While he was building a stopping he was getting a block off the pallet and one fell off hitting his right thumb. (He returned to work on his next shift 7-20-09. This was a medical report only.)
He was attempting to throw a set of steps over a hand rail, the steps came back and struck him on the right wrist, causing a laceration that required 8 sutures.
Injured and co-worker were unloading 8 inch aluminum pipe from a scoop. As the co-worker picked an end of the pipe up injured's left middle finger was caught between the pipe and scoop smashing the finger tip. He sustained a left distal phalanx fracture and began losing time from work on 07/02/09.
Injured began losing time when he was diagnosed with a torn left rotator cuff tear and surgery was necessary to repair it. He is an underground mechanic and the repetitive motion necessary to perform tasks for this job contributed to it. He began missing time on 7/27/09.
Four men were hanging miner, loader, bolter cables and 1 1/2" hose, when he felt pain in left shoulder. He continued to work at his regular duties, but also continued to have pain. He eventually was taken off work for left shoulder surgery on 11/13/2009.
While the employee was putting belt structure in on a belt move, he felt something pull in his right groin area.** Employee started missing work on 6/3/09.
