucket resulting in a broken thumb. injured was wearing gloves.
Inj employee and co-worker were stacking monorail. as injured stacked the monorail his glove became caught on the monorail end. the monorail being stacked pulled his hand between it and another piece of monorail resulting in a fractured finger.

THE PULLY TURNED, CATCHING INJURED LEFT LITTLE FINGER IN A PINCH POINT BETWEEN THE HOUSING AND THE PULLEY RESULTING IN AMPUTATION OF END OF LITLE FINGER.

INJURED WAS INSTALLING A 8 FOOT RESIN ROOF BOLT. INJURED WAS SPINNING THE ROOF BOLT TO MIX THE RESIN. THE PLATE ON THE ROOF BOLT TURNED. INJURED REPOSITIONED THE PLATE WHILE SPINNING THE BOLT. THIS RESULTED IN A LACERATION TO HIS RIGHT INDEX FINGER. 6 STICHES WERE REQUIRED.
INJURED WAS CLEANING THE CANOPY TOP OF LONGWALL SHIELD. A ROCK FELL WHILE INJURED WAS PULLING ROCK FROM THE CANOPY. THE ROCK STRUCK INJURED ON THE RIGHT HAND RESULTING IN A FRACTURE AND LACERATION.
Injured picked up a bag of pull chains in his right hand and swung the bag over his right shoulder. He felt a pain in his lower back while swinging bag over shoulder. He then fell striking his right wrist on pallet lying on floor.

THE TENSION CAUSED THE ROLLER CABLE HANGER TO BE PULLED DOWN JUST AS THE INJURED EMPLOYEE WALKED INTO THE "V" TO CHECK IT. THE COUPLING STRUCK THE INJURED ON HIS HARD HAT AND PUSHED IT INTO HIS SAFETY GLASSES WHICH RESULTED IN A LACERATION TO HIS RIGHT LOWER EYE LID.
Injured was dragging a belt roller out of the way. As he was dragging the roller the hooked end caught a track tie causing injured to twist his back.

EMPLOYEE WAS REMOVING A COVER FROM THE ROOF BOLTER TO WORK ON THE CABLE REEL. AS HE SLID THE COVER OUT OF THE WAY, THE COVER BEGAN TO SLIP OFF OF THE MACHINE. HE GRABBED THE COVER AS IT WAS SLIDING OFF AND TWISTED HIS BACK. SHORTLY AFTER THE INCIDENT HE BEGAN TO HAVE SPASMS IN HIS BACK AND WAS TRANSPORTED OUTSIDE.
slack was being taken out of face conveyor chain. the chain was anchored with wedges. pressure from the chain flights on the hold down metal caused the metal to break allowing the flight to strike injured on the left leg.
TWO EMPLOYEES WERE LOADING A LARGE VALVE INTO A MAN BASKET. THE VALVE DROPPED INTO THE BASKET, ROLLED OVER AND MASHED ONE EMPLOYEE'S LEFT PINKY FINGER.
INJURED EE WAS HELPING PUT TOP BELT ROLLER FRAMES ONTO BELT STRUCTURE. AS CO WORKER PUSHED THE DROLLER FRAME INTO POSITION FROM THE OPPOSITE SIDE INJURED RIGHT INDEX FINGER WAS CAUGHT BETWE EN THE ROLLER FRAME AND THE BELT STRUCTURE.
EE WAS HANGING LINE CURTAIN. SHE CLIMBED A LADDER WITH A PIECE OF CURTAIN AND STRAINED HER HAND AS SHE LIFTED THE CURTAIN.
