Employee's were installing a new auger in a wash plant trying to align bolt holes in the two flange assembly. The employee put the tip of EE's finger in the bolt hole to align up the two holes. One of the two flange pieces slid down and lacerated the tip of EE's right middle finger.

EE was working around the assembly of the wash plant installing a leg that fell over onto EE's leg, pinning it underneath. Ultimately breaking EE's leg. Two coworkers assisted in picking up the metal leg off of EE's leg and pulled EE out from under it. EE was taken by a coworker to St. Francis hospital.
