Subject taking a cover off a piece of equipment to repair a hydraulic leak. The cover slipped and EE's right ring finger was pinched.
Miner was stepping down a ladder and received a Jones Fracture to EE's left foot as EE transferred EE's weight.
Removed a tee on a 2" aqua line. Line sprung back at the injured breaking / chipping a front tooth.
Employee was helping fix a shuttle car chain. The boom was being lowered and employee saw a shovel. EE reached in to retrieve the shovel and accidentally got EE's other hand pinched by the shuttle car boom causing injury.
The employee was standing on the roof bolter. While moving the hung miner cable by hand, they lost balance falling backwards hitting the drill mast, resulting in a 1-inch laceration on the right side of their neck.
Employee was changing out gas bottles and put one on a dolly. It started to fall off and employee tried to catch from falling and accidentally caused injury to fingers.
Employee was carrying 3 drill steels bound together, as they walked through a known slick area, they transitioned between solid floor and mud which caused them to slip and fall with steels in hand, they reached back with right hand to break their fall, crushing right pinky finger.
Employee was cleaning up a belt splice area and employee was lowering the rollout of the belt core. Hook came off end and the core fell which caused injury to their pinky finger
While pulling old conveyor belt off and pulling new one on the a frame pulled loose landing on employee's foot, this caused two metatarsal fractures.
Employees were loading a butterfly valve onto an underground truck. The valve shifted and accidentally rolled onto employee's hand that was on the bed of the truck. Employee pulled EE's hand out from underneath the valve causing injury to two fingers.
While pulling a skid out of crosscut EE noticed a bolt that needed to be moved and as EE attempted to move it the skid hit EE's right foot.
While troubleshooting fan shroud on equipment, Employees thumb was struck by fan.
Once underground employee got out of mantrip to check equipment. When getting back in EE shut their thumb in the door.
EE was roof bolting mesh to the roof as EE was raising the ATRS to the roof EE noticed that the mesh needed to be adjusted. EE lowered the mesh and was adjusting the mesh when it kicked upwards causing EE's finger to be caught between the mesh and the mat pulling EE's middle finger nail back.
Unloading Richwood impact clusters off the cookie sheet of an M2 shield hauler. Unloaded two cluster without incident where one man would push and the other would pull. The injured pulled the third cluster into a stationary cluster, smashing EE's finger between the two. EE sustained a small fracture in the tip of EE's right middle finger. The wound required 8 sutures to close.
Employee was pouring drain cleaner and it came out faster than expected and accidentally splashed on EE's arm and then running down under EE's glove causing injury to arm. Employee did not miss work until Dec 26 when EE received additional medical treatment for EE's burn and then was put off work.
Employee was fusing pipe and trying to line up pipe with fuser. The pipe slid forward and accidentally caused employee's finger to get caught between two pipes causing injury
Employee was throwing blocks and smashed EE's finger between two blocks causing injury to EE's finger. Note: This employee works a 4-10 (4 days a week) schedule and this MSHA 7000-1 was submitted within 10 working days of the incident.
IE and co-worker were hanging tubes in #1 entry of the 7 North 3 Left Tailgate, they had hung the tube and IE was holding it up while co-worker was fixing the chains and hook. They switched positions and when they did, the tube rolled causing a latch back to come loose and catching IE's nose which caused a laceration.
Two EE's were retrieving their bolting steels from the Kitchen toolbox. One EE opened the lid and was holding it opened as EE moved towards EE, EE moved around EE, due to someone standing up the table shifted, and the lid swung down striking EE on the right pinky finger resulting in the injury.
Surface Miner was trying to close a damaged roll up door. The door was tweaked so EE was trying to raise one side in order for door to be level. The door dropped onto a crib EE had placed under the door. EE's thumb was caught and was fractured. No surgery was required.
EEs were taking down rib support in 1L7N XC-20. EEs were taking out the X member piece. EE was loosening off the last bolt and when it finally broke loose, EE's body shifted bumping the X piece knocking it off the bolt and causing it to swivel downward catching EE's thumb on left hand in-between the two pieces of metal.
Employee was cleaning and working on beltline and smashed a finger while handling a belt roller.
While EE was performing routine maintenance on the traction planetary on CM-111 EE felt a pop in EE's elbow on EE's right arm.
While EE was helping to install a high voltage nip into the power center on the longwall, they were holding the nip and when the other EE repositioned to plug in the nip EE felt a pain in their right shoulder.
EE finished drilling outside hole and top steel got stuck. EE used 4' steel to try to get it out. EE was holding down the mat with their elbow to connect the two steels together to pull the stuck one out. While pulling the steel down the mat slipped from under their elbow, springing up and cutting through their long sleeve shirt causing a laceration on right forearm.
While bolting in 7 North Mains #1 entry the employee was adjusting the mesh on the ATRS EE's right arm got caught on the mat which caused it to tear EE's rain jacket and long sleeve shirt causing a laceration on EE's arm by the elbow.
Employee was throwing a crib behind a steel set leg and pinched right hand pinkie finger between the crib and the steel set leg. Employee was wearing cut resistant gloves. This incident resulted in a laceration to tip of finger and loss of nail.
EE was hand unloading face conveyor flyghts that were brought in to section in back of dodge truck. As EE was putting flyghts in jaws of the can setter, their index finger was pinched. This caused a fracture to the tip of the finger.
EE had placed a crib under a shield to be able to unload it from the cookie sheet on the 650, EE placed the crib under the shield it needed to be repositioned, so EE used left foot to push the crib into position while pushing on the crib, EE overexerted right leg causing the right knee to be displaced.
EE was hanging a string cutter on 4 East #2 belt. The string cutter consisted of metal tubing. The EE first inserted the off-walkway side into the bracket and was holding the walkway side in place with index finger on top of the tubing. The off-walkway side slipped out of the bracket, fell down resulting in EE's index finger being pinched between the tubing and the bracket.
A plugged chute switch shut the plant feed belt off. An individual reported to the chute. EE saw 6" - 18" rock plugging the grizzly in the chute. EE attempted to dislodge one of the rocks with hand. Another rock dislodged dropping onto EE's hand smashing the right hand, little finger between the two rocks. The outside edge of EE's little finger was lacerated requiring stitches.
Employees were removing a Kennedy Stopping with a mandoor. Injured employee was holding the mandoor with right hand as the clips on the cross bars were being removed. When the clips on the panel next to the door were removed, the door fell to the side and cut the palm of the EE hand. EE was wearing cut resistant gloves and still received a cut that required stitches.
Bolter operator was pushing the end of a mat up to wire it in place so a bolt could be installed. Operator was pushing up with their head while wiring. The mat slipped off operator's hardhat and fell striking the operator. This caused a 1 1/2 inch laceration that required 3 stitches.
EE was doing a pre-op and cleaning the equipment, there were (2) five-gallon buckets of oil on the loader. EE grabbed one of the buckets to take it off the loader and when EE stepped off the loader the bucket handle came out of the bucket and slipped through EE's hand hooking EE's glove and puncturing right ring finger causing a laceration on the finger.
While unloading a pump out of the back of a pickup truck, the pump slid to the side. The tip of the miner's middle right hand finger was pinched. This resulted in 8 stitches.
Employee was in the process of installing the kickstand on the sloughing plate of the shear. Another employee had started to pull a staple to repair a leaking fitting. The employees were reminded to get the plate blocked. Work on the fitting stopped. Sloughing plate come down pinning the employee. The pinned employee received 2 staples in the back of EE's head.
EE was replacing a scoop push plate ram. Two miners were assisting employee. As the connection pin was being hit through the block and into the ram eye, the injured EE stuck a finger in the block hole to see if it was lined up. At that point the eyelet dropped a couple inches and the tip of the finger was pinched. This resulted in 11 stitches and the tip of finger fractured.
EE bent over to pick up one end of a belt roll hanging bar. EE barely had it off the ground when they heard a pop in shoulder and pain down right arm. It is determined by Specialist that Bicep Tendon Tore off. First day of missing work is 01/11/2021.
While finishing a splice on Pines East #2 belt, EE was folding the 60" belt back over to line it up to put a pin in. EE was pushing the belt alone when the belt was caught on the bottom of the structure, at that time EE's left shoulder was dislocated.
Employee was throwing mesh on bolter and felt pain in shoulder
Two employees were pushing a 150 hp pump to the back of the bed of a dodge truck to unload it. The pump was top heavy and it tipped over and pinched the pinkie finger of the employee.
While employee was pulling the cable out of the throat of the Feeder Breaker, employee had a foot pushing off a flight to pull the cable when they felt something in left foot and when they applied body weight to it, it began to hurt.
EE was changing bits on the Headgate drum, when EE turned to walk out of the pan, EE's right patella popped out of the patellar groove of the knee on EE's right leg.
EE tossed a bottom roller into the bed of a dodge truck. The roller hit already loaded rollers and bounced back at EE. The tip of EE's left hand ring finger was pinched between two rollers, resulting in a crack in the tip of EE's finger.
Employee was operating a 834 RTD, when they stopped and exited the machine, employee was in the process of closing the operators cabin door when they inadvertently had fingers between the door and door frame, smashing the middle finger on left hand.
An employee was installing a hose wrap on the hydraulic hoses up on the drill booms of a roof bolter. In the process employee's right pinky finger hyperextended and employee felt a pop. It was later discovered the employee has a torn ligament.
Employee was pulling cable and felt pain in mid-section. Employee did not miss work until 8/7/2020 when employee got surgery.
Employee was hanging vent tube and it spun while trying to install a new piece. The tub fell and a piece of tube broke and cut employee's arms causing injury.
