Employee woke up on 10/11/2016 with pain in left wrist. EE thought it might be from changing screens the previous day because it was a lot of repetitive movement. EE continued to have pain through out the week and went to the doctor on 10/14/16. EE was told EE had tenosynovitis and was put on restrictions.

Showing all 2Employee was pulling wires for a sprinkler system when EE felt pain in EE's shoulder back in May of 2016. EE continued to have pain over the next few months and went to EE's doctor on 9/13/16. EE had a follow up appointment on 9/27/16 where EE was told EE had a torn tendon/ligament and would need to see a specialist. EE notified EE's supervisor on 9/28/16 about this injury.
