Removed a tee on a 2"" aqua line. Line sprung back at the injured breaking / chipping a front tooth.

Bolter operator drilling roof. When adding pusher steel, stumbled / slipped and put uncontrolled feed pressure up while right hand was still around the steels EE was marrying. This caused the steels to bend toward the mast, pinching EE's right ring finger between the steels and the feed cylinder. This caused a skin avulsion roughly 3/4"" x 3/4"" at the tip of the finger.

Employee was carrying 3 drill steels bound together, as they walked through a known slick area, they transitioned between solid floor and mud which caused them to slip and fall with steels in hand, they reached back with right hand to break their fall, crushing right pinky finger.

Tasked with pre-use inspection of a high voltage switch gear. Injured noticed dust on the receptacle and went to wipe with a rag. A capacitive discharge entered EE's hand and body. This resulted in a small burn to EE's left and where the discharge exited and muscle soreness in forearms, shoulder, back.

Employee was cleaning up a belt splice area and employee was lowering the rollout of the belt core. Hook came off end and the core fell which caused injury to their pinky finger

While building a Kennedy overcast, injured was kneeling on the solid, left for overcast crossing, over a 1-2' gap above the completed overcast wall below. The injured lost balance and slid between the gap. Injured stuck left arm out to brace against impact, catching the Kennedy decking before sliding approximately 6' between the wall and solid ground below.

Unloading Richwood impact clusters off the cookie sheet of an M2 shield hauler. Unloaded two cluster without incident where one man would push and the other would pull. The injured pulled the third cluster into a stationary cluster, smashing EE's finger between the two. EE sustained a small fracture in the tip of EE's right middle finger. The wound required 8 sutures to close.

EE was hanging line brattice in xc20 between #4 and #5 entry with a insulated hook and when EE lowered EE's hand the end of the hook had caught EE's glove tearing into the back of EE's left hand, between EE's index finger and thumb

Employee was operating the left side of roof bolter, when the canopy got hung up in the roof mesh, they tried lowering the canopy and it would not come down, then they tried lowering the whole boom and it broke loose abruptly striking their hard hat/head against the canopy.

Crew was walking out of the mine. Employee, without warning, fell to the ground. EE was immediately checked, and no pulse was found. CPR was initiated without success. Internal investigation has determined that this was a personal medical condition.

EE WAS SITTING IN A CHAIR EATING HIS LUNCH WHEN SOMEONE HIT THE FLOOR UNDER HIM CAUSING HIM TO JUMP OUTOF THE CHAIR & FALL TO THE FLOOR

THE EE WAS HELPING LOAD SOME MATERIAL ON A SCOOP. HE SIGNALED THE SCOOP OPERATOR TO STOP BUT THE OPERATOR DIDN'T SEE HIM SIGNAL AND KEPT COMING IN AN CAUGHT THE EE FOOT BETWEEN THE DUCK BILL A ND THE FLOOR.

EE WAS DROPPING HIGH VOLTAGE CABLE DOWN. WHEN HERELEASED IT FROM THE HANGER, IT SLIPPED AND FELLSTRIKING HIM ON THE CHIN.

EE WAS WALKING DOWN THE FACE WHEN A BOUNCE OCCURRED CAUSING COAL TO BE THROWN STRIKING THE EE ON THE RIBS.

THE EE WAS HELPING REMOVE MONO-RAIL WHEN A BOUNCED OCCURRED CAUSING HIM TO JERK, WHILE HOLDING A PIECE OF MONO-RAIL OVER HIS HEAD, HURTING HIS BACK.

THE EE WAS IN THE PROCESS OF INSTALLING A COUNTER BALANCE ON A SHAKER. THEY HAD THE COUNTER BALANCE RAISED ABOUT E WHEN THE ANCHOR BROKE DROPPING THE BALANCE TO THE FLOOR CATCHING THE EE'S FOO T BETWEEN THE BALANCE AND THE FLOOR.

THE MINER PARTICIPATED IN A MED EXAMINATION & ITWAS DETERMINED THAT HE HAD A SUFFICIENT DEGREE OF PNEUMOCONIOSIS

EE WAS USING A WASH DOWN HOSE TO CLEAN A SCOOP, THE HOSE SLIPPED OUT OF HIS HAND AND STRUCK HIM ON THE UPPER LIP.

EE WAS IN THE PROCESS OF COUPLING A WATER LINE TOGETHER WHEN HE PINCHED HIS FINGER BETWEEN THE PIPE AND COUPLING.

AT APPROX. 2130, A BOUNCE OCCURRED ON THE TAILGATE OF THE LONGWALL. THIS BOUNCE BROKE A TORQUE SHAFT ON THE SHEARER AND HEAVE THE PAN UP TO A POINT THAT WE COULD NOT GET TO THE SHAFT TO CHANGE IT OUT WHICH INTERRUPTED NORMAL PRODUCTION FOR OVER ONE HOUR.

EMPLOYEE WAS PULLING IN A SHIELD WHEN A PIECE OF COAL FELL FROM BETWEEN 2 SHIELDS AND STRUCK HIM ON THE FOOT CAUSING A FRACTURE.

EE WAS LIFTING A 60"" BELT ROLLER WHEN IT SLIPPED AND STRUCK HIM ON THE KNEE.

EMPLOYEE WAS CROSSING AN OVERCAST GOING DOWN THESTEPS WHEN HE TRIPPED & FELL CAUSING A FRACTURE TO HIS LEG.

EE & HIS PARTNER WERE MOVING THE ROOFBOLTER HIS PARTNER ROTATED THE MAST FOR CLEARANCE & HIS FINGERS WERE PINCH BY A STEEL THAT BECAME BOUND UP.THE INJ EE WAS HELPING FREE HIS PARTNERS' FINGER S WHEN THE STEEL FLIPPED UP STRIKING HIM IN HIS CHEST.

EMPLOYEE WAS IN THE PROCESS OF PULLING THE TRAILING SHIELDS DURING A LONGWALL MOVE WHEN THE ROOF COLLAPSED AND A PIECE OF ROCK STRUCK HIM ON THE LEG CAUSIN G AFRACTURE OF THE FIBULA. THIS CA VE WAS ABOUT 20'LX10'WX8'H.

EMPLOYEE WAS INSTALLING A POWER SUPPLY FOR LIGHTS ON SHIELDS, WHEN LIFTING IT UP HE HURT HIS BACK.

THE EE WAS TRYING TO PUT A ROPE CLAMP ON. HE WAS HOLDIN THE 2 ROPES TOGETHER WHEN A ROCK FELL FROM THE ROOF AND CAUGHT HIS FINGER BETWEEN THE ROPE & THE ROCK CAUSING A LACERATION.

EMPLOYEE WAS DRAGGING A HOSE BETWEEN THE RIB AND STAGE LOADER WHEN A ROCK SLID OFF THE RIB LAGGING AND SMASHED HIS FOOT BETWEEN THE DRIVE FRAME AND ROCK.

ON THE AFTERNOON SHIFT THE SHEARER WAS CUTTING INTO THE TAILGATE WHEN AN OUTBURST OCCURRED FILLING THE PANLINE FULL OF COAL AND ROCK FROM SHIELD 103 TO 116 CAUSING THE PANLINE TO STALL. THE OU TBURST DID NOT CAUSE ANY DAMAGE.

THE EE WAS OPERATING THE SHEARER AND WAS STRUCK BY A PIECE OF COAL ON THE SHOULDER.

REPAIRS WERE BEING MADE TO A LIFT JACK BRACKET ON THE SLOUGHING PLATE OF THE SHEARER. DURING THE WELDING OF THIS BRACKET A COUPLE OF SMALL METHANE FEEDERS WERE IGNITED AND EXTINGUISHED WITH A WASH DOWN HOSE.

AN EE WAS MAKING THE PUMPS IN THE BLEEDER ENTRIED OF THE LONGWALL AND DISCOVERED THAT A CAVE HAD OCCURRED 100' INBY THE 7TH RIGHT ENTRIES. THIS CAVE WAS IN A X-CUT THAT WAS USED AS A WALKWAY, BUT THESE ENTRIES ARE COMMON AND YOU COULD TRAVEL EITHER WAY. THIS CAVE WAS ABOUT 30' WIDE BUT YOU COULD NOT TELL HOW HIGH OR HOW LONG.

THE EMPLOYEE WAS WORKING ON THE SHEARER. HE WAS BETWEEN THE SHEARER AND THE FACE WHEN A BOUNCE OCCURED IN WHICH HE WAS STRUCK BY COAL ON THE BACK AND PUSHED INOT THE SHEARER WHICH FRACTURED 3 RIBS.

EMPLOYEE HAS DEVELOPED CARPAL TUNNEL BY USING HIS HANDS OVER A PERIOD OF TIME.

EMPLOYEE WAS HELPING ANOTHER EMPLOYEE REPAIR A BELT TRANSFER CHUTE. THEY WERE DOING A LOT OF CUTTING AND WELDING DURING THE SHIFT AND THE EMPLOYEE BURNED HIS EYES.

THE EE WAS INSTALLING HOSES ON THE SHIELDS OF A LONGWALL, HE SLIPPED ON THE TOES OF THE SHIELDS AND TWISTED HIS KNEE.

THE EE STOPPED THE EQUIPMENT HE WAS OPERATING, GOT OUT AND WAS WALKING TO USE A TELEPHONE WHEN HE STRUCK HIS HEAD AGAINST A PLASTIC WATER PIPE.

EE WAS LIFTING THE BELT HARDWARE WITH SOME COMALONGS, HE LOOKED UP AND GOT SOMETHING IN HIS EYE.

EE WAS PULLING ON A WATER HOSE WHEN THE VALVE OPENED CAUSING THE HOSE TO FLIP UP AND STRIKE HIM ON THE FACE.

EMPLOYEE WAS CLEANING TRASH OUT OF FEEDER BREAKER WHEN HE SLIPPED AND FELL HURTING HIS ARM AND SHOULDER.

THE EMPLOYEE WAS SPOTTING SHIELDS BETWEEN 40 & 50 IN AN AREA WHERE THE FACE WAS HIGH. WHILE PULLING AND SETTING THE SHIELDS A BOUNCE OCCURED AND THREW THE BRETBY OUT OF THE PAN AND THIS STRUC K THE EMPLOYEE IN THE RIBS. THE SHEARER WAS AT THE HEADGATE.
