The injured employee was installing new HDPE piping on the pipe rack at SX. As they were pulling a section of pipe into place, they felt a sharp pain in the right shoulder.

The injured employee was re-stacking a pallet of fiber drums so he could load them on a sea container for shipment. As he was lifting on one of the drums he felt something pop in his right elbow. Employee requested to see the Dr. on 12/16/15.

The injured employee was helping rebuild a Denver pump. The pump assembly slipped out of the workbench vise and landed on the employee's left hand smashing the tip of his middle finger between the work table and pump shaft.

EE was helping hold the Johnson Glad (approx. 55lbs) in place as another mechanic was trying to thread it into the end of a cooling screw. After completing the job ee was removing his white suit and felt a pull/strain in his lower right side abdominal area. Requested to see Doctor on 4/1/15.

The injured employee was replacing the seals on the small pod. As he was positioning the west end housing back into place, a small amount of caustic cleaning solution got into his left boot causing a chemical burn to his lower leg and ankle area. (Deemed reportable on 8/19/2014)

The injured employee was working as part of the maintenance crew assembling and installing new sections of piping. A section of the HDPE pipe hung on the forks of the forklift and the injured employee reached in with his left hand and attempted to free the pipe. The flange released striking/pinching the employees left hand against the forks, fracturing his middle and ring fingers.

EE was lifting the lid on the trona railcar to prevent a vacuum from occurring the lid slipped out of his right hand striking the top of his left hand, pinching it between the lid and board used to prop open lid. EE did not request to see doctor until 3/31/2014.

The employee was taking a sample from a VOCL3 tank. As he was removing the sample bottle and snorkel from the tank he reached up to closed the valve on the bottle. As he was turning the valve he felt a pop in his right shoulder. Did not request to see Doctor until 6/25/13.

The injured employee was helping another mechanic remove bolts from the bottom of a shaker screen assembly that was propped up on its side. As the 2nd mechanic attempted to rotate the assembly to get to the next set of bolts the unit fell over and struck the injured employee across the right knee. This resulted in a right knee contusion injury.

The injured maintenance employee was helping remove old HDPE piping from a plant pipe rack. In the process of removing the pipe he broke another line containing 50% caustic soda which sprayed him on the right side of his upper body and face. As a result a small amount of the material got behind his safety glasses and into his right eye.
