I.E. Stated EE was helping put up a waffle beam across the road when the helper lost grip of the beam dropping it smashing two fingers on left hand between beam and tractor. X-Ray showed 2 broken fingers and EE received 18 stitches.

When the stored energy was released the anti-toppler hit employee in left arm. Employee went to hospital where x-rays were taken with no broken bones. Employee is off with a sore arm.
IE was moving oil cans to a pallet when EE pulled a muscle in EE's back.

I.E. was helping carry water pipe when the person who was carrying the other end suddenly dropped their end causing I.E. to drop their end of the pipe on their foot.
I.E. was dropping miner cable that was hung across the entry. When they cut the rope hanger loose with right hand they caught the cable with their left hand. They then felt a little pain in shoulder but continued to finish the shift. The next morning they woke up with a burning sensation in their shoulder.

I.E. was putting in a rail for the top slope belt when they slipped and fell face first hitting their face on the bottom belt.

I.E. was recovering belt structure when they felt a pop and pain in right shoulder.
I.E was building cribs in HG 17 bleeder near a regulator and felt something get blown into their eye.
I.E. stated they were dragging a tube from P.C. area to 2 left when they felt a pull in left shoulder. I.E. took themselves to the hospital for C.T. scan, waiting on results.
I.E. was attempting to move a top roller and a roller came loose and pinched EE's small finger.
I.E. was helping set a beam on top of a shield during recovery. The beam became hung on the mesh while employee was trying to pull it over and felt something pull in their bicep.
I.E. was stepping up onto the back of the bolter carrying supplies when they felt a pop in their right knee

I.E. stated EE was setting up to weld and cut on miner head, Employee set oxygen bottle up and when picking up acetylene bottle the oxygen fell smashing EE's left thumb between the 2 bottles. X-ray showed displaced fracture.
I.E. was loading structure onto a structure trailer when EE mashed EE's finger between the trailer and a roller causing a laceration.
I.E. was stepping up onto the roof bolter carrying a bundle of plates when I.E. felt a burning sensation in stomach area. I.E. was diagnosed with a hernia requiring surgery.

Allegedly a rock fell out of the face in entry #4 and hit IE in the shoulder knocking IE to the ground. IE landed on a rock and injured their knee.
I.E. stated EE was washing out the radiator of a Bobcat and the radiator cap blew off spraying EE's left arm with anti-freeze causing 2nd degree burns.
I.E. stated EE was taking belt structure apart when EE smashed EE's finger between top roller frame and belt rail. I.E. received 4 stitches in EE's left ring finger with follow up with Orthopedic doctor.
I.E. was dragging a cable when the EE in front of IE dropped the cable unexpectedly causing I.E.'s shoulder to be dislocated.

IE was standing next to a piece of plastic pipe so it would not hit a truck that was by the slope drive. The pipe got caught on something in the slope and when it broke free the pipe hit swept IE off of EE's feet. IE received 6 sutures on EE's bottom lip.
IE was lifting a 4" slick line up to hand it when EE heard EE's shoulder pop. IE never missed a day of work till 10/28/2024
I.E. stated they were hanging chains for a belt move when I.E. felt a strain in their neck. Employee went to Hospital where it was determined to be a strained muscle, employee was prescribed a muscle relaxer and returned to work, no lost time.
I.E. was loading the bolter and smashed a finger between a metal cover and the frame. I.E. was diagnosed with a broken middle finger.
I.E. stated EE was changing a top roller on A-slope when the roller slid smashing EE's left ring finger. I.E. went to franklin hospital where EE received 3 stitches and x-ray showed tip of finger was fractured.
