The blaster was working with anfo on the anfo-rig. Blaster states the equipment they were utilizing was blowing dust significantly more than usual. They became short of breath with chest tightness. Employee was brought out from underground where they continued to have breathing difficulty.

During removal the wash unit tipped from the telehandler forks and guard on washer unit caught the mechanics glove cutting EE's right middle finger and scraping stomach. Mechanic was taken to the hospital and received 4 stitches in EE's right middle finger.
The employee was loading anfo into holes on the face when they stepped backwards causing a strain to their L knee.
Employee was moving lumber outside the training room. They bent down to move a 2x4, as employee was getting up said they felt sudden pain in lower back.
An employee was helping offside and was moving bolts to the bolter and dropped the bundle on their left foot.

The employee picked up a large rock to move it out of the underground road. While tossing the rock, EE dropped the rock on L foot. The rock struck EE's foot just behind the steel toe guard, causing a fracture to the L great toe.
A cover door on an AD-30 was lifted to access the transmission as the employee leant down to inspect the area. The door fell down crushing both hands, breaking right thumb.
Employee was loading powder and pulling up on a anfo hose when EE felt a pop in their shoulder.
Employee was moving cribbing when the crib employee grabbed dislodged another that fell onto employee's thumb. This resulted in a break in the tip of employee's right-hand thumb.
The injured employee was overseeing a trailer being unloaded. The chain snapped and caught IE's ear. IE was wearing all of their required PPE. The injury was minor and required just a few stitches.
While reaching to open an air door manually with EE's right hand, the EE felt a sharp pain in EE's R upper arm and EE heard a tearing sound. The EE sustained a right dorsal bicep rupture that required surgical repair.
The employee was replacing the rear axel pin in the scaler when the axel moved, pinching Left middle finger resulting in a laceration and fracture.
While preforming clean up in the con-shed, the EE pulled on a piece of plywood to remove it from the shed when EE felt a "pop" to the Left lower abdomen/groin. EE was diagnosed with an inguinal hernia.
They employee was assisting the Geologist with surveying an up hole, when employee lost grip on the survey instrument. The geo moved out of the way, but the injured employee attempted to catch the falling survey instrument and it struck employee in the upper right quad of the abdomen, causing broken ribs, a pneumothorax, and a liver laceration.
Hanging and bolting full line of 6" HDPE pipe on roof. Pipe resting on pipe roller shifted and fell on miner's left index finger causing miner to receive medical attention and 7 stitches on left index finger.
Employee was closing the door to the panning station when EE's Left index finger was caught between the door latch and the door frame causing a laceration that required #6 sutures to close the wound.
Employee was checking on a haul truck door that had been reported hard to open and jamming all day. While attempting to open the door, the door handle broke and with the force of opening the door, the broken edge caused a 1/2 in. laceration to the employee's right pinky finger. As a result of the incident, employee received 5 stitches.
Employee was assisting in removing a oxy cylinder from the back of a tractor when their right hand got caught between the tractor and cylinder. Employee suffered a laceration to the right palm requiring 4 stitches as a result of the incident.
The machine was getting the tire removed, the tire was being slid off the rim. When the tire was released from the hub, the tire spun slightly on the slings and crushed the employees hand between the tire and the frame of the machine causing bruising to EE's right wrist. Employee was given medical treatment, a splint was applied and EE returned to work with no restrictions.
Employee was helping transport pipe from trailer to the rig floor. EE was assisting with one end of the pipe while the other end was being lifted with the elevator. EE's left thumb was caught between the pipe and the rig floor. EE sustained a laceration to left thumb. "The EE was transported for treatment of injury and following medical evaluation, was placed on light duty."
EE was working with the utility crew in 454-1 taking down waterline. As EE was taking the clamp loose the pipe separated unexpectedly, falling and catching EE's right hand between the pipe and the rail of the tele-handler basket. Employee was given ice by safety then taken to the doctor to have hand examined. X-Rays were negative but was given <5lbs lifting restrictions.
EE went to grab and remove last lug nut of LHD tire when the load shifted and the wheel pinched finger between the lug nut and wheel.
Employee sustained an avulsion injury and crush injury to EE's Right 5th finger while attempting to remove a bolt from the anfo tank on the blasting rig, when the bolt slipped smashing the employees Right 5th finger.

Low back strain-R side-occurred while pulling/jerking on pallet jack lodged under pallet, felt a sharp pulling in lower back. Not immediately reported, thought they could work through it. To medical first aid at 3PM (ice/massage/heat/biofreeze) advised rest on reg. sched. Days off. Sched. With MD on 11/05/2018 when pain cont/ Placed on light duty with pending f/u.
Employee was engaging the feed on hydraulic drill to begin drilling when the feed knob broke and sprayed. Employee had eyes washed by safety, then went to ER later to have eyes washed again. Follow up with eye doctor on 06/08/2018 where RX written for drops and ointment.
Employee was hanging escape markers, bending metal strapping when metal broke and lacerated R thumb.

while ee tying shoes a basket came loose from the hook on the bench and fell to its lowest point. An item attached to the basket struck the employee in the head resulting in a laceration and swelling
When climbing into cab of truck felt pull in abdomen.
EE set up a hand pump press with one end against the left loader frame and the other end against the right lift cylinder pin. EE had hand pump engaged with pressure on the pin. The pin did not move. EE climbed down off the machine to get a drink and as he walked toward his truck, he heard a pop noise and he turned around to see what the noise was the pin ejected out, hit his head.
Employee bent over to pick up wheel chock from under tire and twisted wrong when standing up
Mill maintenance worker while installing a plastic wear liner experienced a pinch/cut to his left hand.
Felt burning and stinging in abdomen while lifting a heavy pump
EE was taking the kelly rod off. Due to the drill mast being rolled to one side, the kelly rod fell off the rear mast roller, smashing his middle finger on his right hand between the kelly rod and the side of the mast. Even though gloves were used it still split his finger on the inside first knuckle. First aid was given. Hand was washed and bandaged.
Repairing a water swivel when his hand slipped and water swivel landed on his hand
