EE and partner were setting up a jig to assist in fit up of the ring to the duct work. EE was holding the piece low to assist in getting the jig welded to the duct. EE's partner stuck EE's weld rod to tack up the piece and rod got stuck. When EE went to remove the rod, the piece fell and landed on EE's pinky fingertip which caused a laceration. EE did have gloves on.

Showing all 37EE was assisting spreading a telehandler forks, while they tilted mast down to facilitate tine adjustment, the right tine was moved, after EE began sliding the L tine outward. During this process, the R tine slid back unexpectedly. EE attempted to reposition the R tine w/EE's gloved L hand, EE L middle finger got pinched between tine and mast's lower bar. Skin tear on L finger.
Cable pull, picking up cable for refeeding into vault. Cable was being fed into vault and pulled out by hand to get it prepped for next pull into building from this vault.
EE first said EE had been experiencing soreness for sometime but no specific event occurred, then EE changed EE's story and now claims EE lost EE's footing a bit while climbing a column, and noticed soreness later that day. However, EE failed to inform anyone until Sunday, March 29th.
The injured worker states that EE has right elbow pain. Today, while adjusting the pipe bender with a coworker, EE felt/heard a "pop" in EE's right elbow. There is no appreciable bruising at this time. There is no numbness/tingling into EE's hand or fingers. There are no open wounds
Removing bolts from larger bolt, 45# bolt slid down onto EE's left thumb/hand.
Doubler plate became dislodged from it's rigging and dropped, pinching the workers fingers between the doubler plate and the steel under it.
Forklift brought in a piece of 3'x15'x1' rubber, 3 people lifting together, EE bent down to lift it up and felt pain in back while starting to lift.
While worker was flying in a section of steel into place, the steel slipped down catching the workers right thumb between another piece of steel.
Employee was working on a loader and the employee bent over at the waist to pick up a socket that was laying on the ground. When he stood up he felt a sharp pain in his back.
While changing out rollers on a belt line, EE struck a sharp piece of metal with his right forearm, resulting in a cut requiring 6 stitches. Collins returned to work the same day.
EMPLOYEE WAS OPENING FAN DOORS AND ONE OF THE DOORS SLAMMED ON HIS RIGHT ARM BRUISING THE ARM
EE WAS REPLACING A LEAKING COOLANT HOSE AND THE HOSE CAME LOOSE AND SPRAYED HIS RIGHT HAND WITH OPERATING TEMP COOLANT
Try to lift 4 inch water with water still in it. EE either strain himself or rupture his self.
While pulling at a hose, EE stated that he had hurt his back. The preliminary diagnosis was a muscle strain.
Hanging miner cable & water line. Felt a pop in middle of back & felt sharp pain.
Employee stated that while fueling equipment, he was dragging the fuel hose in the mud when he felt a sharp pain in his lower abdomen. He did not go to the doctor until 3/30/2011. He stated that he had a Hernia but did not and has not had it repaired. This is a Partial Permanent Disability.
While jacking up a 777 rock truck the fitting in the 100 ton ram (jack) came out of ram hitting employee in the head & chest knocking him unconscious.
While helping unload belt structure pulled a muscle in left arm and chest area.
EE was unloading tools off green service trk and was putting a vise on another service trk. He dropped a vise on left hand, smashing the ring finger.
MECHANIC WAS CHECKING SQUEALING NOISE ON JOB FOREMAN TOYOTA PICKUP. HE WAS SPINNING THE FRONT RIGHT WHEEL WITH HIS LEFT HAND. WITH HIS THUMB ON THE RIM HIS GLOVE CAUGHT SOMETHING ON THE BACK AND PULLED HIS THUMB INTO ONE OF THE HOLES ON THE RIM. HIS THUMB STOPPED ON THE BRAKE CALIPER AND THE WHEEL KEPT TURNING. HIS THUMB WAS MASHED AS A RESULT.
EE was unhooking a cable from a coal truck that was stuck, the cable was twisted and when it was released, it came back and hit EE in the upper part of his left eye.
EE was helping hang a mine curtain in #2 entry.
While changing a level wine on #15 car, EE and another EE were helping lift level wine. He strained his lower back.

The ring finger on his left hand was pinched between two pieces of metal. When screen was rotated around, finger was caught. There was no conditions that contributed to accident.
EE was pulling on metal pole on the highline supply sled when the weld broke hitting him in the face.

EE was manufacturing a guard and guard slipped and smashed right middle finger. X-ray showed that the finger is hairline fracture.
Was hanging up mier cable when he felt a sharp pain between his shoulder blades.
Employee was removing the water pump from a D11 dozer and the pump slipped out of his hand, catching his hand between the water pump and frame.
Changing batteries on scoop. Cut top of hand and got battery acid in the cut along with rust.

When getting into service truck, EE reached for the grab rail to pull himself into the truck. When he did he felt a sharp tear like pain in his right shoulder. EE did not mention this incident until a few days later when he sought medical attention.
