Employee was shoveling ore leakage from under a conveyor belt. He has been treated for a non work related back since May 2012. He shoveled 3-4 shovel fulls when he pulled a muscle in his lower back.

Employee lifted a small table that was being used as a weight to hold the cardboard down. When he swung with the table in his hands to his right to set it down, he smashed the back of his right hand and upper ring finger into a metal tray shelf that was attached to the wall.

Employee bagged 50 bags at 50# each on a pallet. He said on his last bag he felt a sharp pain from his groin to his stomach. He went to the emergency room. He is excused from work at least until his next doctors visit on 1/21/2011.

EE was shoveling a powder type ore with a shovel and by pulling it with a solid rake. He did this for 30-45 mins. The shift ended, then he called in the next day. He went to the emergency room, then made a doctors appointment. Has a pain in his lower back, hip and left leg.

EE was packaging 50# bags. He felt a tightness in his back. We had a Save a Back class on 8/11/10. EE decided to do some exercises that he had learned in the class. At that time he got a sharp pain in his right side/lower back. He was unable to finish the day. He has a doctor's appointment in one week.

Employee drove a small endloader up to a spill at the east side of Granusol. It had rained hard all day. Employee stood on a wet surface as he turned to put a shovel full of ore in the loader bucket. As he turned, he twisted his body, then slipped. He did not fall down but pulled a muscle in his lower back.
