Showing all 16Employee was lifting a bucket containing an aggregate product sample for QC testing onto pick-up truck bed. While lifting, EE incurred a thoracic strain, resulting in restricted work duty.
The rod that holds an access door open failed and the door was not otherwise secured. The door fell and struck a technician in the head. In turn EE's head hit struck the machine and it cut EE's forehead and knocked out a tooth.
Employee injured EE's right hand while operating the CR-6 swing diverter gate at the Finishing Plant. Contact with adjacent handrail resulted in a contusion on the back of the right hand.

Changing rollers on C-1 tail pulley. EE laid the roller flat and then pulled it out, felt a sharp pain in middle of his back. EE continued to hold roller to keep it from falling and then set the roller down. Pain was in the middle of his back, between shoulder blades. EE was sent to the clinic where he was x-rayed and diagnosed with a muscle strain and placed on light duty.
EE was lifting a wheel flange with another teammate and felt a sharp pain in his back while lifting the flange, then he finished the job and then filed the work injury.

Rock being transported to dump area was lodged in truck tailgate. When employee was attempting to remove the tailgate he injured his fingers.
EE was removing a drive pulley guard when it shifted and pinched his finger between the guard and the support leg causing a laceration. EE received several sutures.
EE's front tooth is broken when the hanging bin control push button box he reaches for slips from his hand and swings back striking the tooth.
It had been raining all day. At the time of the accident the rain was very heavy and oxygen bottle was slippery. The injury that resulted was a lacerated/smashed finger tip on right hand ring finger when attempting to load new oxygen bottle in mechanic's truck.
EE was assisting a customer close the truck tailgate when a rock fell from bed striking EE's shin causing laceration requiring 3 stitches.
While using a porta-power and torch to a seized pin while changing out the lift cylinder of a Euclid R-40 haul truck, the pin shot out while under pressure and struck the safety glasses lense of the employee, causing the frame to make a laceration just under his left eyebrow.
Employee was fitting together pieces of a receiving trough and pulled muscle in chest area. Note: employee did not seek medical treatment until 01/27/2012 and did not advise our company until 01/30/2012.
