Over-exertion in pulling or pushing objects

Employee pulled pallet jack manually into position instead of taking an extra step to use forklift to move pallet jack or ask for assistance. Suffered groin strain to lower right side.

Employee was stripping scrap copper wire with a razor knife. The knife slipped and cut his left wrist. The laceration was repaired with sutures.

EE reported to me that on 5/31/11 he strained his back while bagging. He reported this on 6/2/11. He was sent to the medical center for evaluation. He is scheduled for massage therapy with ice and medication. EE is to be reevaluated 3-4 days.

Employee and co-worker were dumping drums into a blender feed hopper. They were using a metal pipe to hit the side of the drum in an effort to soften the material. While co-worker was swinging the metal pipe, the pipe hit the side of the hopper and then struck employee's hand.

Injured and another employee were replacing a 10 ft x 3"" angle. While putting it in place it slipped out of his hands pinching his finger between the angle and cement wall.

Was getting some empty sacks to fill, stepped back and fell over a skid with a small sack on it.

He was helping replace a packer. He bent down to pick something up and hit his neck on a super sack fill pipe when he stood up.

Injured EE and another EE were trying to remove a bearing with a puller and a hydraulic pump, when the puller slipped and hit EE in the head and shoulder. EE received 4 stitches, he did have his hard har and safety glasses on.

Was helping maintenance change screen. Maintenance had unit in air, not rigged proper and not slid out of way. Lid broke and fell 4 inchs and hit his arm.

EE WAS OPERATING ROTATING DISH PELLETIZER. IN ANEFFORT TO CHECK PELLET SIZING HE SAMPLED DISH DISCHARGE PELLETS INTO LAB TEST SIEVES. AS HE SEPARATED THE TEST SIEVE, THE SIEVE CONTACTED THE ROTATING DISH. THE DISH PULLED THE SIEVE OUTOF HIS HAND CUTTING OPEN HIS INDEX FINGER.

2 EES ATTEMPTING TO PLACE STEEL TUBE LEGS ONTO STEEL FLAT PLATE FOR WELD WITH A FORKLIFT. THE LEGS DID NOT LINE UP, SO EE TRIED TO LIFT AND PLACE. THE STEEL LEGS WERE TOO HEAVY AND HE DROPPED THEM ONTO HIS WRIST.

WHILE TRAINING A NEW EE, EE LIFTED A FEW 50# BAGS. HE IS RESTRICTED FROM LIFTING MORE THAN 20 LBS. HE DID NOT ADHERE TO HIS DR.'S RESTRICTION AND DID NOT FOLLOW CO. INSTRUCTIONS. HE REAGGRAVAT ED A PRIOR INJURY.

EE TRAVELED THE WRONG WAY WITH A FORKLIFT AN CONTACTED A SHEET OF ICE. AS A RESULT THE FORKLIFT LOST CONTROLL AND HIT A PLANT TRAILER. DURING THIS COLLISION EE INJURED HIS RIGHT HAND.
