Employee was told to pull a loose rib that had timbers set in front of it. When employee pulled the loose rib, the material dislodged a timber striking employee in the head.

Caught in, under or between a moving and a stationary object

Employee placed his left hand on the outside of the canopy of the scoop that he was operating. The scoop went into a mud hole and slid into a rut smashing his left hand between rib and scoop canopy. After surgery, Dr. said that he would lose 1/2 of his left ring finger and tip of his middle finger.

Installing torque arms on miner felt pain in stomach. Employee filled out accident form but refused medical attention, and continued work until 11/10/11. Employee had surgery on 1/11/11.

Roof fall was found at 9:10pm in the return on 1 left panel in 20 south mains at break # 41. Fall started in the intersection of #7 entry and went approximately 150' thru the intersection in # 9 entry from spad 2245 to spad 1166. This area was mined around the first of June. Area was dangered off.

EE was checking a pump at 1 west mains when he discovered a fall at 37 brk entry 7,8,9 in the xcuts. Roof fall is inby spad # 1271, # 9 entry. Roof fall is approx. 100' long X 19' wide X 8' to 9' thick. Fall has been dangered off.

Foreman as walking the intake airway when a fall was dicovered @ 8:55 am. Location was break 15 #7 heading spad # 324. Fall was 65' long x 18 1/2' wide x 4' thick. Area was dangered off and cribs set at all entrance points. 5' torque tension bolts were installed previously in this area. Falling rock pulled some of the bolts from their anchorage point.

Roof fall was at #3 x-cut on 2 left west mains. Fall was 18'w x18'L x 8' to 9' thick. Two cribs were set at all approaches to fall. Area had been bolted with 5' torque tension bolts. and spotted with 8' bolts. Fall was in neutral 1st x-cut outby spad # 339. Area was last mined October of 2009.

EE was hanging miner cable. When the miner cable started to be pulled, the hook on a rope hanger caught the web of his hand between his thumb and his index finger against the cable causing a cut/laceration on his right hand. Required 6 stitches.

Individual was installing roof bolts when he was struck on the left wrist by a piece of drill steel. He suffered a nondisplaced hairline fracture to his left wrist. He did not cease work to his left wrist. He did not cease work and was treated later and placed in a wrist splint.

Roof fall at No 5 entry between 15 & 16 x-cut on 2 south mains. Fall was 20' wide X 50' long X 5'-9' thick. Cleaned up fall and rebolted area. 10 day reporting window extended due to mine holiday and vacation.

Roof fall at No. 11 & 12 x-cut, No. 1 entry of 1st left panel off of 1 west mains. fall was 20'W x 70'L x 3'-6' Thick. Cribbed and dangered off area, will not be cleaned up. Area bolted with 5' torque tension 3/4"" roof bolts. Immediate roof was shale with laminated sandstone above it. Fall inby loading point.

Roof fall at No. 11 x-cut, No. 9 entry of 1 west mains. Fall was 20' W x 35' L x 3' - 5' thick. Cribbed and dangered off. Will not clean up. Area bolted with 5' torque tension 3/4"" roof bolts. Immediate roof was shale with laminated sandstone above it.

Fall onto or against objects

EE was hanging a water line when he slipped and struck his head on the rib. EE suffered a laceration to the right side of his head, which required 4 sutures.

EE states he was in a squatting position grinding metal, when he felt back pain and hip pain when standing. EE sought medical treatment on 7/2/09 and was taken off work by his treating physician.

The employee was unhooking a piece of pipe connected to the bucket of an excavator when the operator engaged the emergency stop lever so he could move the bucket. The operator didn't realize his leg was depressing the bucket joystick. The bucket dropped onto the employee's left hand, pinching his fingers. Surgery was performed & ee is off work.

Struck against a moving object

Employee states that he was tramming up an incline with a beam on the forks of a forklift when the forklift stopped running causing brakes, steering & hydraulics to fail. The forklift rolled backwards, caught traction with tires & proceeded to roll over on it's top. Employee suffered head contusion.

Employee states he felt low back pain while he was unloading a truck bed.

Fall from ladders

EE was assisting in the installation of a jib on a 90ton crane, while standing on a ladder, the jib fell off the crane and onto the ladder, knocking the ladder over and the employee fell to the ground, suffering a contusion to his lower back and tailbone.

Over-exertion in lifting objects

Employee states he was getting steel beams ready for crane lift and felt low back pain/left leg pain when he bent over and lifted end of beam. He delayed seeking medical attention to see if pain would ease alone. Condition did not improve and treatment was sought 10/24/08.
