Employee was working on carrier feed hopper guard. While another employee was using grinder to remove the guard. Employee pulled and guard popped "snapped" out striking employee and forehead causing skin to be open at point of impact.

Showing all 13While on a ladder beneath double dump on bag house 3, cleaning with one hand, employee suddenly felt a sharp pain around mid back region. Medical treatment diagnosed with thoracic muscle spasm, given 2 days off work, and light duty restrictions.
Employee used leverage, via pushing by hand, to break a 4.5" PVC pipe, upon breaking the pipe flipped up and struck the employee in the mouth causing a laceration on the lower lip.
EE was entering the skidsteer when EE's phone fell out of EE's pocket. It fell on the bucket step into a corner. EE sat down and went to pick it up quickly and when EE pulled hand up noticed the left index finger was bleeding. When EE flexed the finger EE noticed the severity of the cut and it started bleeding profusely.
Employee was opening the lid on a pneumatic truck. The lid on the handle was tight and employee exerted a good amount of force to get it to open and felt something pull in shoulder muscle. Employee called and said employee wanted to take pain medication and see if it felt better. After about an hour the pain was not getting better and we decided to take employee to get checked out.
Employee got sand in EE's right eye while performing housekeeping duties. Was treated onsite with first aid measures, then taken to an offsite facility for evaluation where EE received an eye drop prescription.
Employee was cleaning the oversize protection on BE306, as they were putting the screen back in place it caught on something, and hit employee's lip, causing them to cut their lip
Employee was replacing the guarding on the top of BE307 and when they picked up a 5 gallon bucket and turned to walk back to the man lift. Employee stepped on a race from one of the bearings that had been replaced and fell hitting right leg on some guarding.
Employee was installing a new belt on the 410 conveyor. Employee was manually rolling the gear box. While doing so they pinched the tip of left middle finger between the drive belt & pulley.
On 11/29/2020 Load-out operator was opening hatches on trucks and felt something pull on EE's left side. The operator stated EE did not need medical care at the time of the incident. On 12/7/2020 the operator decided to seek medical care and I met EE on 12/8/2020 at the clinic. The doctor took x-rays and the operator had 1 fractured rib
While shoveling accumulated sand at the base of the Primary A hopper, employee slipped and fell backwards striking back of head on steel beam. All PPE was in place. Hard Hat flew of as slip occurred. Laceration to back of head.
Employee received a small puncture and laceration to the 4th and 5th fingers of the left hand while touching a piece of metal with bare hands.
Employee was struck in the eyelid by a metal shard which was created by striking metal on metal. Employee was prescribed a medication for the eye to prevent injury. Returned to work with no restrictions.
