Employee stated that EE was walking beside the 222 cm when EE saw a piece of the rib falling. EE stated that EE tried to get out of the way but could not and the rib fell striking EE on the left leg and pushed EE into the operator side of the cm. The rock measured 2' x 2' x 1' thick.

Employee was dismounting D11 dozer when EE's foot slipped on bottom step. Employee struck the top part of push arm with EE's side resulting with injury to the left rib area.

Employee stated was walking down the #1 entry at 33 crosscut when employee stepped in a hole and felt pain in left knee. The employee worked until 8/10/2020 when employee had surgery to repair meniscus.

The employee was in the three right crosscut roof bolting when a piece of rock fell striking the employee in the right arm, causing a laceration. The employee sought medical attention at CAMC where is received 28 sutures. The employee return to work the next scheduled shift. This incident if being reported as medical only.

Employee stated that EE was attempting to get a can of hydraulic oil from the oil sled. The cans were stacked 3 high, when EE pulled the top can off a second can fell from the 2nd row landing on the side of EE's right foot resulting in a fractured little toe.

Employee lifted and carried a rail motor coupler and felt a pain in shoulder. EE injured this same shoulder in January 2019 that resulted in loosing time from work. It was thought that this was a re-occurrence but diagnostic tests results received by the operator on 6/2/2020 indicated that a new tear in the bicep was found in a different location than before.

Employee was traveling to the surface on a mantrip after shift when the mantrip came to an abrupt stop due to a broken fish plate. Due to the stop the employee struck face on the mantrip which resulted in 5 sutures to upper lip and a fractured tooth. This is a medical reportable incident only, the employee returned to work on the next scheduled shift.

Employee was operating a Cat D8T dozer pushing coal in the clean coal pile when a cavity above the #3 feeder gave away. This caused the dozer to be covered in coal, trapping the operator inside the cab of dozer. There was no injuries to operator and rescue was successful after 2 hours.

Employee was prying up on an MBC rub rail with a slate bar, the bar slipped and they felt a pain in left shoulder. Employee continued to work and was then referred to a specialist on 3/10/2020. The orthopedic specialist diagnosed employee on 4/28/2020 with a tear on rotator cuff and took employee off work.

Employee tripped hitting EE's knee on mine floor. Employee was transported to get x-rays and they was negative. EE received a full release. Two shifts later EE went back and was referred to a specialist, on March 23rd EE was diagnosed with a torn meniscus. Due to covid-19 and furloughed we are just returning since March 20th.

Employee was struck by a hood from a 777C truck. Employee lifted hood up to work inside of motor compartment. Hood was missing a safety latch. Gust of wind blew and the hood fell over striking the employee on crown of head. Employee was transported to area hospital where the employee received 8 staples.

Employee stated that EE was in the process of removing coal that had built up on the side of EE's shuttle car to help EE's visibility. EE stated that when EE pushed the coal into the shuttle car EE's right forearm was cut on a sharp edge of the sideboard caused by wear and tear.

Employee was struck by a piece measuring approximately 5' in length X 2.5' in width X 8.5" in thickness tapering down to 0" while bolting. Incident resulted in a laceration and fracture to the employee's left hand middle finger that required surgery.

condition was damp. area was 19 feet wide and 6 feet 10 inches high. EE, and 2 others, was stacking a brattice when another man threw a block into work area while EE was picking another block striking EE in the right index finger resulting in an impact laceration. EE finished the shift. Went later to the Emergency Department and received 5 sutures.

Employee stated EE was in the process of hanging fly pads in the #4 entry, on the Unit 2 Section. In the process of EE hanging a fly pad from the 6" ladder, the ladder tipped over, and EE fell to the ground. This caused EE to land on EE's head, shoulder, and back. Note: EE was standing on the bottom 2nd step of the ladder when this incident occurred.

Employee stated that EE was using a sledgehammer to knock a stopping when the injury occurred, Employee was knocking a stopping and the employee was "choking up" on the hammer handle while swinging into a block stopping causing EE's finger to be caught between the stopping and the hammer handle causing a laceration below the knuckle on employees left index finger

While breaking down the belt structure, they had to remove 6" aluminum water line that was hanging from the mine roof with belt chains. As employee and the other 2 employees was lifting the water line up to remove the chain from 3rd joint they were letting it down it fell and struck employee on the left ankle.

Employee stated that EE was using the canopy of the longwall shield to lift a relay bar, during the procedure the tip of the shield came in contact with a built set of cribs the tip of the canopy slipped off the cribs and came downward in a spring-type motion which caused the employee to be struck by the bottom side of the shield canopy and the top of the pan line.

The injured employee and another EE were in the process of re-banding a pallet of 30" crib block. While doing this, there was another empty pallet on top of the crib blocks, the employee attempted to pull the empty pallet off of the crib blocks and EE's left foot slipped and caused the empty pallet to fall, striking the top of EE's right foot.

A roof fall at the 23 crosscut was discovered while doing the weekly with a Federal Inspector present. The fall was in the #6 entry on 2 mains. The fall measured 18'by 80'by 12' high. The fall fell on the hi-line going to the section.

Employee Stated as the owl crew were departing the motor barn on #1 rail runner mantrip. The mantrip struck the derail and wrecked. The derail was located one foot outside of the motor barn. Injured had a hand positioned at the top of the batteries. When the mantrip wrecked the batteries slid forward catching right pinky finger between the batteries and frame

Employee was using a slate bar to remove a rock from the Right CM on 006 MMU. While pushing down on the slate bar to slide the rock off the slate bar slipped out of employee's hands causing the bar to hit employee in right eye.
