Showing all 10Employee was told to pull a loose rib that had timbers set in front of it. When employee pulled the loose rib, the material dislodged a timber striking employee in the head.
Installing torque arms on miner felt pain in stomach. Employee filled out accident form but refused medical attention, and continued work until 11/10/11. Employee had surgery on 1/11/11.
EE was hanging miner cable. When the miner cable started to be pulled, the hook on a rope hanger caught the web of his hand between his thumb and his index finger against the cable causing a cut/laceration on his right hand. Required 6 stitches.
Inby air-lock door exiting mine was not operating properly. Crew tried four times raising and lowering door, after failing to get door to raise higher than 3', EE tried to manually assist door. EE was struck by garage door when man trip began tramming forward with no warning. EE had to be treated for a laceration on his chin, approx 3"L X 1/4"deep.
While making repairs to the miner, He lifted a rub rail to replace it and in doing so he strained his back.

After repairs to miner had been completed, EE was assisting in the replacement of the rubrail. During the procedure, the rubrail slipped & smashed his right middle finger above the last knuckle against the frame of the machine.
