Employee stated that EE was attempting to get a can of hydraulic oil from the oil sled. The cans were stacked 3 high, when EE pulled the top can off a second can fell from the 2nd row landing on the side of EE's right foot resulting in a fractured little toe.

Employee lifted and carried a rail motor coupler and felt a pain in shoulder. EE injured this same shoulder in January 2019 that resulted in loosing time from work. It was thought that this was a re-occurrence but diagnostic tests results received by the operator on 6/2/2020 indicated that a new tear in the bicep was found in a different location than before.

Employee stated that EE was in the process of removing coal that had built up on the side of EE's shuttle car to help EE's visibility. EE stated that when EE pushed the coal into the shuttle car EE's right forearm was cut on a sharp edge of the sideboard caused by wear and tear.

condition was damp. area was 19 feet wide and 6 feet 10 inches high. EE, and 2 others, was stacking a brattice when another man threw a block into work area while EE was picking another block striking EE in the right index finger resulting in an impact laceration. EE finished the shift. Went later to the Emergency Department and received 5 sutures.

Employee and 2 others were in the process of recovering belt structure. While breaking down the belt structure, they had to remove 6" aluminum water line that was hanging from the mine roof with belt chains. As employee and the other 2 employees was lifting the water line up to remove the chain from 3rd joint they were letting it down it fell and struck employee on the left ankle.

Employee stated that EE was using the canopy of the longwall shield to lift a relay bar, during the procedure the tip of the shield came in contact with a built set of cribs the tip of the canopy slipped off the cribs and came downward in a spring-type motion which caused the employee to be struck by the bottom side of the shield canopy and the top of the pan line.

The injured employee and another EE were in the process of re-banding a pallet of 30" crib block. While doing this, there was another empty pallet on top of the crib blocks, the employee attempted to pull the empty pallet off of the crib blocks and EE's left foot slipped and caused the empty pallet to fall, striking the top of EE's right foot.

Employee was removing an underground mandoor and the door inadvertently shut onto EE's left middle and ring fingers. A medical procedure and stitches were required on the finger tips.

Employee stated that they were in the process of pulling miner cable loop out from under the belt drift to rack on # 1 miner, when they threw a loop of cable on their shoulder they felt a sharp pain towards the middle of their back towards their spine.

Employee states that they were removing a brake caliper from a strut and felt pain down their leg. Employee was seen by a specialist on October 14, 2019 and was diagnosed with a protruding disc. Employee was removed from work for treatment.

Employee stated EE was pulling C/M cable in the #7 entry when EE felt a sharp burning pain in EE's lower back. Employee continued to work the remainder of the shift. On EE's way outside, the employee stated the pain got worse. EE went to the bathhouse after getting outside, and then returned to the mine office to report the incident before leaving mine property.

The employee was shoveling at six surface tail when EE twisted wrong and felt "pop" in EE's stomach. The employee was taken to CAMC, where EE was diagnosed with a strain to the abdomen. The employee was taken off work. This incident is being reported as an LTA.

The employee stated they were in the process of cleaning off the top of a spray box when their wrist got caught on a piece of expanded metal guarding, causing a laceration. The employee sought medical treatment where they received three sutures to the wrist. The incident is being reported as medical only.

Employee stated that IE was attempting to load a shuttle car when the car became hooked on the cable hook of the boom. When the shuttle car pulled away from the miner, the hook broke off the miner. IE stated IE picked up the hook to toss it against the rib and was cut by a bur on the hook.

Employee stated they were in the process of assisting the crew to prepare for a belt move on the Unit 1 Section. In the process of the employee lifting up a piece of top structure EE felt a pain in arm. EE was lifting 48" belt structure when this occurred. NOTE: This incident didn't become reportable until 10/30/2019. This is due to EE going off for surgery.

Employee stated that EE was attempting to unlatch EE's seat belt when the seat belt buckle struck EE in the mouth chipping EE's left front tooth. The employee had EE's tooth fixed the next day, but has not missed any scheduled work due to the incident.

Injured employee stated EE was operating the left Fletcher bolter on the #1 section, stated that EE was installing 10ft cable bolts in the intersection between entries 3 & 4 at #251 break, stated EE was 6ft in depth for a 10ft deep hole, stated that when bolt partner dropped pot it caused stress on EE's steel causing it to break and therefore strike EE's left pinky finger.

Employee was lifting 16' roof mats onto a scoop when they felt the left shoulder pop twice which resulted in pain and a sprain/strain injury, employee was seen by a specialist on 5/14/2019 at which time employee was taken off work. This is now being reported as a Lost Time incident.

Employee stated that as EE was attempting to raise the battery lid on the scoop, the lid slipped and caught EE's left ring finger between the frame of the battery and the battery lid. This caused a laceration and a fracture to the finger.

The employee stated EE and three other EE's were trying to raise the lid on a tailpiece at break 4 of the longwall belt when it shifted. The employee stated as they were letting it down EE's left middle finger got caught in hinge point, amputating the tip of the finger.

Employee stated was working on the company #311 10S/C Shuttle Car. Employee was in the process of installing a tie rod end on the dump end of the operator side of the machine on the Unit 2 section. In the process of lining up the tie rod end the employee felt right shoulder pop.
