Injured was taking out the garbage. When he pushed down on the garbage can liner to compact the contents a tin soup can lid penetrated the bag cutting his hand.

Employee was holding on to the drive end of a pump while another employee was turning the pump impeller into the shaft. The employee's fingers got pinched when the impeller seated down on the shaft.

2 employees were removing a guard. Turned left to place guard on floor grating and felt a pop in his mid back. Body mechanics was the contributing factor of the injury. The guard was small and light weight.

Injured was removing a bad magnet by hand from rev 1 lower deck with help from a co worker. They lost control of the magnet. Injured attempted to try and regain control causing what was believed to be a strained shoulder. Injured didn't feel medical attention was necessary at the time. Note: 4-8-15 notification was given that the injured would undergo surgery for a torn labrum.
