IT WAS DETERMINED THAT THE VICTIM WAS MISSING SHORTLY AFTER 10:00 PM. A SEARCH BEGAN WHICH CULMINATED IN THE VICTIM'S RETRIEVAL FROM THE ""A"" RAW COAL SILO AT APPROXIMATELY 7:55 PM ON MARCH 15, 2001. IT IS INCONCLUSIVE AS TO EXACTLY WHERE THE VICTIM ENTERED THE RAW COAL SYSTEM. CONSEQUENTLY AN EXACT CAUSE OF THE ACCIDENT WAS NOT DETERMINED.

Dropping gate on grizzly and extra rocks fell out onto grizzly causing more weight and employee pulled back.
Employee was kicking at an obstruction with EE's left foot and struck EE's knee against the spill board of the 24 belt head. The employee was able to perform EE's work and was scheduled for an MRI on 12/12/18 and was kept out of work until EE sees an orthopedic doctor and is therefore off from 12/12/18
Employee was walking in the pump house when employee slipped and fell injuring back, neck and head.
Employee was walking down the steps and stepped onto coal accumulations when EE slipped and fell landing on EE's back.
Employee felt a pain in lower back while shoveling at the S-6 belt landing.
Employee was removing a rock from the #59 Rock Chute on the 5th Floor of the Prep Plant when the rock gave way and the momentum caused the employee to fall onto back from ground level.

Employee has shown a Reportable Standard Threshold Shift after an annual hearing exam.
Employee was struck in the back by loose material and driven down into a pile of spillage.

Employee was adjusting the drive belts on unit 98-A pump. He was using a pipe wrench to tighten the motor adjustment bolt on the drive side. The pipe wrench slipped catching fingers between the wrench and drive belt guard causing a hairline fracture to the left middle finger.
A maintenance employee was standing on a temporary work platform, four feet off of the ground, cutting bolts loose from a bracket that was holding a 10"" poly pipe elbow. As he cut one of the bolts the torch flashed, startling him and he jumped off of the platform. As he landed on the concrete floor, his ankle twisted sideways, causing a fracture.
Employee was tramming bulldozer to load a train at the clean coal stockpile when he backed over a wind row and felt a pain in his lower back.

Employee was backing down off a machine and slipped on a damaged iced step, falling to the ground. He twisted his right knee. Has continued to work without restrictions until now. Scheduled for surgery 12/9/15 to repair a torn meniscus in right knee. Will begin loosing time from work starting 12/9/15.
The employee was dismounting a Caterpillar D10T dozer when he tripped and fell injuring his back and neck. The mines went on two weeks vacation from 28 JUN thru 11 JUL. The injured employees first scheduled day back was Monday, 14 JUL 2014.

Employee was descending the stairs when his right foot slipped and he overextended his left arm while holding onto the handrail. Employee worked regular shifts until 2/15/2012 at which time he elected to have surgery on his shoulder.
Using grinder, metal for in right eye. Eye swelled went to the eye doctor and foreign body was removed from effected eye.
Exiting office trailer, slipped and fell down two steps. Continued working his regular shift for 10 days. Sought medical treatment 2/19/2011. At that time was put on modified duty. Diagnosis contusion, hip and shoulder. Thinking this was a first-aid treatment, I did not file a report. After receiving notice on 4/8/2011 about the referral to an Ortho doctor, I am filing this report.

EE was tigtening bolts with a cresent spud wrench, the wrench slipped off of the bolt and hit the left side of EE's chin causing a small laceration. EE completed shift and RTW the next day.
Installing a set of stairs, had to trim sheeting and steel with a cutting torch. A piece of slag popped and got behind cutting glasses and into right eye. Completed shift, sought medical attention after work hours, and returned to regular duties 11/21/10.
EE was using his hands to pull on a section of handrail to move it enough for a come-a-long chain to clear. The handrail broke loose and he fell backwards about 5 to 6 ft, EE completed the shift. No work was schedule for Monday 9/6/10 due to Labor Day Holiday. When EE returned to work on Tuesday 9/7/10 his knee was swollen and was taken to the doc and was put on light duty.

Employee was standing outside a garage door to the warehouse, when he took two steps backward and stepped on a wooden pallet and fell, fracturing his ankle.

Injured had lifted a roller into place at unit #119 dryer at Pinnacle Prep Plant. Injured worked until Jan 15, 2010 and was off work until Feb 12, 2010. Injured stated that his back was hurting prior to Dec 9, 2009 and that on Jan 15, 2010 he had lifted a 5 gallon bucket of anti freeze feeling a twinge in his back went to see a doctor. MRI showed degenerative arthritis.
Injured was holding a special tool to drive a bearing into place. Foreman was striking tool with a sledge hammer. He missed striking injured on right thumb. Causing a laceration to right thumb. Injured was transported to hospital.
Injured was carrying a 6"" flange weighing approzimately 11lbs down the strairway between the 5th and 6th floor when his ankle gave out causing him to stumble striking his face against the handrail. Injured was transported to the hospital where he was treated for a laceration above his left cheek and eye.

Injured was backing a dozer off of the clean coal stock pile when the emergency brake set causing the dozer to come to a sudden stop. The injured jerk his neck when this happened.
Clearing of rock on the grizzly on 8 1/2 floor. Rock bounced off floor struck individual in left foot causing individual to twist left knee. Claim is being contested. This claim form is being submitted to terminate an MSHA citation.
Injured was removing belt clamp from #24 belt. The counter weight was suspended and held by 2 sets of 6 ton chain hoist. Chains broke, dropping the counter weight tensioning the belt. This threw the injured upwards into the structure causing injury on left side; compression fracture to the 10th vertabra and he was fitted for a back brace.
Employee was removing bolts from equipment. While removing, he was using an impact gun in one hand and with the other hand was applying pressure to the bolt head. Bolt came free from the employee's grip and cut the knuckle of the ring finger on the left hand, requiring stitches.
Building braces for belt line. I-beam slipped, pinning left hand between two I-beams.

EE was stripping wire with a razor knife, knife slipped and cut into upper thigh on right leg. Treatment consisted of 24 stitches.
Injured was using a air hose to blow wet coal off of the dryer bed. When he turned on the air he lost his balance causing him to turn his ankle.

Injured was walking with a new air compressor and stepped across a ditch as he stepped across his left knee gave out causing him to fall forward and landing on his hands and knees.
Injured was walking from parking lot to the bath house. As he was disposing a piece of gum, the gum landed on his tennis shoe. Injured tried kicking the chewing gum from the top of his shoe by kicking his foot, injured kicked the concrete base for the thickner fracturing his right big toe.
Injured stepped down off of a dozer. As he stepped down off dozer arm his foot slipped causing him to fall backwards. As he fell he tried to catch himself with his left hand.
