Employee went over to the north side of the TM to remove a blank that had fallen out of the guides. While employee was pulling up on the blank he suddenly felt pain in his lower back.
Employee picked up core box full of core sample (25-30 lbs.) and sprained lower back
Employee was working on a Cat jack stand that was stuck in lower section of the stand. The injured employee was supporting the stand while another employee was using a pipe as a slide. The injured employee's finger accidentally went into a hole of the stand as the pipe slid into the stand striking the rt 2nd digit. The impact caused a partial amputation.
Employee was installing a 42" culvert pipe and received a laceration to his right thumb while he was positioning the pipe.

Employee was changing the tire on a skid-steer loader. As he was attempting to slide the tire off the lug bolts, he did not know that the tires where foam filled and the weight of the tires. When the rim slid off the lug bolts, the employees right middle finger got pinched between the rim and hub. Employee cut his middle finger, receiving six stitches.
Employee was cleaning up 3" ball mill balls for the floor. He put a ball into the bin and as he was reaching down to pick up another ball, the first one fell out and hit his left ring finger pinching it between the two balls fracturing his left ring finger.
Changing GET on 012 grader. After taking a wear plate off and sitting it on the ground it slipped and the edge cut ee hand between thumb and index finger on left hand. He received 5 stitches it happened on the last hour of the last day worked before scheduled vacation. Will not be back to work until 17th. Stitches come out on the 10th.
Employee was moving the spile remover down the track to the next bar. His right hand was between yoke and cylinder housing. Ram inadvertently retracted smashing right index finger.

While cleaning out the bed of maintenance truck employee went to pull the ladder that was between the weld machine and the truck bed, he yanked on it and felt a pull in his upper left shoulder
Employee was working in the crusher on a down day. Employee was working on a mount on an air canon. The mount on the air canon broke. Employee grabbed the canon so it would not hit the ground. As he grabbed it, it pinched his hand between the canon and the ground. Per the enclosed letter, ee had surgery 12/22/08 making this reportable.
Deplaying liner, EE was holding front corner of panel when pulley broke loose. This sent rope into his right shin, flipping and causing him to land on his right shoulder and head. The result was a bruised shin, bump on the head, and pain in right shoulder. He was discharged by dr on 4/22/08, cleared for work next day with light duty for one week/limit us of right leg/shoulder
The Crew were tripping the drill pipe out, employee grabbed the pipe (that was hooked to the winch line) and turned and swung it to the helper on the pipe truck when he states he felt a pop in his left shoulder.
EE was lifting chip tray, as he lifted the tray he felt his arm pop. The weight of the ship was 10-15 pounds,
3 maint. techs were rebuilding floor sump pump. EE tech holding wear plate, 2 EEs thread impeller in place, wear plate slipped pinching EE right index finger in between the wear plate and the pump shaft causing a laceration 6 stitches.
Employee was changing his wire in an LN-25 and the wire stuck in the first guide tube. He took the tube out and pulled the wire out with his bare hands. He is not sure how the wire went through his finger.
Employee was opening valve to fill water truck. Valve was operated by a pull chain. Valve was stuck, employee gave the chain a fast pull causing slack to hit little finger of left hand.
EE was drilling, while drilling he was looking out the door at the drill cuttings. He turned to sit down and shut the door. When he shut the door, he shut door on his right hand, cutting two fingers.

Employee was getting a part to install on a new haul truck. He picked up the part and he smashed his finger between the part and the handrail.
Employee & co-worker were organizing the laydown yard, including stacking 16" culverts. EE was lifting from one end of the 20 ft culvert while the co-worker was lifting from the opposite end. While lifting, EE felt pain in his lower back. He reported the accident to his Supervisor the next day 6-13-07 but did not seek treatment until 6-22-07. Diagnosed with lumbar muscle strain.
Installing right tire on Komastu WA 1200 front end loader, lock ring that was being installed, slipped hitting the employee on the left index finger, severing it.
When taking lock ring off of tire and drop lock ring to clean mud from it. It bounced and when it hit the ground the second time it twisted in hand and pinched left little finger cutting it (7 stitches)
EE was picking up wood, turned to throw the wood into the bucket of the loader, twisted his knee on the left leg.
Putting metal shelfs together. The person on the other side was pushing his end in place, shelf slipped and corner was bent down when it slipped it hit his right leg cutting it.

EE was stepping up onto service truck back bumper when he felt a popping noise... couldn't go up or get down. Lt ankle sprain or torn ligament.
Employee was working on water truck rear end in a tight spot. When work was done his leg was stuck. He pulled on his leg to free it. When he pulled it popped. An MRI in July showed a torn meniscus in his right knee. Operation on 7/27 made this a medical reportable, light duty.
While retracting the stabilizer arm on a light plant, it slid quickly pinching the employees finger.
WHILE FUELING EQUIPMENT, OPERATOR VIOLATED POLICY & REMAINED ON THE DECK WHERE DIESEL SPRAYED INTO HIS EYES WHILE FUELING.
While mechanics were changing the ripper shank, it got stuck. The operator grabbed the ripper trying to free it. When it came loose, there wasn't enough tension of the lifting line and it fell onto the operator's hand lacerating his finger.
Employee alleges that he injured himself while lifting/twisting machine parts. Employee alleges conditions had worsened over a short period of time. The employee stated that there was no specific incident that precipatated the injury.

Employee was removing side cover from hitachi 5500 shovel. Cover dropped down striking inside of right forearm.
Employee moving cable for BE drill, dropping cable trees, right foot was in coiled up ropes, rope was being pulled by truck and took employee with it. Broke lower right leg-cast & crutches for approx. 6 weeks on light duty.
EMPLOYEE WAS CHANGING A BULB OUT ON A DROP LIGHT. HE INSTALLED A 12V BULB INSTEAD OF THE 110V AND THE BULB EXPLODED CAUSING DEBRIES INTO HIS RIGHT EYE

EE pinched finger between dumpster and forklift waile attempting to open lid on dumpster.
WHILE MOVING SOME EMPTY OXYGEN BOTTLES, EE EXPERIENCED PAIN IN THE GROIN AREA. HE WAS DIAGNOSED WITH A HERNIA BUT CONTINUED HIS FULL JOB DUTIES PRIOR TO GOING IN FOR SURGERY ON 5/13/05.
EMPLOYEE WAS CLEANING SNOW & DEBRIS FROM THE BOX OFF OF A SERVICE TRUCK. GROUND CONDITIONS WERE SPOTTY & SNOW PACKED.
EMPLOYEE WAS SHOVELING BY HAND AND BACK STARTED TO HURT. PAIN MOVED UP BACK INTO HIS NECK. HE WAS LAID OFF 12-20-04. WENT TO DOCTOR ON 12-21-04 AND NOTIFIED ME ON 12-21-04.
Employee was changing GTE on rubber tired dozer, EE failed to follow procedures and failed to secure it before loosening. GTE fell and struck ee on left leg and ankle.
