While transferring grease from one barrel to the other...the barrel slid and cut the forearm of an employee.

Working on the track of the skid loader with a crowbar. Got hand (baby finger) in harm's way and crushed / cut finger --- requiring 2 stitches

EE's front tooth is broken when the hanging bin control push button box he reaches for slips from his hand and swings back striking the tooth.

Employee was collecting a test sample of stone and lost control of the sample tray she was holding. The tray came down on her wrist, pinching it against a metal railing. A permanent access platform has been installed for easier access and will be modified to reduce the risk of injury.

Employee was using a Porta-Power tool which slipped from it's location and struck him in the nose and face.

Employee's hand became pinched between a manlift basket and a crusher flywheel.

Employee was ""striking an arc"" with one hand and holding the welding helmet with the other hand. He did not properly use the welding helmet and received ""welder's flash."" Employee is being trained on proper welding techniques to prevent any further injuries.

EE WAS UNLOADING AN OXYGEN CYLINDER FROM THE BUCKET OF A SKID LOADER ONTO A TRUCK. THE SKID LOADER HAD BACKED UP, BUT THEN PULLED FORWARD AGAIN WHEN THE OPERATOR SAW THAT THE EE AND CYLINDER WERE ABOUT TO FALL. EE'S LEG GOT CAUGHT BETWEEN THE SKID LOADER BUCKET AND THE TRUCK.

EMPLOYEE OPENED AN INSPECTION DOOR APPROX 24"" X 24"" IN SIZE, IN ORDER TO SEE WHY THE MACHINE WASN'T OPERATING PEOPERLY. WHILE WORKING ON THE MACHINE, THE UNSECURED DOOR FELL ON HIS RIGHT HAND AND WRIST.

WHILE GRINDING ON A CONE TOP, A HOT SPARK CAME DOWN BETWEEN HIS SHEILD AND LANDED IN HIS EYE. THE PEICE OF METAL BURNED INTO EYE AND HE COULDN'T RELIEVE THE PAIN.

Struck by falling object

WHILE STANDNG ON A 8'LADDER INSTALLING CURVED SHEET METAL AND PANEL. THE PANEL WAS BEING LOWEREDINTO PLACE, LINE UP BARS WERE INSTALLED AND PANEL COULD NOT BE HANDLED BY HIS END, IT SLIPPED CO MING DOWN AND STRUCK HIM ON RIGHT ARM.
