Employee was stung by a bee on EE's right hand

Employee was working on carrier feed hopper guard. While another employee was using grinder to remove the guard. Employee pulled and guard popped ""snapped"" out striking employee and forehead causing skin to be open at point of impact.

Struck against stationary object

Employee was bending a damaged guard with a hammer when the guard shifted and the employee struck their hand against the edge. This caused a laceration on their right index finger requiring 3 stitches. No restrictions were put in place.

Employee was repairing leaks on baghouse & valve separated from wall and struck EE in the face. EE was driven to ER for medical attention by co-worker.

Fall onto or against objects

Loadout operator was on top of a box truck closing hatches. The top of the box had a thin layer of sand causing the EE to slip and stumble backwards, tripping over the small rail on the edge of the box causing EE to fall over the side and between the the 2 boxes on the trailer and hit the bridge of nose on the second box which was directly behind EE.

Fall onto or against objects

Belt press attendant was cleaning a floc mix bowl, after flushing out the bowl EE went hang the hose up and slipped backwards due to floc on the ground. EE extended EE's hand out to catch fall & got a cut from a metal grading step. After EE fell, we asked EE if EE was okay, EE said ""yes"" we helped EE clean the cut on EE's hand after that EE was taken for more medical attention.

There was light rain and the employee did not fall, but slipped while walking to the fuel station and felt pain in EE's left knee.

Fall to the walkway or working surface

Employee stepped into an area where there was some ""FLOC"" residue causing the concrete slab to be slippery. Employee slipped on EE's left ankle and fell to EE's right side landing mostly on EE's right shoulder then right hip.

Struck against a moving object

Employee operating skid steer when it suddenly pulled hard to one side causing employee to hit their head on side of window and later experiencing neck pain. Employee did not report incident and went to ER on their own.

Rubbed or abraded

Blisters on feet due to friction inside of boots. Blisters infected - Cellulitis.

Fall onto or against objects

Employee opening lid hatch for Trailers. Employee opened double hatch lids and was attempting to cross back to the platform and stepped into the open space of box with left foot and fell onto cross member of Box; employee sustained a bruised right knee, left elbow & perineum area of buttocks. Initially, was reported as a first aid injury only.

Struck against stationary object

MPCS division independent contractor was installing floor grating. EE had cut a piece off and returned to cut more and stepped onto a piece of metal buried in the sand. The pointed sharp edge punctured the boot and caused a laceration under the left big toe.

Caught in, under or between a moving and a stationary object

EE was helping assemble an excavator. Attempting to attach arm to boom with a steel pin (273.25 lbs.). Pin was held in place with a nylon strap. Pieces were misaligned. The EEs began to remove the pin and pieces shifted, causing the injured EE's finger to be pinched between the pieces, causing soft tissue injury. EE was treated for degloving of the distal phalanx of fifth finger.
