Fall from machine

EE was operating forklift to move hydraulic hoses. The fork lift arm was extended up with the hoses when the back tires of the lift came up. The EE quickly tried to lower the lift and then the lift started to tip over. The EE end up falling out of the lift and landing on the ground. The EE went to the doctor and was fine but went back two days later because EE felt sore.

Struck against a moving object

*****did not go for treatment until - 9/24/2019*** Riding in the passenger seat of a bulk truck, truck started jumping attempting to get up a short ramp in the sand, thrown up twice in the seat, legs became sore.

Fall to the walkway or working surface

Employee was stepping off of the track mobile up at the load out building while turning to step away employee's left foot slid out on the rocks causing employee's right leg to twist and buckle. EE fell on right knee with all employee's weight. EE went to doctor the following morning as the pain got worse. EE was put on restrictions for knee pain.

Employee woke up on 10/11/2016 with pain in left wrist. EE thought it might be from changing screens the previous day because it was a lot of repetitive movement. EE continued to have pain through out the week and went to the doctor on 10/14/16. EE was told EE had tenosynovitis and was put on restrictions.

Employee was shoveling to clean under the conveyor and hit hand and cut right thumb on the sharp edge of the conveyor. The cut required 5 stitches.

Over-exertion in pulling or pushing objects

Employee was pulling wires for a sprinkler system when EE felt pain in EE's shoulder back in May of 2016. EE continued to have pain over the next few months and went to EE's doctor on 9/13/16. EE had a follow up appointment on 9/27/16 where EE was told EE had a torn tendon/ligament and would need to see a specialist. EE notified EE's supervisor on 9/28/16 about this injury.

Employee climbed down from a piece of equipment and was walking towards another piece of equipment. While walking employee started to slide on gravel and lose his balance. Employee twisted his torso to regain his balance and felt a pop on his right side just above his waist. Employee felt immediate pain in hip and numbness on the lower left side of his torso just above his hip.

Employee was on the ground walking when he stepped on/or tripped over a rock and lost his balance. He reached out his arm to grab a pipe and catch his fall.

Constructing dry-plant at new sand-mine location. Employee walking across a concrete slab he was looking up while walking attempting to get the attention of co-worker in a boom-lift. While walking he tripped over a piece of rubber conveyor belting that he personally had placed in its location a few minutes prior. He fell and sprained his elbow.
