At the time of the incident, the injured employee had just cut a sheet loose and was trying to pry it up, they went to stand up and missed a step, stepping off the edge of the sheet and fell through the opening from the first removed panel. The crew had previously hung self-retracting lifelines for tie-off, but the injured employee was not tied-off at the time of the fall.
The injured employee was attempting to blow argon through the VOCL3 transfer line to clean out the system's transfer pump. When they charged the line with argon, it ruptured the filter housing, spraying hot VOCL3 over the lower half of their body, burning their left ankle.
The injured employee was walking down a catwalk at the feed station and slipped on some ice. As EE fell, EE grabbed the handrail, pulled EE's shoulder, and landed on EE's right knee and elbow.
The injured employee was working on the Leach belt filter vacuum box. As EE was adjusting the lifting arm, EE slipped, and EE's left leg went under an I-beam, causing EE to twist EE's left knee.
The injured employee was thawing out a frozen water line in EE's work area. As EE was working EE's way along the line EE slipped on a patch of snow/ice.
The injured EE was exiting the hourly employee clock in building and slipped on a patch of snow/ice, causing EE to twist and fall to the ground.
Feed Operator was walking across the concrete feed pad and stepped on a rock, rolling left ankle.
EE (Plant Operator) was checking the circuit. As EE exited the vehicle and begin walking up to the stairway, EE rolled right ankle, resulting in an ankle sprain.
The EE was working on a 60"" shaker screen and as EE was installing a section ring, it slipped causing EE's left forearm to slide across the edge of the unit resulting in a laceration to EE's left forearm that required stitches.
The injured EE was helping free a frozen gate latch on the bed of a Mack dump truck. EE inadvertently placed EE's hand between the gate latch and the truck bed and as the air latch closed it pinched EE's right-hand pointer and middle fingers between the bed and gate latch.
The employee was operating a forklift in the Mill yard helping unload feed material and doing general cleanup. As EE stepped off the forklift EE rolled EE's left ankle.
The injured employee was tightening a 2"" flange and felt a pop in right elbow.
The injured employee was installing new HDPE piping on the pipe rack at SX. As they were pulling a section of pipe into place, they felt a sharp pain in the right shoulder.
The employee was using a 24"" inch pipe wrench to remove a fill spout from a shipping tank. The pipe wrench slipped off the fill spout mashing finger between the wrench and tank.
The injured employee was traveling up a staircase with a flashlight to inspect a piece of equipment, as ee reached the midway point of the stairs ee felt a pop in ee left calf muscle.
The injured employee was installing a fill valve assembly on a product tank. As the employee was adjusting the valve with a 24"" pipe wrench it slipped off the valve assembly causing employee to fall forward into the side of the tank. As employee fell forward felt a pop in right shoulder.
The injured employee was working on top of the #1 Crystallizer at MVO. As ee took a step ee lost balance and put right hand down to help steady self. EE hand flexed open a thin sheet metal joint and right hand ring finger went into/under the sheet metal cutting the tip and side of finger.
The injured employee was re-stacking a pallet of fiber drums so he could load them on a sea container for shipment. As he was lifting on one of the drums he felt something pop in his right elbow. Employee requested to see the Dr. on 12/16/15.
The injured employee was helping rebuild a Denver pump. The pump assembly slipped out of the workbench vise and landed on the employee's left hand smashing the tip of his middle finger between the work table and pump shaft.
The employee was washing down the bottom floor of her work area. She attempted to step over a rolled up wash hose that was still connected and hung her foot, causing her to fall and land on her right shoulder.
EE was helping hold the Johnson Glad (approx. 55lbs) in place as another mechanic was trying to thread it into the end of a cooling screw. After completing the job ee was removing his white suit and felt a pull/strain in his lower right side abdominal area. Requested to see Doctor on 4/1/15.
The injured employee was using a utility knife to cut/split a 1/4"" rubber hose. He was holding the hose in his left hand and cutting with his right hand. The knife came out of the rubber hose and cut through his leather glove and into his palm.
The injured employee was replacing the seals on the small pod. As he was positioning the west end housing back into place, a small amount of caustic cleaning solution got into his left boot causing a chemical burn to his lower leg and ankle area. (Deemed reportable on 8/19/2014)
The injured employee was working as part of the maintenance crew assembling and installing new sections of piping. A section of the HDPE pipe hung on the forks of the forklift and the injured employee reached in with his left hand and attempted to free the pipe. The flange released striking/pinching the employees left hand against the forks, fracturing his middle and ring fingers.
The injured employee was walking up the stairs to the area control room. The area's external process control alarm sounded, startling the employee, causing him to fall at the top of the stairs and land on his knees, arms and leg.
EE was lifting the lid on the trona railcar to prevent a vacuum from occurring the lid slipped out of his right hand striking the top of his left hand, pinching it between the lid and board used to prop open lid. EE did not request to see doctor until 3/31/2014.
The employee was taking a sample from a VOCL3 tank. As he was removing the sample bottle and snorkel from the tank he reached up to closed the valve on the bottle. As he was turning the valve he felt a pop in his right shoulder. Did not request to see Doctor until 6/25/13.
The employee was unplugging an overflow pipe in a tank with an air lance. As he was working the lace up and down the pipe he felt a sharp pain in his lower back. Did not request to see Company Doctor until 5/6/13.
The injured employee was helping another mechanic remove bolts from the bottom of a shaker screen assembly that was propped up on its side. As the 2nd mechanic attempted to rotate the assembly to get to the next set of bolts the unit fell over and struck the injured employee across the right knee. This resulted in a right knee contusion injury.
The injured employee was removing a series of bolts from a flow meter flange. As he moved his body position to reach the back of the flange he felt a sharp pain in his lower back. Which resulted in a strain to his lower back.
The injured maintenance employee was helping remove old HDPE piping from a plant pipe rack. In the process of removing the pipe he broke another line containing 50% caustic soda which sprayed him on the right side of his upper body and face. As a result a small amount of the material got behind his safety glasses and into his right eye.
The injured employee was using a shop drill press to drill holes in a piece of flat metal. During the drilling process several metal shavings had built up around the bit. The employee then reached in to remove the shavings away from the bit and as he pulled on one of them it sliced through his leather glove, cutting his left index finger.
The employee was opening a crate of shipping boxes. He removed the plastic binding straps and threw them on the ground next to the container. As he turned to walk away his feet became entangled in the straps that he had thrown on the ground, causing him to fall and land on his arm and chest.
The injured employee was placing a pump barrel on the bottom shelf. As she maneuvered it into place one of the other barrels on the shelf fell over smashing her finger between the two pump barrels.
The employee was cleaning the plant bathroom and slipped on the wet floor, striking the left side of his face/eyelid.
The injured employee was walking across the mill yard, stepped over a wash hose and slipped down.
After EE completed grinding a weld on a handrail with a 4"" grinder, he raised his face shield and a small debris particle came off the faceshield, went between his eyebrow and glasses and got is his left eye. He missed one day of work for a doctor's exam to check his eye. He was treated and he was prescribed eye drops. There is no permanent damage to the left eye.
Employee was moving/adjusting an extension ladder. He got his left wrist in between the lower and upper rung sections of the ladder, resulting in a bruise to wrist. Taken to emergency room-xray (no fracture), velcro brace, ice, pain medication given. Light duty 10/20/05 (our last work day of week), full duty 10/24/05. Physician follow up visit 11/2/05.
EE was working in the warehouse, helping reorganize & clean the inventory shelves. As he was cleaning, a pump shaft rolled across an upper shelf, fell to the floor, bounced, and then struck him across his right hand, smashing his middle finger.
EE had placed a 55 gallon barrel in a barrel inverter. As he was pulling up on the handle to tighten the chain around the barrel to lock it in place, he felt a pop in his left shoulder.
The injured employee and his partner were transporting a welding machine from the bottom floor of the MVO building to the top floor. As EE was shifting the welder to fit through an opening he smashed his left hand pinky finger between the frame of the welder and the handrail around the hoist way opening.
Employee was removing old pump part from inventory that had been discontinued. As he was removing a casing for a galigher pump from the bottom shelf it fell over. EE stated he then reached down and tried to pick up the casing that had fallen. As he was trying to lift it up he felt a sharp pain in his neck and shoulder blades.
THE INJURED EMPLOYEE WAS OPENING A 2"" WATER VALVE ON TOP OF A MIXER/SETTLER TANK. HE HAD REACHED OVER THE RAILING & BEGAN OPENING THE VALVE WHEN HE FELT A SLIGHT PAIN IN HIS UPPER BACK.
THE INJURED EMPLOYEE HAD RECEIVED A DELIVERY AT THE WAREHOUSE. UPON OPENING THE CONTAINER SHE DISCOVERED THAT THE PACKING LIST WAS UNDERNEATH THE PUMP PARTS. SHE REACHED DOWN & GRABBED THE LIST WITH HER RT. HAND & TRIED TO ROLL THE PARTS OFF OF THE LIST WITH HER LEFT HAND. AS SHE MOVED THE PARTS SHE FELT A PAIN IN HER LOW BACK, GOING DOWN HER HIP.
A PLANT MECHANIC WAS REMOVING A 3/4"" CAP OFF OF AN AMMONIA LINE TO PURGE THE SYSTEM. HE SHUT OFF THE MAIN VALVE TO THE TANK BUT NEGLECTED TO BLEED THE PRESSURE OFF THE LINE BEFORE REMOVING THE CAP. THE CAP BLEW OFF ENGULFING HIM IN A VAPOR CLOUD OF AMMONIA. THIS RESULTED IN MINOR BURNS TO HIS RIGHT HAND AND CHEEK AND INHALATION INJURIES.
SEVERAL MECHANICS WERE USING AN ELECTRIC HOIST TO LIFT A PUMP UP TO THE THIRD FLOOR OF THE MVO BUILDING. AFTER HOOKING UP THE PUMP, THE INJURED STARTED UP THE STAIRS. AS HE REACHED THE SECOND FLOOR HE SAW THE PUMP HIT AN I-BEAM AND SNAP THE CHOKER, CAUSING THE PUMP TO FALL. HE RAN BACK DOWN THE STAIRS TO CHECK ON THE MECHANICS ON THE BOTTOM FLOOR AND FELT A SLIGHT PAIN IN HIS KNEE.
EE HAD SPOTTED A RAILCAR FOR UNLOADING, AND HAD CLIMBED UP THE LADDER ON THE CAR TO SET THE BRAKE. AS HE WAS TURNING THE BRAKE WHEEL ON THE CAR HE FELT A PULL IN HIS RIGHT SHOULDER.
THE INJRED EE WAS WORKING OUT OF A CRANE MANBASKET REPAIRING A FLANGE ON A WATER COOLING LOOP. HE ASKED HIS PARTNER TO HAND HIM ANOTHER BOLT FOR THE FLANGE, AND AS HIS PARTNER REACHED OUT TO H AND IT TO HIM HE LEANED AGAINST AN ACID LINE CAUSING IT TO START LEAKING AT THE NIPPLE TO THE FLOW METER. ANOTHER MECHANIC THEN TRIED TO SHUT THE ACID OFF AT THE VALVE, WHICH CAUSED THE NIPPL
INJURED EE & HIS PARTNER WERE REMOVING BOLTS FROM A FLOW METER ON AN OVERFLOW PUMP. AS EE WAS REMOVING LAST BOLT, HE WAS HOLDING THE HEAD OF THE BOLT W/HIS LT HAND & REMOVING THE NUT W/HIS RIG HT, HE FELT A SLIGHT POP IN HIS LT SHOULDER WHICH CAUSED HIS LT ARM & SHOULDER TO FEEL NUMB. EE WASN'T IN AN IMPROPER POSITION OR USING EXCESSIVE FORCE WITH HIS LEFT HAND.
THE INJURED EE WAS DRAGGING A 2"" WATER HOSE USED TO WASH DOWN THE MILL YARD AREA. AS HE WAS DRAGGING THE HOSE ACROSS THE YARD IT CAUGHT UNDERNEATH THE WHEEL OF A PARKED FORKLIFT, STOPPING HIS MOMENTUM AND CAUSING HIM TO JERK BACK, RESULTING IN A STRAIN TO HIS LT. BICEP.
THE INJURED EE STRAINED HIS LOWER BACK WHILE INSTALL A WHEEL BACK ONTO A TRACK MOVILE THAT HE WAS PERFORMING MAINTENANCE ON.
THE INJURED EE WAS LIFTING 110 LB. FIBRE DRUMS FROM A PRODUCTION LINE AND PLACING THE DRUMS ON A PALLET. AS HE LIFTED ONE OF THE DRUMS AND SQUATED DOWN TO PLACE IT ON THE PALLET HE FELT A PULLING IN HIS GROIN AREA.
EMPLOYEE WAS AT STRATCOR TO UNLOAD. HE WAS GETTING OUT OF HIS TRUCK WHEN HE TRIPPED AND FELL. HEHIT HIS HEAD ON THE CONCRETE CAUSING A LACERATION.
THE INJURED EMPLOYEE WAS EXITING THE CONTROL ROOM. AS HE WAS LEAVING THE STATION HE STEPPED ON APIECE OF TRASH THAT WAS ON THE FLOOR, CAUSING HIM TO STUMBLE AS HE WENT OUT THE DOOR. THERE IS APPROXM A 10 INCH DROP FROM THE CONTROL ROOM FLOOR TO THE WORK AREA FLOOR, AND AS HE MADE THE STEP HE LANDED ON HIS LEFT ANKLE, RESULTING IN A MILD SPRAIN.
THE INJURED EE WAS CUTTING BAGS OF ASH FEED MATERIAL IN OUR MILL YARD, AND PULLING THE PLASTIC LINING OUT OF THE FEED STOKC PILE. AS HE WAS PULLING THE PLASTIC FROM THE STOCK PILEHE FELT A PAI N IN HIS LOWER GROIN AREA, WHICH RESULTED IN A HERNIA.
THE INUJURIED EE WAS CHANGING OUT A 5 GALLON WATER JUG LOCATED IN HIS CONTROL ROOM. AS HE WAS REPLACING THE EMPTY CONTAINER HE FELT A POP IN HIS NECK AS HE GOT THE CONTAINER CHEST HIGH, THIS P OP RESULTED IN A STRAINED TRAPEZIUS MUSCLE IN HIS LEFT SHOULDER AND NECK AREA.
THE INJURED EE HAD RELOCATED HIS KEYBOARD TO THE TOP OF HIS DESK IN HIS OFFICE. THE USE OF HIS KEYBOARD IN THIS POSITION RESULTED IN HIS WRIST BEING PLACED ON THE EDGE OF HIS DESK DURING DATA ENTRY. THE AWKWARD POSITION CAUSED IRRITATION TO THE TENDONS IN HIS RIGHT WRIST AND FOREARM.
EE WAS FUELING A FRONT-END LOADER AT THE FUELINGSTATION. HE TURNED TO WALK TO THE PUMP AND AS HESTEPPED OVER THE FUEL LINES HIS LEFT FOOT CAUGHTTHE LINE CAUSING HIM TO LOSE HIS BALANCE. AS HE FELL ALL OF HIS WEIGHT SHIFTED TO HIS RIGHT KNEEWHICH TWISTED UNDERNEATH HIM, CAUSING A STRAIN.
THE INJURED EMPLOYEE WAS REMOVING A HYDRAULIC PUMP FROM UNDERNEATH AN OIL TANK ON A HOODLOADER. HE OVEREXTENDED HIMSELF PULLING A SHOULDER MUSCLE AS HE TRIED TO REMOVE THE PUMP FROM IT'S BASE.
THE INJURED WAS CLIMBING DOWN A FIXED LADDER IN OUR PRODUCT PARKING AREA. AS HE EACHED THE BOOTOM OF THE LADDER HE MISSED THE LAST WRUNG, CAUSING HIM TO PUT ALL OF HIW WEIGHT ON HIS LEFT LEG K NEE AND LEG.
THE INJURED WAS LOOSENING BOLTS ON THE DISCHARGE FLANGE OF AN UNDERFLOW PUMP. THE BOLTS ON THE FLANGE WERE SLIGHTLY RUSTY AND HARD TO REMOVE, AS THE INJURED WAS TRYING TO REMOVE THE SECOND BOL T WITH WRENCH, HE PULLED A MUSCLE IN HIS LEFT SHOULDER.
THE INJURED EE WAS WASHING MATERIAL OUT OF A TROUGH AT THE END OF THE ROD MILL WITH A WATER HOSE. AS HE PULLED THE HOSE FORWARD IT BECAME INTANGLED IN A WODEN 2X4, HE THEN PULLED FORWARD AGAIN S WHICH CAUSED THE 2X4 TO KICK OUT AND GO INTO THE ROD MILL. THE MILL THEN KICKED THE 2X4 BACK OUT AT HIM, STIKING HIM IN THE SHOULDER AND GRAZING HIM ACROSS THE CHIN.
THE INJURED WAS INSPECTING THE LID OF A CLACINER, THAT WAS LOCATED IN THE SHOP. THE LID HAD BEEN TURNED UPSIDE DOWN SO THAT IT COULD BE INSPECTED AND NEW REFRACTORY WORK PERFORMED. AS THE INJU RED STEPPED OFF THE LID ONTO THE FLOOR HE STEPPED ON A PIECE OF BROKEN REFRACTORY CAUSING HIM TO FALL BACKWARDS. AS HE FELL HE CAUGHT HIMSELF WITH HIS HANDS, RESULTING IN INJURY TO R PINKY.
THE INJURED WAS ATTEMPTING TO PUSH 3 DRUMS OF MATERIAL BACK ON A PALLET TO KEEP THEM FROM FALLING OFF. AS HE PUSHED THE DRUMS BACK ON PALLET HE HAD TO REACH ABOVE HIS HEAD FOR THE TOP ROW AND STRAINED HIS LEFT SHOULDER.
THE INJURIED WAS REMOVING OVERSIZED MATERIAL FROM A FEED BELT. AS HE WAS PUSHING THE MATERIAL OFF THE BELT A PIECE OF THE MATERIAL THAT HAD ALREADY PASSED ROLLED BACK AND CAUGHT HIS RIGHT HAND INDEX FINGER BETWEEN THE TWO PIECES OF MATERIAL
THE INJ WAS USING A UTILITY KNIFE TO CUT PLASTICTIES OF A 55 GAL. DRUM LINERS TO ADD PRODUCT BACK TO EACH DRUM. THE INJ WAS HOLDING THE TOP OF THE ONE OF THE LINERS & CUTTING THE PLASTIC TIES OFF AS HE CUT THRU THE TIE THE UTILITY KNIFE BLADE CUT HIS LEFT HAND BETWEEN HIS THUMB & FORERFINGER
