Struck against stationary object

Employee was performing maintenance on a GMC 645 service truck. While in the process of walking around the front of the service truck EE inadvertently stepped on a rock causing EE to ""roll"" EE's ankle, fracturing the right ankle.

Over-exertion in pulling or pushing objects

The employee was helping the mechanic remove the left front steer tire from a Volvo Hydro seeder, while they were sliding the tire off the lugs the employee aggravated a previous work related injury to their left shoulder.

Struck against a moving object

Injured employee was Operating a Caterpillar 777 haulage truck when the loader operator that was loading EE's truck dumped a large rock into the truck that ""jarred"" the truck. Employee was jarred and felt pain in their lower back.

Dismounting truck and slipped on mud on the cat walk and fell bruising left knee on the cat walk of the truck.

While employee was exiting/dismounting excavator employee stepped down on step and felt a ""pop"" in left knee.

Employee was helping clean snow and ice out of the dump trailer with another employee when EE slipped on the ice and caught self. While catching self from falling, EE felt a pull in the lower right side rib area.

While attempting to manually line a piece of drill steel up with the head, the deck wrench pivoted catching the employee's left big toe creating a caught between, in or on accident.

While on mine property, a security guard working for Guardco Security LLC shot and fatally injured a trespasser. This accident is being investigated by WV State Police.

Fall from machine

Employee stepped on D11R track and opened right side engine compartment door in order to repair air filter housing lid, employee stated that he does not know if he stepped back or lost balance when he opened the door, but he fell between the push arm and the track and landed on the tilt cylinder mount bracket on his left side.

Struck by flying object

EE was putting a new ball stud in a sterring jack, While striking it with a hammer a piece of metal flew off and hit him in the left eye. EE was taken to PMC Hospital immediately after the accident, and then was transported to Hospital in Morgantown.

Employee was working on a loader and the employee bent over at the waist to pick up a socket that was laying on the ground. When he stood up he felt a sharp pain in his back.

While employee was changing bits in the cutter head, the hammer he was using hit the chisel and slipped off and struck the back of his hand. He was holding the chisel in his left hand, the hammer was in his right.

The employee was dismounting the D-75 Cat dozer via the right side push arm. The employee had inadvertently stepped on a rock with his left foot, at which time he fell to the ground and felt pain in his left knee. Employee stated he maintained 3 points of contact while dismounting.

Operator was dismounting WT-10 Caterpillar water truck when he slipped on step and fell backwards striking his right hand on the ground breaking the fifth bone in his hand was treated and released from hospital.

EE fell from a Caterpillar 777-D Rock Truck. It appears that the employee was transferring his dinner bucket from one rock truck to another from the operator's deck to the next rock truck off side fender that was 4ft. away. He over extended himself and slipped and fell between the two parked trucks to the ground. EE has 3 broke ribs and brusing on his left side.

The employee was operating a 777 cat trk on the haul rd when he stated he hit a bump in the rd. At which time he felt a pinch in his lower back and tingling. The employee had stated that he had previously had problems with his back when in the military in the 1990's

Employee hit his left elbow on a gear while working on a drill carousel, grinding on a rotation gear. He noticed swelling in his elbow while driving home.

The employee was operating a 777 rock trk when the trk bounced and the air ride seat bottomed out. At which time the employee experienced discomfort in his lower back.

Mechanic stepped out of truck to work on lights on D-32. He stepped on a rock rolling his left ankle.

EE WAS REPLACING A LEAKING COOLANT HOSE AND THE HOSE CAME LOOSE AND SPRAYED HIS RIGHT HAND WITH OPERATING TEMP COOLANT

EE was leaving the mt top area after fueling the equipment, leaving the mt top ee lost control of the fuel truck coming down the hill causing the truck to roll over on to its side

EE was opening gate for traffic slipped on gravel as he fell he was still holding on to the gate. EE felt pain in his right shoulder.

EE was running roller on strip and when he out of the roller at the end of the shift. He said his back was hurting him badly and couldn't hardly walk to hid truck. EE had a knot come up on his back on lower left side.

Workers were trying to find the problem with the power. One worker was at the substation resetting the ground fault monitor. Three other workers were standing with the IE while he was working on the ground fault monitor. IE called out and 2 co-workers pulled him off.

While jacking up a 777 rock truck the fitting in the 100 ton ram (jack) came out of ram hitting employee in the head & chest knocking him unconscious.

EE was on the right side front fender going to cat walk by the cab step and hit his head on the miner bracket.

EE was unloading tools off green service trk and was putting a vise on another service trk. He dropped a vise on left hand, smashing the ring finger.

Boom on loader kicked out hitting bed rail of trip 67 causing driver to hit his head.

EE was unhooking a cable from a coal truck that was stuck, the cable was twisted and when it was released, it came back and hit EE in the upper part of his left eye.

EE was using a hammer and chisel to remove u-bolts from the leaf spring of the mantrip. The contact surface of the chisel slipped off the head of the hammer causing the chisel to strike him in the mouth chipping off part of a tooth.

This was an illness. No conditions contributed to the illness. Employee was removed from site by ambulance, experiencing chest pains. Possible heart attack was initially feared.

Flash burns (electric)

Employee was energizing the 995 breaker on a generator for a High Wall Miner. The breaker arced and flash burnt his left arm from wrist to his elbow. 99% of the injury was first degree, with a minimum amount of blistering. Upon investigation of the incident, it was found that a loose wire had fell down and crossed over one phase of the breaker which caused the arc.

Emp. was dismounting D9T Doz. when his Rt. foot slipped off a step & fell back on the track. Step was wet due to early rain fall. He bruised his Rt. hip.

Mechanic was cutting back rubber on 777 transmission harness to put in new plug. He was using a utility knife when it cut through the rubber and sliced into his left pinky finger.

Installing lifting eyes in a D11 converter to set in hole. One of the lifting eyes broke and the other eye was unable to hold the weight and torque fell on left arm causing laceration.

EE was cleaning up a stockpile area around the outside edge and hit a rock that threw him forward hitting his right leg.

While on the Bench, wind was really strong, the wind blew dirt into eye.

EE exited his dozer, walking up tilt jack to top of blade to talk with another dozer operator. When returning to his dozer the same way, he fell off of blade and tilt jack head first, between blade and hoist jack, hitting his ribs.

Walking into security bldg at start of shift. As she stepped into bldg, she tripped and fell forward.

Removing a pin in a roller frame. He hit the pin with a hammer. He slipped causing the hammer to miss the pin and cut his finger.

992G loader operator mistakenly hit the quick drop on loader while reaching into floor board area to pick up a snuff can. The boom came down on the side of the truck jamming the truck driver; driver felt pain in back.

Employee was working on the bucket of an excavator. He dropped the bucket pin on his right hand. He smashed his right hand at approximately 10:00 pm. He went to CAMC Memorial division. CAMC x-rayed his right hand. He broke his middle finger and stoved two others.

EE was loading coal trucks. He got out of his loader and the door slammed shut pinching his finger.

Employee was servicing a Dozer. He was checking the oil level when he slipped and fell between the push arm and track hitting his knee.

Employee set up a ladder to check the grease tank inside his service truck. When he stepped out of the truck on the ladder, it slipped causing him to fall.

Using a torch to cut out master bad bolt when slag popped in and around side shield. Employee did not seek treatment until 07/24/2007 and did not inform employer until 07/31/2007.

Climbing on a 992G to check the radiation area his right foot slipped and he hit his shin on something cutting it. He could not go back and show us what he did.

Found a airline leaking on a slip joint. Pulled the line off. To trim the end off while cutting the line the knife slipped and cut his forearm. We took him to the er room where they put 5 stitches in the cut.

Loading a shot when loose AN Prill blew off of the top of the truck going between glasses & left eye, eye became irritated.

Loading in shot, large rock rolled off the top of shot and hit on loader bucket. Dislocated 4th and 5th vertebrae in operator's back.

STARTED WORKING AND EMPLOYEE WAS TOLD THAT OTHER WORKERS HAD MISS FIRED ON A SHOT. THEY HAD TO HOOK UP THE SHOT AGAIN AND EE'S WERE TOLD TO GET UNDER EQUIPMENT. ROCK WENT UNDER TRUCK AND HIT EE ON THE KNEE & ELBOW, CALLED 911.
